   For the purpose of evaluating effluent requirements for TDS, NO2-NO3, fluoride, phenolics, sulfate and chloride, the existing/proposed downstream potable water supply, considered during the evaluation is the PA-American Water Company intake located on the Clarion River approximately 4 miles below point of discharge.

   The receiving stream, the Dry/Intermittent Tributary to Blyson Run, is in watershed 17-B and classified for EV, aquatic life, water supply and recreation.

   The proposed effluent limits for Outfall 001 are based on a design flow of 0.0004 mgd.

   Description of Proposed Action/Activity: Construction/Operation of Plant Expansion including Phase II.

   WQM Permit No. 0691419 Amendment 05-1, Sewerage, Twin Valley School District, 4851 Twin Valley Road, Elverson, PA 19520. This proposed facility is located in Caernarvon Township, Berks County.

   Description of Proposed Action/Activity: Replacement of a portion of the existing sand filter with a new self-cleaning filter.

   Northcentral Region: Water Management Program Manager, 208 West Third Street, Williamsport, PA 17701.

   WQM Permit No. 1805402 Sewerage Municipal, Woodward Township Sewer Authority, 86 Riverside Terrace, Lock Haven, PA 17745. This proposed facility is located in Woodward Township, Clinton County.

   Description of Proposed Action/Activity: Reconstruction of the existing Upper Lockport Pump Station in Riverview Park with new wet well, submersible pumps, controls and associated piping.

   WQM Permit No. 5905401 Sewage 4952, Liberty Area Municipal Authority, P. O. Box 73, Liberty, PA 16930. This proposed facility is located in Liberty Boro/Liberty Township, Tioga County.

   Description of Proposed Action/Activity: The applicant proposes to construct and operate a sewer collection system and a 75,000 gallon per day recirculating sand filter sewage treatment plant; 28,600 feet of the sewer will be a septic tank effluent collection system; 8,700 feet will be conventional gravity sewer. Disinfection will be by UV light with a backup chlorinator during flood stage events. A cascade aerator will increase dissolved oxygen levels prior to discharge to Blockhouse Creek.

   WQM Permit No. 1204401, Sewerage 4952, Mid-Cameron Authority, 421 North Broad Street, Emporium, PA 15834. This proposed facility is located in Shippen Township, Cameron County.

   Description of Proposed Action/Activity: Mid-Cameron Authority proposes treatment upgrades and an increase in design flow of their wastewater treatment plant in Shippen Township, Cameron County. The proposed treatment upgrades will provide for the existing needs as identified in the Act 537 Plan, reduce the number of combined sewer overflow events and address the Chesapeake Bay Program nutrient reduction efforts.

   WQM Permit No. 0805402, Sewerage Municipal, Athens Township Authority, 370 Pennsylvania Avenue, Sayre, PA 18840. This proposed facility is located in Athens Township, Bradford County.

   Description of Proposed Activity: Modification to two pump stations and a sewer main extension consisting of approximately 11,000 feet of gravity and 2,850 feet of force main.

PUBLIC WATER SUPPLY (PWS) PERMIT

   Under the Pennsylvania Safe Drinking Water Act (35 P. S. §§ 721.1--721.17), the following parties have applied for a PWS permit to construct or substantially modify a public water system.

   Persons wishing to comment on a permit application are invited to submit a statement to the office listed before the application within 30 days of this public notice. Comments received within the 30-day comment period will be considered in the formulation of the final determinations regarding the application. Comments should include the name, address and telephone number of the writer and a concise statement to inform the Department of Environmental Protection (Department) of the exact basis of a comment and the relevant facts upon which it is based. A public hearing may be held after consideration of comments received during the 30-day public comment period.

   Following the comment period, the Department will make a final determination regarding the proposed permit. Notice of this final determination will be published in the Pennsylvania Bulletin at which time this determination may be appealed to the Environmental Hearing Board.

   The permit application and any related documents are on file at the office listed before the application and are available for public review. Arrangements for inspection and copying information should be made with the office listed before the application.

   Persons with a disability who require an auxiliary aid, service or other accommodations to participate during the 30-day public comment period should contact the office listed before the application.
