   E38-127.  Encroachment. Whitmoyer Laboratories, 99 Creamery Rd., Myerstown, PA 17067. To perform remedial excavation to bedrock for removal of onsite contaminated soils/sediments, backfilling of all excavated areas, and placement of a clean soil cover over onsite areas located within the floodway of the Tulpehocken Creek (TSF) and the Union Canal. Project is located at the Whitmoyer Laboratories Superfund Site located between Creamery Street and Fairland Avenue (Richland, PA Quadrangle N:  18.9 inches; W:  10.6 inches) in Jackson Township, Lebanon County.

   E44-097.  Encroachment. Kistler Borough, 94 Cedar St., Mount Union, PA 17066. To construct and maintain approximately 375 feet of 6-inch diameter sanitary sewer force main pipe across the Juniata River in conjunction with the development of the Kistler Borough Wastewater Collection Facilities Project located at Kistler Borough (Newton Hamilton, PA Quadrangle N:  1.0 inch; W:  16.3 inches) in Wayne Township, Mifflin County and Mount Union Borough, Huntingdon County.

   E12-116.  Encroachment. Mid-Cameron Municipal Authority, 421 North Broad Street, Emporium, PA 15834. To construct and maintain a gravity sanitary sewer line (mostly 8 inches in diameter) across West Creek (HQ-CWF), a side channel of West Creek (HQ-CWF) at three locations, Day Run (HQ-CWF) at two locations, Thayer Run (HQ-CWF), Lyons Run (HQ-CWF) at two locations, Towner Run (HQ-CWF) at two locations and across ten wetlands. The sewer line begins at the western end of the Borough of Emporium (Emporium, PA Quadrangle N:  1.7 inches; W:  16.5 inches) and extends west along Route 120 to a location approximately 3 miles from the Borough (West Creek, PA Quadrangle N:  21.1 inches; W:  6.4 inches) in the Borough of Emporium and Shippen Township, Cameron County. Total estimated stream disturbance is 300 feet and total wetland impact is 0.53 acre.

   E03-382.  Encroachment. PA Department of Transportation, Engineering District 10-0, P. O. Box 429, Indiana, PA 15701. To remove the existing bridge and to construct and maintain a prestressed concrete I-beam bridge having a single normal span of 43.5 meters and an underclearance of 5.7 meters over Crooked Creek (WWF). The applicant also proposes the construction and maintenance of a temporary causeway in Crooked Creek (WWF) for the purpose of removing the existing piers. The project is located along S. R. 156, Segment 0362, Offset 2777, Station 7+009.000m at the Village of South Bend (Elderton, PA Quadrangle N:  0.6 inch; W:  16.7 inches) in South Bend Township, Armstrong County.

   E30-190.  Encroachment. PA Department of Transportation, Engineering District 12-0, P. O. Box 459, Uniontown, PA 15401. To remove the existing bridge and to construct and maintain a precast concrete spread box beam bridge having a single normal span of 80.0 feet and an underclearance of 13.2 feet across South Fork Tenmile Creek (HQ-WWF). Also to conduct channel cleaning within 50 feet upstream and 50 feet downstream from the bridge. The project is located along S. R. 0018 approximately 200 feet south of its intersection with S. R. 0021 (Holbrook, PA Quadrangle N:  22.5 inches; W:  4.5 inches) in Center Township, Greene County.

   E65-734.  Encroachment. Ligonier Township Supervisors, 18 Old Lincoln Highway West, Ligonier, PA 15658-8763. To construct and maintain earthen dikes having a maximum height of 1.5 feet and extending approximately 700 l. f. along an unnamed tributary to Mill Creek (CWF) and to remove sediment deposits and debris from the stream banks, for the purpose of minimizing flooding. The project is located off of Township Road 519 (Wilpen, PA Quadrangle N:  0.1 inch; W:  14.0 inches) in Ligonier Township, Westmoreland County.

   E65-736.  Encroachment. Pennsylvania Department of Conservation and Natural Resources, Bureau of Forestry, Rachel Carson State Office Building, 400 Market Street, P. O. Box 8451, Harrisburg, PA 17105-8451. To rehabilitate and maintain a bridge having a clear span of 4.61 m and an underclearance of 1.8 m across Grove Run (HQ-CWF) located on Linn Run Road (T-958) in Linn Run State Park. The project includes removing the existing steel I-beams and open steel grid deck and replacing it with a glued laminated timber deck (Ligonier, PA Quadrangle N:  5.1 inches; W:  14.1 inches) in Cook Township, Westmoreland County.

