APPROVALS TO USE NPDES AND/OR OTHER GENERAL PERMITS
The following parties have submitted Notices of Intent (NOIs) for Coverage under (1) General NPDES Permit(s) to discharge wastewater into the surface waters of this Commonwealth. The approval for coverage under these general NPDES permits is subject to applicable effluent limitations, monitoring, reporting requirements and other conditions set forth in the general permit; (2) General Permit(s) for Beneficial Use of Sewage Sludge or Residential Septage by Land Application in Pennsylvania. LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

Preamble 3 The following final report was submitted to the Department of Environmental Protection under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908).
Provisions of Chapter 3 of the Land Recycling and Environmental Remediation Standards Act (act) require the Department of Environmental Protection to publish in the Pennsylvania Bulletin a notice of submission of any final reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the act's remediation standards. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected.

Maple Creek Mining, Inc. The Bureau of Deep Mine Safety has approved a request for variance from Maple Creek Mining, Inc. The following notification contains a summary of this request. Complete copies of the variance request may be obtained from Matthew A. Bertovich by calling (412) 439-7469.
Summary of the Request: Maple Creek Mining has requested a variance to allow for the common ventilation of belt conveyor entries with other entries at the Maple Creek Mine.
Summary of the Decision: The basis for the Bureau's determination for approval can be summarized in the following statements:
1. The area covered by the variance was previously a double track haulage. By installing the belt in ''C'' track entry, additional needed intakes and return entries have been provided. Also, the belt is totally isolated from the adjacent track/trolley entry.
2. The main intake entries are pressurized, and the main source of ventilation to the working sections.
3. An early warning fire detection system is provided.
The proposed system will minimize the amount of air on the belt entryway necessary for effective ventilation and will still enable the belt entryway to act as an escapeway from the face to the main air current. The entryway being ventilated in common with the belt entryway is neutral and will not contain potential fire sources.

ENCROACHMENTS The Department of Environmental Protection has taken the following actions on previously received Dam Safety and Encroachment permit applications, requests for Environmental Assessment approval and requests for Water Quality Certification under section 401 of the Federal Water Pollution Control Act (33 U.S.C.A. 1341(a)).
Persons aggrieved by this action may appeal, under section 4 of the Environmental Hearing Board Act (35 P. S. § 7514) and the Administrative Agency Law, 2 Pa.C.S. E03-363. Encroachment. Armstrong County Commissioners, Administration Building, 450 Market Street, Kittanning, PA 16201. To remove the existing structure and to construct and maintain a single span bridge having a normal span of 102.0 feet and an underclearance of 7.5 feet across Pine Run for the purpose of improving transportation safety. The project is located on T-771 Station 4+51.75 (Distant, PA Quadrangle N: 15.5 inches; W: 0.9 inch) in Redbank Township, Armstrong County.
E03-364. Encroachment. PA Department of Transportation, P. O. Box 429, Indiana, PA 15701. To remove the existing bridges and to construct and maintain three single span, prestressed concrete spread box beam bridges over Garretts Run (WWF). Bridge No. 1 having a normal span of 46.0 feet and an underclearance of 8.7 feet; Bridge No. 2 having a normal span of 50.0 feet and an underclearance of 9.1 feet; and Bridge No. 3 having a normal span of 36.0 feet and an underclearance of 9.8 feet. During construction, temporary stream diversions will be maintained in the stream channel. The project is located along SR 2025 between 1,000 feet and 2,000 feet southeast of its intersection with SR 0066 (SR 2025, Section 155) (Kittanning, PA Quadrangle, Bridge No. 1 N: 7.2 inches; W: 1.7 inches, Bridge No. 2 N: 7.4 inches; W: 1.7 inches, Bridge No. 3 N: 7.6 inches; W: 1.8 inches) in Manor Township, Armstrong County.
E30-180. Encroachment. PA Department of Transportation, P. O. Box 459, Uniontown PA 15401. To remove the existing structures and to construct and maintain two box culverts in Garner Run. Culvert #1 will be a single cell reinforced concrete box culvert having a normal span of 14.0 feet and an effective underclearance of 4.9 feet (invert depressed 1 foot below streambed) located on SR 0018, Section K00, Station 265+06.05 (Holbrook, PA Quadrangle N: 12.4 inches; W: 12.1 inches). Culvert #2 will be a single cell reinforced concrete box culvert having a normal span of 18.0 feet and an effective underclearance of 7.2 feet (invert depressed 1 foot below streambed) located on SR 0018, Section K01, Station 252+91.69 (Holbrook, PA Quadrangle N: 13.0 inches; W: 12.2 inches). This permit also authorizes the placement and maintenance of fill in a de minimis area of wetlands less than or equal to 0.05 acre, the relocation and maintenance of 63.0 feet of Garner Run downstream from Culvert #1 and the placement and maintenance of rock riprap bank protection along 25.0 feet of Garner Run upstream from Culvert #2 for the purpose of widening SR 0018. The project is located just north of the Village of White Cottage in Jackson Township, Greene County.
E56-277. Encroachment. Summit Township, P. O. Box 27, Meyersdale, PA 15552. To construct and maintain two 6.0-foot RCP culverts in Miller Run with the invert of one culvert depressed 6 inches below the stream bed for the purpose of accessing proposed businesses located off SR 219 across from Meyersdale High School (Meyersdale, PA Quadrangle N: 8.25 inches; W: 5.4 inches) in Summit Township, Somerset County. This permit was issued under section 105.13(e) Small Projects. This permit also includes 401 Water Quality Certification.
E63-428. Encroachment. PA Department of Transportation, P. O. Box 459, Uniontown, PA 15401. To remove the existing dual bridges and to construct and maintain new dual three-span bridges having clear spans of 80.5 feet, 83.0 feet and 52.5 feet with an underclearance of 36.5 feet across Tenmile Creek (TSF) and to extend and maintain the existing retaining wall on the left bank of said stream for a distance of 35.0 feet downstream. The project is located on I-79 near the Marianna Exit approximately 1.3 miles north of the Greene County--Washington County boundary line (Amity, PA Quadrangle N: 3.2 inches; W: 7.1 inches) in Amwell Township, Washington County. This permit also authorizes the construction and maintenance of a temporary causeway and stream diversion structures. This permit was issued under section 105.13(e) Small Projects. To construct and maintain a bridge (Wyit Sprowls Bridge) having a span of 43.0 feet with an underclearance of 9.5 feet across Templeton Fork located on the east side of T-414 approximately 500 feet east from the intersection of T-410 and T-414 (Claysville, PA Quadrangle N: 7.0 inches; W: 4.0 inches) in East Finley Township, Washington County. This permit was issued under section 105.13(e) Small Projects. This permit also includes 401 Water Quality Certification.

To construct and maintain the following structures on two unnamed tributaries to Wilson Run as part of a service plaza parking area expansion and rehabilitation: a 22-foot extension to the existing 42-inch RCCP on the deceleration ramp station, 13+07.5; a 52-foot extension to the existing 7-foot × 10-foot CMP arch on the truck ramp, Station 24+50; to install and maintain approximately 110 L.
