AIR QUALITY
PLAN APPROVAL AND OPERATING PERMIT APPLICATIONS The Department has developed an ''integrated'' plan approval, State Operating Permit and Title V Operating Permit program. This integrated approach is designed to make the permitting process more efficient for the Department, the regulated community and the general public. This approach allows the owner or operator of a
facility to submit permitting documents relevant to its application for all sources related to a facility or a proposed project, affords an opportunity for public input, and provides for a decision on the issuance of the necessary permits.
The Department received applications for Plan Approvals or Operating Permits from the following facilities.
Copies of the application, the Department's analysis, all pertinent documents used in the evaluation of the application and subsequently prepared proposed plan approvals/operating permits are available for public review during normal business hours at the appropriate Department Regional Office. Appointments for scheduling a review must be made by calling the appropriate Department Regional Office. The address and phone number of the Regional Office is listed before the application notices.
Persons wishing to file a written protest or provide comments or additional information, which they believe should be considered prior to the issuance of a permit, may submit the information to the Department's Regional Office. A 30-day comment period from the date of this publication will exist for the submission of comments, protests and information. Each submission must contain the name, address and telephone number of the person submitting the comments, identification of the proposed Plan Approval/Operating Permit including the permit number and a concise statement regarding the relevancy of the information or objections to issuance of the permit.
A person wishing to request a hearing may do so during the 30-day comment period. A public hearing may be held, if the Department, in its discretion, decides that a hearing is warranted based on the information received. Persons submitting comments or requesting a hearing will be notified of the decision to hold a hearing by publication in the newspaper, the Pennsylvania Bulletin or by telephone, when the Department determines this type of notification is sufficient. Requests for a public hearing and any relevant information should be directed to the appropriate Department Regional Office.
Permits issued to the owners or operators of sources subject to 25 Pa. Code Chapter 127, Subchapter D or E, or located within a Title V facility or subject to 25 Pa. Code § 129.51(a) or permits issued for sources with limitations on their potential to emit used to avoid otherwise applicable Federal requirements may be submitted to the United States Environmental Protection Agency for review and approval as a revision to the State Implementation Plan. Final Plan Approvals and Operating Permits will contain terms and conditions to ensure that the sources are constructed and operating in compliance with applicable requirements in the Air Pollution Control Act (35 P. S. §§ 4001—4015), 25 Pa. Code Chapters 121—145, the Federal Clean Air Act (42 U.S.C.A. §§ 7401—7671q) and regulations adopted under the Federal Clean Air Act.

28-03045A: Dominion Transmission, Inc. (5000 Dominion Blvd, Glen Allen, VA 23060) for a gas turbine project, as described below, at the Chambersburg Compressor Station in Hamilton Township, Franklin County. Plan Approval No. 28-03045A will authorize the installation of a new natural gas fired Solar Mars gas turbine, rated at 13,220 hp. The potential emissions from the turbine are: NOx 29 tpy, CO 3 tpy, VOC 2 tpy, SOx 1.8 tpy, PM10/2.5 8 tpy, and HAPs 0.4 tpy. Review by DEP of the information submitted by the applicant indicates that the air contamination sources as constructed or modified will comply with all regulatory requirements pertaining to air contamination sources and the emission of air contaminants including the best available technology requirement (BAT) of 25 Pa. Code §§ 127.1 and 127.12, and will be subject to 40 CFR Part 60 Subpart KKKK—Standards of Performance for Stationary Combustion Turbines. Based on these findings, the Department proposes to issue a plan approval for the proposed construction. If, after the project has been implemented, the Department determines that the sources are constructed and operated in compliance with the plan approval conditions and the specification of the application for plan approval, the requirements established in the plan approval will be incorporated into a State Only Operating Permit pursuant to the administrative amendment provisions of 25 Pa.Code § 127.450.