   E63-475.  Encroachment. Missionary Partners, Ltd., 859 Missionary Drive, Pittsburgh, PA 15236. To place and maintain fill in approximately 0.08 acre of wetlands (PEM) adjacent to an unnamed tributary to Peters Creek (TSF) for the purpose of constructing a business building located on the west side of Route 88, approximately 600 feet southwest from the intersection of Route 88, Finley Avenue and Boyka Drive (Hackett, PA Quadrangle N:  21.5 inches; W:  0.5 inch) in Union Township, Washington County.

   E65-737.  Encroachment. Unity Township Board of Supervisors, 1104 Beatty County Road, Latrobe, PA 15650. To remove the existing structure and to construct and maintain a bridge having a normal clear span of 14.33 m and an underclearance of 2.45 m across Ninemile Run (WWF). The structure is located on T-896 (Schultz Lane) at a point approximately 200 feet north of its intersection with S. R. 982 (Derry, PA Quadrangle N:  3.4 inches; W:  17.175 inches) in Unity Township, Westmoreland County.

   E20-482.  Encroachment. Cambridge Springs Borough, 26 Federal Street, Cambridge Springs, PA 16403. To demolish the existing water treatment building and to construct and maintain a water treatment building and a finished water storage tank within the FEMA mapped 100-year floodplain of French Creek. The project is lo-cated adjacent to the existing water treatment facility for Cambridge Springs Borough approximately 1,000 feet west of the intersection of SR 99 and SR 6/19 (Cambridge Springs, PA Quadrangle N:  10.2 inches; W:  8.0 inches) located in Cambridge Springs Borough, Crawford County.

   E20-484.  Encroachment. Beaver Township, 725 Artman Road, Conneautville, PA 16406. To remove the existing bridge and to construct a steel stringer bridge having a span of 23 feet and an underclearance of 5 feet across a tributary to Conneaut Creek on T-348 (Cole Road) approximately 500 feet south of SR 198 (Beaver Center, PA Quadrangle N:  1.35 inches; W:  4.9 inches) in Beaver Township, Crawford County.

   E62-350.  Encroachment. Cochran and Zandi Lumber Co., P. O. Box 547, Sheffield, PA 16347. To construct and maintain a steel beam bridge having a span of 28 feet and an underclearance of 4 feet across a tributary to Arnot Run (EV, Wild Trout). This project is located across a tributary to Arnot Run approximately 3,200 feet south of the intersection of Cherry Grove Road (SR 2001) and Sorenson Road (Cherry Grove, PA Quadrangle N:  3.3 inches; W:  1.8 inches) located in Cherry Grove Township, Warren County.

The proposed dam will be located approximately 1,800 feet east of the intersection of Deer Creek Road and Kyle Road (Valencia, PA Quadrangle N:  9.25 inches; W:  1.00 inch) in Middlesex Township, Butler County.

   The Department of Environmental Protection (Department) has taken the following actions on previously received permit applications and requests for plan approval and has issued the following significant orders.

   Persons aggrieved by this action may appeal, under section 4 of the Environmental Hearing Board Act (35 P. S. § 7514) and 2 Pa.C.S. §§ 501--508 and 701--704 (relating to the Administrative Agency Law), to the Environmental Hearing Board, Second Floor, Rachel Carson Street State Office Building, 400 Market Street, P. O. Box 8457, Harrisburg, PA 17105-8457, (717) 787-3483. TDD users may contact the Board through the Pennsylvania Relay Service, (800) 654-5984. Approvals to Use NPDES and Other General Permits

   The following parties have submitted (1)  Notices of Intent (NOIs) for coverage under General NPDES Permit(s) to discharge wastewater into the surface waters of this Commonwealth; (2)  NOIs for coverage under General Permit(s) for Beneficial Use of Sewage Sludge or Residential Septage by Land Application in Pennsylvania; or (3)  Notifications for First Land Application of Sewage Sludge.

   The approval of coverage under these General Permits may be subject to one or more of the following:  pollutant or effluent discharge limitations, monitoring and reporting, pathogen and vector attraction reduction requirements, operational standards, general requirements, management practices and other conditions set forth in the respective general permit. The Department of Environmental Protection (Department) has reviewed the NOIs and determined that they comply with administrative requirements of the respective permit application. Also, the Department has evaluated the First Land Application of Sewage Sludge for the sites applying for coverage under PAG-7, PAG-8 and PAG-9 and determined that the sites are suitable for land application of sewage sludge.

   The EPA Region III Regional Administrator has waived the right to review or object to this permit action under the waiver provision:  40 CFR 123.24.

   The application and related documents, effluent limitations, permitting requirements and other information are on file and may be inspected and arrangement made for copying at the contact office noted.

The rehabilitation included repair, cleaning, painting, disinfecting, water quality testing and related items.

   Permit No. 5999501. The Department issued a construction permit to Municipal Authority of the Borough of Mansfield, 19 East Wellsboro Street, Mansfield, PA 16933, Richmond Township, Tioga County, for construction of a new well, Well No. 2.
