Service of Notice of Motor Carrier Applications

 The following temporary authority and/or permanent authority applications for the right to render service as a common carrier or contract carrier in this Commonwealth have been filed with the Pennsylvania Public Utility Commission. Formal protests and petitions to intervene must be filed in accordance with 52 Pa. Code (relating to public utilities). A protest shall indicate whether it applies to the temporary authority application, the permanent authority application, or both. Filings must be made with the Secretary, Pennsylvania Public Utility Commission, P.O. Box 3265, Harrisburg, PA 17105-3265, with a copy served on the applicant by October 2, 2017. Documents filed in support of the applications are available for inspection and copying at the Office of the Secretary between 8 a.m. and 4:30 p.m., Monday through Friday, and at the business address of the respective applicant.

Applications of the following for approval to begin operating as common carriers for transportation of persons as described under each application.

A-2017-2618571. Rafael Amaya (35 West Diamond Avenue, Hazleton, Luzerne County, PA 18201) for the right to begin to transport, as a common carrier, by motor vehicle, persons in paratransit service, from points in Luzerne County, to various factories in Pennsylvania, and return.

A-2017-2619565. John Hartman and Brenda Hartman, Copartners, t/a North Central Touring (P.O. Box 212, Shermans Dale, PA 17090) persons in group and party service, in vehicles seating between 11 and 15 passengers, including the driver, from points in Tioga and Potter Counties, to points in Cherry Springs State Park, Potter County, and return.

Health Star Personal Care Transportation, LLC (13049 Frankstown Road, Penn Hills, Allegheny County, PA 15235) for the right to transport as a common carrier, by motor vehicle in wheelchair and stretcher vans, persons in paratransit service, from points in the Counties of Allegheny, Armstrong, Beaver, Butler, Washington and Westmoreland, to points in Pennsylvania, and return. Pace Healthcare Transportation, Inc., t/a Pace Healthcare Transportation (401 Broad Street, Johnstown, Cambria County, PA 15906) for the amended right to transport as a common carrier, by motor vehicle, persons in paratransit service, limited to persons requiring wheelchair and stretcher van transportation from skilled nursing facilities, from points in the Counties of Berks, Bucks, Carbon, Centre, Dauphin, Fayette, Greene, Indiana, Lehigh, Monroe, Somerset, Washington, Westmoreland and York, to points in Pennsylvania, and return.

Box 529, 269 Green Street, Royersford, Montgomery County, PA 19468) for the right to begin to transport, as a common carrier, by motor vehicle, persons in paratransit service, from points in the Counties of Chester and Montgomery, to points in Pennsylvania, and return.

A-2017-2622194. Susquehanna Valley Taxi Service, Inc. (2225 Ridge Road, Northumberland, PA 17857) for the right to begin to transport, as a common carrier, by motor vehicle, persons, in call or demand service, in the Borough of Riverside, Northumberland County, and Montour County. Application of the following for approval to begin operating as contract carriers for transportation of persons as described under the application.

RM Home Services, t/a Community Home Services (672 Main Street, Harleysville, Montgomery County, PA 19438) for the amended right to begin to transport as a contract carrier, by motor vehicle, persons in residential retirement communities owned by nonprofit entities affiliated with Grace Inspired Ministries which includes The Lutheran Community at Telford, to points in Pennsylvania, and return.