42-028G: Ardagh Glass—Port Allegany Facility (One Glass Place, Port Allegany, PA 16743), for establishing SO2 emission limits for both furnaces due to the Global Consent Decree with EPA in Port Allegany Borough, McKean County. This is a Title V facility. The public notice is required for sources required to obtain a Plan Approval in accordance with 25 Pa. Code § 127.44. This plan approval will, in accordance with 25 Pa. Code § 127.450 or § 127.505, be incorporated into the facility operating permit at a later date.
Plan approval No 42-028G is for the proposed SO2 emission limits required for Furnaces 1 & 3 from Global Consent Decree (GCD) with EPA. This modification will not result in an emission increase at the facility. This Plan Approval will contain emission restriction, testing, recordkeeping, work practice standard and additional requirement conditions, which will satisfy the requirements of 25 Pa. Code § 127.12b (pertaining to plan approval terms and conditions) and will demonstrate Best Available Technology (BAT) for the source including, but are not limited to, the following:
• Source 101 (Furnace 1)
• Normal Operation (Flint): Furnace No. 1 shall not emit more than 2.4 # SO2 per ton of glass produced on a 30-day Rolling Average as measured using an SO2 CEMS, except during the following periods: Abnormally Low Production Rate Days; Color Transition; Furnace Startup; Malfunction of the Furnace; and Maintenance of the Furnace. [This condition replaces Section D Condition #009 from facility operating permit 42-00028 revised on May 19, 2014.]
• Normal Operation (Colored): Furnace No. 1 shall not emit more than 2.4 # SO2 per ton of glass produced on a 30-day Rolling Average as measured using an SO2 CEMS, except during the following periods: Abnormally Low Production Rate Days; Color Transition; Furnace Startup; Malfunction of the Furnace; and Maintenance of the Furnace. [This condition replaces Section D Condition #009 from facility operating permit 42-00028 revised on May 19, 2014.]
• Abnormally Low Production Day (Flint): Furnace No. 1 SO2 Limit during Abnormally Low Production Rate Days the facility may elect to exclude—all Abnormally Low Production Rate Days from the Emission Rate 30-day Rolling Average when Furnace No. 1 is operating at an Abnormally Low Production Rate. During the Malfunction Days excluded from the Emission Rate 30-day Rolling Average, a CEMS shall be used to demonstrate compliance on a 24-hour Block Average with the following pound per day limit: 1,563 #/day SO2 as calculated by 3 x (2.5 # SO2/ton glass) x (P/0.35) where P = 73. A public hearing may be held, if the Department of Environmental Protection, in its discretion, decides that such a hearing is warranted based on the comments received. All persons submitting comments or requesting a hearing will be notified of the decision to hold a hearing by publication in the newspaper or the Pennsylvania Bulletin or by telephone, where DEP determines such notification is sufficient. In accordance with 25 Pa. Code § 127.45, a person may oppose the proposed plan approval by filing a written protest with the Department's Northwest Region Air Quality Program Manager.
If a plan approval has not undergone the above public notice process, the change to an operating permit must be treated as a significant modification. In these situations the Department should follow the procedures described in §§ 127.421—127.431 for state only operating permits or §§ 127.521—127.524 for Title V operating permits.

28-05040: INGENCO Wholesale Power, LLC (811 Main Street, Suite 3500, Houston, Texas 77002) to issue a Title V Permit renewal for the electric generating facility in Peters Township, Franklin County. Reported plant emissions for 2014 included 47.5 tons of nitrogen oxides (NOx), 7.6 tons of particulate matter (PM), and 10.5 tons of volatile organic compounds (VOC). The Title V Operating Permit will include emission limits and work practice standards along with monitoring, recordkeeping and reporting requirements to ensure the facility complies with the applicable air quality regulations. Among other items, the permit renewal will include provisions derived from 40 CFR Part 63, Subpart ZZZZ—National Emission Standards for Hazardous Air Pollutants for Stationary Reciprocating Internal Combustion Engines.

