Service of Notice of Motor Carrier Applications

 The following temporary authority and/or permanent authority applications for the right to render service as a common carrier or contract carrier in this Commonwealth have been filed with the Pennsylvania Public Utility Commission (Commission). Formal protests, petitions to intervene and answers must be filed in accordance with 52 Pa. Code (relating to public utilities) on or before April 26, 2021. Filings must be made electronically through eFiling to the Secretary of the Pennsylvania Public Utility Commission, 400 North Street, Harrisburg, PA 17120, with a copy served on the applicant by April 26, 2021. In accordance with the Commission's Emergency Order at M-2020-3019262, all parties participating in matters pending before the Commission are required to eFile their submissions by opening an eFiling account and accepting eService. Individuals can sign up for a free eFiling account with the Secretary of the Commission through the Commission's eFiling system at https://www.puc.pa.gov/efiling/Default.aspx. A protest shall indicate whether it applies to the temporary authority application, the permanent authority application, or both. Protests may only be filed if there is evidence that the applicant lacks fitness. Protests based on endangering or impairing operations of an existing carrier will not be honored. The documents filed in support of the application are only available for inspection through the Commission's web site at www.puc.pa.gov by searching under the previously listed docket number or by searching the applicant's web site.

Applications of the following for approval to begin operating as common carriers for transportation of persons as described under each application.

A-2021-3024472. Restorative Non Medical Transportation, LLC (7800 Haines Road, Cheltenham, Montgomery County, PA 19012) in paratransit service, limited to members of the Restorative Home Healthcare Agency, in vehicles staffed with a driver and either a nurse or nurse's aide, to or from healthcare facilities, or to or from locations necessary to obtain goods required for managing the member's medical conditions, from points in the Counties of Bucks, Chester, Delaware and Montgomery, and the City and County of Philadelphia, to points in Pennsylvania, and return.

A-2021-3024620. First Horizon Transportation Corporation (7325 Rising Sun Avenue, Philadelphia, PA 19111) in paratransit service, limited to members of the First Horizon Homecare Agency, in vehicles staffed with a driver and either a nurse or nurse's aide, to or from healthcare facilities, or to or from locations necessary to obtain goods required for managing the member's medical conditions, from points in the Counties of Bucks, Chester, Delaware and Montgomery, and the City and County of Philadelphia, to points in Pennsylvania, and return.

A-2021-3024844. Azzu Transport Group, LLC (611 North Harrisburg Street, Suite 2, Steelton, Dauphin County, PA 17113) for the right to begin to transport, as a common carrier, by motor vehicle, in paratransit service, between points in the Counties of Cumberland, Dauphin and Lancaster. A-2021-3024869. Liberty Limo Service, LLC (818 Mill Grove Drive, Norristown, Montgomery County, PA 19403) for the right to begin to transport, as a common carrier, by motor vehicle, persons in limousine service, between points in the Counties of Bucks, Chester, Delaware and Montgomery.

Applications of the following for the approval of the right and privilege to discontinue/abandon operating as common carriers by motor vehicle and for cancellation of the certificate of public convenience as described under each application.

A-2021-3024765. Carol Ann Frazier (111 Pheasant Run, Spring Mills, PA 16875) discontinuance of service and cancellation of its certificate—for the right to begin to transport, as a common carrier, by motor vehicle, persons in paratransit service, limited to persons whose personal convictions prevent them from owning or operating motor vehicles, from points in the County of Centre, to points in Pennsylvania, and return.

A-2021-3024835. Philly Limo, Inc. (P.O. Box 2644, Bala Cynwyd, Montgomery County, PA 19004) for the discontinuance of service and cancellation of its certificate, to transport, as a common carrier, by motor vehicle, persons in limousine service, from points in the Counties of Bucks, Chester, Delaware and Montgomery, to points in Pennsylvania, and return; excluding service that is under the jurisdiction of the Philadelphia Parking Authority, which is to be a transfer of all the right authorized under the certificate issued at A-00123405 to Luis A. Escobar, subject to the same limitations and conditions.
