Service of Notice of Motor Carrier Applications

 The following temporary authority and/or permanent authority applications for the right to render service as a common carrier or contract carrier in this Commonwealth have been filed with the Pennsylvania Public Utility Commission. Formal protests and petitions to intervene must be filed in accordance with 52 Pa. Code (relating to public utilities). A protest shall indicate whether it applies to the temporary authority application, the permanent authority application, or both. Protests may only be filed if there is evidence that the applicant lacks fitness. Protests based on endangering or impairing operations of an existing carrier will not be honored. Filings must be made with the Secretary, Pennsylvania Public Utility Commission, 400 North Street, Harrisburg, PA 17120, with a copy served on the applicant by September 9, 2019. Documents filed in support of the applications are available for inspection and copying at the Office of the Secretary between 8 a.m. and 4:30 p.m., Monday through Friday, and at the business address of the respective applicant.

Applications of the following for approval to begin operating as common carriers for transportation of persons as described under each application.

A-2019-3008556 (Corrected). Dolores Jiménez De La Rosa, t/a De La Rosa's Transportation (823 West Front Street, Berwick, Columbia County, PA 18603) persons, in paratransit service, from points in Luzerne County, to points in Pennsylvania, and return.

A-2019-3011884. Lehighton Ambulance Association, Inc., t/a Lehighton Ambulance (P.O. Box 82, Lehighton, Carbon County, PA 18235) for the right to begin to transport, as a common carrier, by motor vehicles, persons in paratransit service, between points in the County of Carbon, the Boroughs of Coaldale and Tamaqua, and the Townships of Rush and West Penn, all located in Schuylkill County.

A-2019-3011973. Fausto Rodriguez, t/a Rodriguez Transportation (319 West Diamond Avenue, Hazleton, Luzerne County, PA 18201) persons in paratransit service, restricted to service to or from a client's place of employment, between points in Luzerne County.

Applications of the following for approval of the additional right and privilege of operating motor vehicles as common carriers for transportation of persons as described under each application.

A-2019-3011475. Medi-Trans Group, LLC (522 Windy Way, New Cumberland, York County, PA 17070) for the right to begin to transport, as a common carrier, by motor vehicles, persons in paratransit service, from points in Dauphin County to points in Pennsylvania, and return.

A-2019-3011889. Red Carpet Limousine Connection, Inc. (2566 Kingview Road, Scottdale, Fayette County, PA 15683) for the right to begin to transport, as a common carrier, by motor vehicle, persons, in group and party service, in vehicles seating 11 to 15 passengers, including the driver, between points in Pennsylvania.

Application of the following for approval of the beginning of the exercise of the right and privilege of operating motor vehicles as common carriers for the transportation of household goods as described under the application.

A-2019-3012174. Innercity Rehab, Inc. (600 Deer Road, Suite 1, Cherry Hill, Camden County, NJ 08034) household goods in use, between points in the Counties of Bucks, Chester, Delaware and Montgomery, and the City and County of Philadelphia.

Application of the following for approval to begin operating as a broker for transportation of persons as described under the application.

A-2019-3011446. Prudentiacare Home Health Services, LLC (1 International Drive, Suite 517, Philadelphia, Philadelphia County, PA 19113) for the right to begin to arrange transportation of passengers, between points in the Counties of Bucks, Chester, Delaware and Montgomery, and the City and County of Philadelphia.

Applications of the following for the approval of the right and privilege to discontinue/abandon operating as common carriers by motor vehicle and for cancellation of the certificate of public convenience as described under each application.

A-2019-3011185. Frank H. Barton, t/a F. H. Barton Transportation (19747 State Route 92, Susquehanna, Susquehanna County, PA 18847) discontinuance of service and cancellation of its certificate, to transport, as a common carrier, by motor vehicle, at A-8912174, persons in paratransit service, from points in Potter, Tioga, Bradford, Susquehanna and Lackawanna Counties, to points in Pennsylvania, and return.

A-2019-3011967. Carrie L. Shumway (40 Cupola Road, Honey Brook, Chester County, PA 19344) discontinuance of service and cancellation of her certificate, to transport, as a common carrier, by motor vehicle, persons in paratransit service, limited to persons whose personal convictions prevent them from owning or operating motor vehicles, from points in Lancaster County, to points in Pennsylvania, and return.

A-2019-3012017. Latrobe Area Hospital, Inc. (121 West Second Avenue, Latrobe, Westmoreland County, PA 15650) discontinuance of service and cancellation of its certificate, to transport, as a common carrier, by motor vehicle, persons in paratransit service, between points in the City of Latrobe, Ligonier Township, the Boroughs of Ligonier and Laurel Mountain, and other points in Westmoreland County located within an airline distance of 20 statute miles of the limits of said territory, and from points in said territory, to points in Pennsylvania, and return.