09-00022: Cleveland Steel Container Corporation (350 Mill Street, Quakertown, PA 18951), located in Quakertown Borough, Bucks County. This action is a renewal of a Synthetic Minor Operating Permit. The facility's emission points include a pail line spray coating line, a roller coater, two (2) curing ovens, and one (1) lithographic printing press. Volatile Organic Compound (VOC) emissions from the pail line coating process are controlled by a Regenerative Thermal Oxidizer and VOC emissions from the roller coating process are controlled by a Catalytic Incinerator. The facility elects to limit facility-wide VOC emissions to less than 25 tons per year; therefore, the facility is considered a Synthetic Minor for criteria pollutants and an area source of Hazardous Air Pollutant (HAP) emissions. There are no new sources at the facility; however, the requirements of 25 Pa. Code § 129.77 apply to adhesives used when gluing a rubber gasket onto pail lids which has been addressed with this renewal permit in Source ID 109 (Adhesives). The renewal permit will include monitoring, recordkeeping and reporting requirements designed to keep the facility operating within all applicable air quality requirements.

67-05013: York City Sewer Authority (1701 Black Bridge Road, York, PA 17402) to issue a State Only Operating Permit for a waste water treatment plant and heating sources located in Manchester Township, York County. The primary emissions from the facility is VOC, the actual 2014 emissions from the facility was 18 tons. The Operating Permit will include emission limits and work practice standards along with monitoring, recordkeeping and reporting requirements to ensure the facility complies with the applicable air quality regulations. Among other items, the conditions include provisions for the source derived from 25 Pa. Code §§ 127.441, 127.444, and 135.3 for monitoring, recordkeeping, and reporting.

41-00017: Gulf Oil, L.P. (100 Crossing Blvd, Framingham, MA 01702-5401) to issue a renewal State Only Synthetic Minor Operating Permit for their South Williamsport Terminal facility located in Armstrong Township, Lycoming County. The facility is currently operating under State Only Synthetic Minor Operating Permit 41-00017. The facility's main sources include gasoline storage tanks, diesel storage tank, and gasoline and distillate loading operations. The facility has poten-
tial annual emissions of 0.07 ton of carbon monoxide, 0.02 ton of nitrogen oxides, 0.01 ton of sulfur oxides, 0.02 ton of particulate matter (including particulate matter less than 10 microns in size and particulate matter less than 2.5 microns in size), 49.71 tons of volatile organic compounds, 11.08 tons of hazardous air pollutants, and 72 tons of carbon dioxide equivalent. The tanks are subject to Subpart BBBBBB of the National Emission Standards for Hazardous Air Pollutants for Source Categories, 40 CFR 63.11080—63.11100 (National Emission Standards for Hazardous Air Pollutants for Source Category: Gasoline Distribution Bulk Terminals, Bulk Plants, and Pipeline Facilities) and the loadout operations are subject to Subpart XX of the Standards of Performance for New Stationary Sources, 40 CFR 60.500—60.506 (Standards of Performance for Bulk Gasoline Terminals). The emission limits, throughput limitations and work practice standards along with testing, monitoring, record keeping and reporting requirements have been included in the operating permit to ensure the facility complies with all applicable Federal and State air quality regulations. These operating permit conditions have been derived from the applicable requirements of 25 Pa. Code Chapters 121—145 as well as 40 CFR Parts 60 and 63. All pertinent documents used in the evaluation of the application are available for public review during normal business hours at the Department's Northcentral Regional office, 208 West Third Street, Suite 101, Williamsport, PA 17701. 41-00059: Bimbo Bakeries USA, Inc. (3375 Lycoming Creek Road, Williamsport, PA 17701) to issue a renewal State Only Synthetic Minor Operating Permit for their Williamsport Plant facility located in Old Lycoming Township, Lycoming County. The facility is currently operating under State Only Synthetic Minor Operating Permit 41-00059. The facility's main sources include a 5.25 MMBtu/hr natural gas-fired boiler, a 2.86 MMBtu/hr natural gas-fired boiler, a 1.25 MMBtu/hr natural gas-fired heater, a 2.1 and a 1.0 MMBtu/hr natural gas-fired heaters, a 1.12 MMBtu/hr natural gas-fired boiler, one 4.389 MMBtu/hr natural gas-fired bake oven, and one 3.388 MMBtu/hr natural gas-fired roll oven, and 10 natural gas fired space heaters with a combined heat input rating of 1.125 MMBtus/hr. The facility has potential annual emissions of 24.30 tons of carbon monoxide, 11.03 tons of nitrogen oxides, 0.05 ton of sulfur oxides, 0.81 ton of particulate matter (including particulate matter less than 10 microns in size and particulate matter less than 2.5 microns in size), 31.57 tons of volatile organic compounds, 0.90 ton of hazardous air pollutants, and 12,883 tons of carbon dioxide equivalent. The emission limits, throughput limitations and work practice standards along with testing, monitoring, record keeping and reporting requirements have been included in the operating permit to ensure the facility complies with all applicable State air quality regulations. MINING ACTIVITY NPDES DRAFT PERMITS This notice provides information about applications for a new, amended or renewed NPDES permits associated with mining activity (coal or noncoal) permits. The applications concern industrial waste (mining) discharges to surface water and discharges of stormwater associated with mining activities. This notice is provided in accordance with 25 Pa. Code Chapters 91 and 92a and 40 CFR Part 122, implementing provisions of The Clean Streams Law (35 P. S. §§ 691.1—691.1001) and the Federal Clean Water Act (33 U.S.C.A. §§ 1251—1376).
The Department of Environmental Protection (Department) has prepared a draft NPDES permit and made a tentative determination to issue the NPDES permit in conjunction with the associated mining activity permit.
Effluent Limits for Coal Mining Activities
For coal mining activities, NPDES permits, when issued, will contain effluent limits that are the more stringent of technology-based (BAT) effluent limitations or Water Quality Based Effluent Limits (WQBEL).
The BAT limits for coal mining activities, as provided in 40 CFR Part 434 and 25 Pa. Code Chapters 87—90 are as follows:

