INDIVIDUAL PERMITS

APPROVALS TO USE NPDES AND OTHER GENERAL PERMITS The following parties have submitted (1) Notices of Intent (NOIs) for coverage under General NPDES Permit(s) to discharge wastewater into the surface waters of this Commonwealth; (2) NOIs for coverage under General Permits for Beneficial Use of Sewage Sludge or Residential Septage by Land Application in Pennsylvania; or (3) Notifications for First Land Application of Sewage Sludge.
The approval of coverage under these General Permits may be subject to one or more of the following: pollutant or effluent discharge limitations, monitoring and reporting, pathogen and vector attraction reduction requirements, operational standards, general requirements, management practices and other conditions set forth in the respective general permit. The Department of Environmental Protection has reviewed the NOIs and determined that they comply with administrative requirements of the respective permit application. Also, the Department has evaluated the First Land Application of Sewage Sludge for the sites applying for coverage under PAG-7, PAG-8 and PAG-9 and determined that the sites are suitable for land application of sewage sludge.
The EPA Region III Regional Administrator has waived the right to review or object to this permit action under the waiver provision: 40 CFR 123.24.
The application and related documents, effluent limitations, permitting requirements and other information are on file and may be inspected and arrangement made for copying at the contact office noted.

HAZARDOUS SITES CLEANUP
Under the Act of October 18, 1988
Notice of Settlement under the Hazardous Sites Cleanup Act.
Montgomery County The Department of Environmental Protection (Department) has entered into a settlement with 152 Mathers Associates, LLC (Mathers) relating to the Chemigraphics HSCA Site (site) in the Department's Southeast Region. The site is located at 152 Mathers Road, Whitpain Township, Montgomery County, PA, and was formerly used for a variety of light industrial and commercial purposes which resulted in the release of trichloroethylene (TCE) and other hazardous substances onto the ground surface at the site.
Mathers is the first mortgage holder to the site and intends to foreclose on its mortgage and redevelop the site for industrial/commercial purposes. Under the terms of the settlement, Mathers will remit $3,000 to the Department toward its response costs at the site in exchange for a covenant not to sue from the Department and contribution protection from third parties.
This notice is provided under section 1113 of HSCA, 35 P. S. § 6020.1113. Copies of the proposed Consent Order and Agreement are in the possession of the Department and may be examined at the Department's offices at Lee Park, Suite 6010, Conshohocken, PA. Interested persons should contact George Danyliw at (610) 832-5967 or Anderson Lee Hartzell, Esquire, at (610) 832-6300 during normal business hours. The Department will provide a 60-day public comment period from the date of this notice. Persons may submit comments to George Danyliw at the noted address. The Department will provide a response to all significant comments received during the public comment period, and the proposed consent Order and Agreement authorizes the Department to withdraw or withhold its consent in the event that the Department determines, based upon the comments received during the public comment period, that the Consent Order and Agreement is inappropriate, improper, or not in the public interest.
LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

Preamble 2
The following final reports were submitted under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908).
Provisions of Chapter 3 of the Land Recycling and Environmental Remediation Standards Act (act) require the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin a notice of submission of final reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the act's remediation standards. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected.
For further information concerning the final report, contact the Environmental Cleanup Program Manager in the Department's Regional Office under which the notice of receipt of a final report appears. The report is intended to document remediation of the site to the site-specific standard.

The Department has taken action on the following plans and reports under the Land Recycling and Environmental Remediations Standards Act (35 P. S. §§ 6026.101--6026.908) and Chapter 250 Administration of Land Recycling Program.
Provisions of 25 Pa. Code § 250.8 Administration of Land Recycling Program requires the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin a notice of its final actions on plans and reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the remediation standards of the Land Recycling and Environmental Remediations Standards Act (act). Plans and reports required by provisions of the act for compliance with selection of remediation to a site-specific standard, in addition to a final report, include a remedial investigation report, risk assessment report and cleanup plan. A remedial investigation report includes conclusions from the site investigation, concentration of regulated substances in environmental media, benefits of reuse of the property, and in some circumstances, a fate and transport analysis. If required, a risk assessment report describes potential adverse effects caused by the presence of regulated substances. A cleanup plan evaluates the abilities of potential remedies to achieve remedy requirements. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected. The Department may approve or disapprove plans and reports submitted. This notice provides the Department's decision and, if relevant, the basis for disapproval.

