Pennsylvania Blue Shield; Experience Rated OptiChoice Vision Contract Option (Form: OPTI-INBO); Filing No. 96010000

   Pennsylvania Blue Shield submitted a new experience rated OptiChoice Vision Care Contract option as filing no. 96010000. This filing would create a vision care program wherein benefits would be payable only if rendered by an OptiChoice network provider.

   Copies of the filing are available for public inspection on Monday, Wednesday and Friday, by appointment, during normal working hours at the Insurance Department's Office in Harrisburg. This filing consists of 39 pages.

   Interested parties are invited to submit written comments, suggestions or objections to Rick Stoner, Insurance Department, Bureau of Accident and Health Insurance, 1311 Strawberry Square, Harrisburg, PA 17120, within 30 days after publication of this notice in the Pennsylvania Bulletin.