Code §§ 87.102, 88.92, 88.187, 88.292, 89.52 and 90.102.
Exceptions to BAT effluent limits may be applicable in accordance with 25 Pa. Code §§ 87.102, 88.92, 88.187, 88.292, 89.52 and 90.102.
Effluent Limits for Noncoal Mining Activities
The limits for noncoal mining activities as provided in 25 Pa. Code Chapter 77 are pH 6 to 9 and other parameters the Department may require.
Discharges from noncoal mines located in some geologic settings (for example, in the coal fields) may require additional water quality based effluent limits. If additional effluent limits are needed for an NPDES permit associated with a noncoal mining permit, then the permit description specifies the parameters.
In addition to BAT or WQBEL limits, coal and noncoal NPDES permits establish effluent limitations in the form of implemented Best Management Practices (BMPs) identified in the associated Erosion and Sedimentation Plan, the Reclamation Plan and the NPDES permit application. These BMPs restrict the rates and quantities of associated pollutants from being discharged into surface waters in this Commonwealth.
More restrictive effluent limitations, restrictions on discharge volume or restrictions on the extent of mining that may occur are incorporated into an NPDES permit when necessary for compliance with water quality standards and antidegradation requirements (in accordance with 25 Pa. Code Chapters 91—96).
The procedures for determining the final effluent limits, using a mass-balance equation or model, are found in Technical Guidance Document 563-2112-115, Developing National Pollutant Discharge Elimination System (NPDES) Permits for Mining Activities. Other specific factors to be considered include public comments and Total Maximum Daily Load(s). Additional discharge limitations may apply in the event that unexpected discharges occur.
Discharge rates for surface mining activities are precipitation driven. Discharge rates for proposed discharges associated with underground mining are noted in the permit description.
Persons wishing to comment on an NPDES draft permit should submit a written statement to the Department at the address of the district mining office indicated before each draft permit within 30 days of this public notice. Comments received within the comment period will be considered in the final determinations regarding the NPDES permit applications. Comments must include the name, address and telephone number of the writer and a concise statement to inform the Department of the exact basis of a comment and the relevant facts upon which it is based.
The Department will also accept requests or petitions for a public hearing on NPDES permit applications, as provided in 25 Pa. Code § 92a.82(d). The request or petition for a public hearing shall be filed within 30 days of this public notice and contain the name, address, telephone number and the interest of the party filing the request, and state the reasons why a hearing is warranted. A public hearing may be held if the Department considers the public interest significant. If a hearing is scheduled, a notice of the hearing on the NPDES permit application will be published in the Pennsylvania Bulletin and a newspaper of general circulation within the relevant geographical area. When a public hearing is held, the Department will consider comments from the public hearing in the final determination on the NPDES permit application.

