Submission of Final Reports on Attainment of Background and Statewide Health Standards
The following final reports were submitted to the Department of Environmental Protection under the Land Recycling and Environmental Remediation Standards Act, act of May 19, 1995 (P. L. 4, No. 2).
Provisions of Chapter 3 of the Land Recycling and Environmental Remediation Standards Act (act) require the Department of Environmental Protection to publish in the Pennsylvania Bulletin a notice of submission of any final reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the act's remediation standards. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or non-residential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected.
For further information concerning the final report, please contact the Environmental Cleanup Program in the Department of Environmental Protection Regional Office under which the notice of receipt of a final report appears. If information concerning a final report is required in an alternative form, contact the community relations coordinator at the appropriate regional office listed. To construct and maintain (1) a roadway crossing of Bow Creek and adjacent wetlands, consisting of a 19.0-foot × 6.3-foot aluminum plate arch culvert, (2) a roadway crossing of a tributary to Bow Creek and adjacent wetlands, consisting of a 43-inch × 27-inch CMP arch culvert, and (3) a sanitary sewer line crossing of Bow Creek and adjacent wetlands, for the purpose of providing access and utilities to an expansion of the Bow Creek Manor II Subdivision. The project will include the placement of fill in approximately 0.22 acre of wetlands. The permittee is required to provide 0.22 acre of replacement wetlands.

To remove the existing structure and to construct and maintain a road crossing consisting of three 60-inch × 38-inch side-by-side culverts in Pond Creek. The project is located on S. R. 2049, approximately 0.2 mile north of the Freeland Borough boundary (Freeland, PA Quadrangle N: 5.3 inches; W: 3.1 inches) in Foster Township, Luzerne County.

To remove the existing structure and to construct and maintain a box culvert having a span of 22 feet with an underclearance of 8 feet (1 foot depressed) in Brush Run, to place and maintain fill in a de minimus wetland area equal to or less than 0.05 acre and to construct and maintain a berm on the right bank of said stream between the VFW bridge and the Vet Clinic bridge. The project is located at the intersection of East McMurray Road (S. R. 1002) and Valley Brook Road, on S. Location: The communities of South Fork, Ehrenfeld, Summerhill, New Germany, Wilmore Heights, Wilmore, Soukesberg, Rockville, Fishertown, Liberty Park, Mt. Hope, St. Michael, Sidman, Allendale, Beaverdale, Lloydell, Dunlo, Llanfair and Richland Township/Cambria County.
Project Description: To construct a wastewater interceptor system with placement of 126,700 linear feet of sewer line, 423 manholes, and 20 stream crossings in connection with section 313 of P. L. 102-58, ''Water Resources Development Act of 1992'' titled ''South Central, PA Environmental Restoration, Infrastructure and Resource Protection Development Pilot Program.'' Upon completion, the regional system will be operated by the Forest Hills Municipal Authority.
Final Action on Request: The certification granted, is subject to the following conditions:
1. A Water Quality Management Permit for the collection, conveyance and treatment system must be secured from the Department of Environmental Protection, Water Management, Southwest Regional Office, 400 Waterfront Drive, Pittsburgh, PA 15222-4745.
2. A Water Obstruction and Encroachment Permit may be required for any proposed structures constructed in the floodway or floodplain of streams or rivers. Contact the Department of Environmental Protection, Soils and Waterways Section, Water Management, Southwest Regional Office, 400 Waterfront Drive, Pittsburgh, PA 15222-4745.
3. A Bureau of Dams and Waterway Management General Permit will be required for proposed stream crossings, as applicable, from the Cambria County Conservation District, P. O. Box 187, Ebensburg, PA 15931.
4. A NPDES Permit for Discharges of Stormwater from Construction Activities must be secured from the Cambria County Conservation District, P. O. Box 187, Ebensburg, PA 15931.
