Permits issued for Construction Permit under the Pennsylvania Safe Drinking Water Act (35 P. S. §§ 721.1--721.17).

This permit authorizes the elimination of the existing 5,000 gallon inground concrete tank (wet sump) and related chlorine dioxide injection point and the rerouting of chlorinated Well No. 3 water directly to the existing 500,000 gallon storage tank.

The following final reports were submitted under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908).
Provisions of Chapter 3 of the Land Recycling and Environmental Remediation Standards Act (act) require the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin a notice of submission of final reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the act's remediation standards. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected.
For further information concerning the final report, contact the Environmental Cleanup Program Manager in the Department Regional Office under which the notice of receipt of a final report appears. If information concerning a final report is required in an alternative form, contact the community relations coordinator at the appropriate regional office listed. The report is intended to document remediation of the site to meet the Statewide health standard. See additional information in another section of this Pennsylvania Bulletin.
Janet Gass Residence, City of Sunbury, Northumberland County. EarthRes Group, Inc. on behalf of their client, Janet Gass, 805 Market Street, Sunbury, PA 17801, have submitted a Final Report concerning the remediation of site soil contaminated with BTEX and PAHs. The Final Report is intended to document remediation of the site to meet the Statewide health standard.
Herman Banks Scrap Yard, West Chillisquaque Township, Northumberland County. A Final Report was submitted on December 21, 1998 concerning the remediation of site soil contaminated with PCBs, heavy metals, PHCs and PAHs. The Final Report was submitted to document remediation of site-specific standards. The Department on January 20, 1999 approved the Final Report and the approval of the Final Report was published in the Pennsylvania Bulletin on February 20, 1999.

The Department has taken action on the following plans and reports under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908) and Chapter 250 Administration of Land Recycling Program.
Provisions of 25 Pa. Code § 250.8 Administration of Land Recycling Program requires the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin a notice of its final actions on plans and reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the remediation standards of the Land Recycling and Environmental Remediation Standards Act (act). Plans and reports required by provisions of the act for compliance with selection of remediation to a site-specific standard, in addition to a final report, include a remedial investigation report, risk assessment report, and cleanup plan. A remedial investigation report includes conclusions from the site investigation, concentration of regulated substances in environmental media, benefits or reuse of the property, and in some circumstances, a fate and transport analysis. If required, a risk assessment report describes potential adverse effects caused by the presence of regulated substances. A cleanup plan evaluates the abilities of potential remedies to achieve remedy requirements. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected. The Department may approve or disapprove plans and reports submitted. This notice provides the Department's decision and, if relevant, the basis for disapproval.

The final report demonstrated attainment of the Statewide health standard, and was approved by the Department on July 28, 2000.
Burle Industries, Inc., Lancaster City, Lancaster County. A Final Report has been submitted concerning the remediation of site groundwater contaminated with solvents. The Final Report demonstrated attainment of the site-specific standard, and was approved by the Department on November 9, 1998.
Former Murata Electronics NA, Inc., CREDC Property, Carlisle Borough, Cumberland County. A Remedial Investigation Report and Final Report were submitted concerning remediation of site soils contaminated with solvents. The Remedial Investigation Report was approved September 4, 1998. The Final Report demonstrated attainment of the site-specific standard, and was approved by the Department on September 4, 1998.

One Montgomery Associates, Norristown Borough, Montgomery County. A Final Report concerning remediation of site soils contaminated with PCBs was submitted to the Department. The report demonstrated attainment of the Statewide health standard and was approved by the Department on October 6, 1998.
Boeing Company, Ridley Township, Delaware County. A Final Report concerning remediation of site groundwater contaminated with solvents was submitted to the Department. The report demonstrated attainment of the Statewide health standard and was approved by the Department on October 9, 1998.
Gilbert's Flower Shop Property (former), Ridley Township, Delaware County. A Final Report concerning remediation of site soils and groundwater contaminated with petroleum hydrocarbons was submitted to the Department. The report demonstrated attainment of the Statewide health standard and was approved by the Department on November 25, 1998.

A Final Report has been submitted addressing soil contaminated with PCBs (polychlorinated biphenyls). The report documented remediation to meet the Statewide health standard. The Department approved the report on October 8, 1998.
USA Waste Management, West Chillisquaque Township, Northumberland County. Wade Fuchs, Waste Management, Inc., R. D. 2, Box 282A, Pleasant Valley Road, Irwin, PA 15642 has submitted a Final Report on October 26, 1998, showing attainment of the Statewide health standard for all constituents of diesel fuel resulting from a diesel spill at a truck accident involving USA Waste Management on August 10, 1998. The Final Report was approved by the Department on November 20, 1998.
Ann Wilson Residence, City of Williamsport, Lycoming County. Response Environmental, Inc. on behalf of their client, Ann Wilson, 2011 Linwood Avenue, Williamsport, PA 17701, has submitted a Final Report concerning the remediation of site soil contaminated with BTEX and PAHs. The Final Report demonstrated attainment of the Statewide health standard and was approved by the Department on July 31, 2000.

Walsh, ARCO Chemical Company, 3801 West Chester Pike, Newtown Square, PA 19073, has submitted a Remedial Investigation Report, Baseline Risk Assessment Report, and Cleanup Plan concerning remediation of site soils, groundwater and surface water contaminated with polycyclic aromatic hydrocarbons, petroleum hydrocarbons, benzene, toluene, ethylbenzene and xylene. The Department on October 9, 1998 approved the Remedial Investigation Report, Baseline Risk Assessment Report and Cleanup Plan.
Tom Morris Residence, Perry Township, Fayette County. Kevin P. Van Kuren, Hydrocon Services, Inc., 2945 South Pike Avenue, Allentown, PA 18103, has submitted a Remedial Investigation Report, Baseline Risk Assessment Report, Cleanup Plan and Final Report concerning remediation of site soil contaminated with benzene, toluene, ethylbenzene, xylene and polycyclic aromatic hydrocarbons. The Remedial Investigation Report, Baseline Risk Assessment Report, Cleanup Plan and Final Report demonstrated attainment of the site-specific standard and was approved by the Department on October 22, 1998.
Johnstown Corporation, Johnstown, Cambria County. Brian I. Fitzpatrick, P.G., QST Environmental, 5205 Militia Hill Road, Plymouth Meeting, PA has submitted a Final Report concerning remediation of site soil and groundwater contaminated with petroleum hydrocarbons. The Final Report demonstrated attainment of the Statewide health standard and was approved by the Department on October 14, 1998.

The site has been found to be contaminated with PAHs and Cutting Oil. A permit modification was issued to modify the closure plan for the City's Philadelphia Northeast Airport sewage sludge trenching site. The modification calls for onsite sewage sludge and contaminated soils to be removed from unlined trenches and disposed in a new, lined landfill that will be constructed on the site. The new landfill and waste reburial project will consolidate the permitted area from approximately 120 acres to 36 acres. The remaining acreage will be commercially developed after the waste has been removed. The permit was modified by the Southeast Regional Office on August 2, 2000.