FEDERAL WATER POLLUTION CONTROL ACT, SECTION 401 The following permit applications, requests for Environmental Assessment approval and requests for 401 Water Quality Certification have been received by the Department. Section 401 of the Federal Water Pollution Control Act (FWPCA) (33 U.S.C.A. § 1341) requires the Commonwealth to certify that the involved projects will not violate the sections 301—303, 306 and 307 of the FWPCA (33 U.S.C.A. §§ 1311—1313, 1316 and 1317) as well as relevant State requirements. Persons objecting to approval of a request for certification under section 401 of the FWPCA, the issuance of a Dam Permit or Water Obstruction and Encroachment Permit or the approval of an Environmental Assessment shall submit comments, suggestions or objections within 30 days of the date of this notice as well as any questions to the office noted before an application. Comments should contain the name, address and telephone number of the person commenting, identification of the certification request to which the comments or objections are addressed and a concise statement of comments, objections or suggestions including the relevant facts upon which they are based.
The Department may conduct a fact-finding hearing or an informal conference in response to comments if deemed necessary. Each individual will be notified, in writing, of the time and place of a scheduled hearing or conference concerning the certification request to which the comment, objection or suggestion relates. Maps, drawings and other data pertinent to the certification request are available for inspection between 8 a.m. and 4 p.m. on working days at the office noted before the application.

To maintain an existing 6-inch diameter petroleum gas pipeline crossing of Harveys Creek (CWF, MF) consisting of a 75-ft wide X 45-ft long articulating block matting and a 12-ft high X 35-ft long concrete revetment wall. The project is located approximately 200 feet southeast of the intersection of S.R. 11 and S.R. 29 (Nanticoke, PA Quadrangle, Latitude: 41°13`14"; Longitude: -76°0`55.08").

To remove the existing structure; and to construct and maintain 50-foot wide road crossing of a tributary to Roaring Brook (CWF, MF) consisting of an aluminum open bottom box culvert with a normal clear span of 15.5 feet and an underclearance of 6.92 feet. The project is located along S.R. 4026, Section 574, Segment 0020, Offset 1355 (Harveys Lake, PA Quadrangle, Latitude: 41°15`21.3"; Longitude: -76°07`10.5").

To install a Type M inlet with a sump at the intersection of North and Spruce Streets in order to reduce the backwater flooding effects from Wiconisco Creek (WWF, MF) and provide easier access for the use of pumps, and to install a new endwall accompanying the replacement of an inlet and a broken 24-inch stormwater discharge pipe that inlets at the intersection of North and Municipal Streets. This Lykens Borough project is located in Dauphin County (Lykens, PA Quadrangle, Latitude: 40°34`6.3", Longitude: -76°42`1.1"). The permit was issued on October 14, 2015.

To install and maintain a 36-inch-diameter pipeline under and/or through the below listed floodways, streams and wetlands as part of the Atlantic Sunrise Project. The new 2.9 mile long pipe will be installed adjacent to an existing pipeline as a loop. The project is an expansion of the existing Transco natural gas transmission system that will enable Transco to provide transportation of natural gas from the Marcellus Shale production areas in northern Pennsylvania to its existing market areas, extending as far south as Choctaw County, Alabama.

1. Construct and maintain stream bank protection along approximately 500` of Gourdhead Run (TSF);
2. Excavate material from the floodway and floodplain, and to reduce the height and slope of both stream banks, along this same reach of Gourdhead Run;
3. Construct and maintain 3` high berms on both sides of Gourdhead Run, in the floodway and floodplain, perpendicular to this watercourse, and at the downstream limit of this same reach of Gourdhead Run;
4. Construct and maintain pedestrian bridges across Gourdhead Run, at the upstream and downstream limits of this same reach of Gourdhead Run;
For the purpose of creating a pedestrian walking trail and the Gourdhead Run floodplain detention project, near the intersection of Duncan Avenue and State Route 8 (Glenshaw, PA USGS topographic quadrangle; Latitude: 40° 33` 35"; Longitude: -79° 57` 36"; Sub-basin: 18A, Pittsburgh Corps District) in the Township of Hampton, Allegheny County.