Village Center Mart (Hashas Cleaners), Bensalem Township, Bucks County. Charlene R. Drake, React Environmental Services Inc., 6901 Kingsessing Avenue, Philadelphia, PA 19142, has submitted a Remedial Investigation Report concerning remediation of site soil and groundwater contaminated with solvents. The report was approved by the Department on November 30, 1999.
Philadelphia Regional Port Authority Pier 80 South Annex, City of Philadelphia, Philadelphia County. James G. Gallagher, P.E., Pennoni Associates, Inc., 950 Clifton Avenue, Clifton, NJ 07013-2790, has submitted a Final Report concerning remediation of site soil and groundwater contaminated with BTEX, and polycyclic aromatic hydrocarbons. The report did not demonstrate attainment of the Statewide health standard and was disapproved by the Department on December 13, 1999.
Philadelphia Regional Port Authority Tioga Maintenance Building and Carpenters Shop, City of Philadelphia, Philadelphia County. James G. Gallagher, P.E., Pennoni Associates, Inc., 950 Clifton Avenue, Clifton, NJ 07013-2790, has submitted a Final Report concerning remediation of site soil and groundwater contaminated with lead, BTEX, and polycyclic aromatic hydrocarbons. The report did not demonstrate attainment of the Statewide health standard and was disapproved by the Department on December 13, 1999.
Connelly Paper Mill (Former), City of Philadelphia, Philadelphia County. Darryl A. Borrelli, Manko, Gold & Katcher, LLP, 401 City Avenue, Suite 500, Bala Cynwyd, PA 19004, has submitted a Final Report concerning remediation of site soil and groundwater contaminated with lead, heavy metals, BTEX, petroleum hydrocarbons and polycyclic aromatic hydrocarbons. The report demonstrated attainment of the Statewide health standard and was approved by the Department on December 16, 1999.
Unisys Downingtown Facility (Former), Downingtown Borough, Chester County. Randy L. Shuler, Environmental Resources Management, 250 Phillips Blvd., Suite 280, Ewing, NJ 08618, has submitted a Final Report concerning remediation of site soil and groundwater contaminated with heavy metals and solvents. The report demonstrated attainment of the Statewide health and site-specific standards and was approved by the Department on December 15, 1999.
Upper Nike Property, Schuylkill and Tredyffrin Townships, Chester County. Joseph W. Standen, Jr., Leggette, Brashears & Graham, Inc., 426 Brandywine Parkway, West Chester, PA 19380, has submitted a Final Report concerning remediation of site soil contaminated with petroleum hydrocarbons. The report demonstrated attainment of the Statewide health standard and was approved by the Department on December 16, 1999.
Madonna Management Property, Bensalem Township, Bucks County. Thomas J. Brady, IV, RT Environmental Services, Inc., Suite 306, Pureland Complex, 510 Heron Drive, Bridgeport, NJ 08014, has submitted a Remedial Investigation/Final Report concerning remediation of site soil and groundwater contaminated with petroleum hydrocarbons. The report demonstrated attainment of the Statewide health and site-specific standards and was approved by the Department on December 20, 1999.
Estate of Martin Spinelli, Tredyffrin Township, Chester County. William G. Murray, URS Greiner Woodward Clyde, 1400 Union Meeting Road, Suite 202, Blue Bell, PA 19422-1972, has submitted a Final Report concerning remediation of site soil contaminated with lead and BTEX. The report demonstrated attainment of the Statewide health standard and was approved by the Department on December 22, 1999.
Spring Mill Estates North Parcel Site II, Upper Providence Township, Montgomery County. Mark Fortna, DelVal Soil & Environmental Consultants, Inc., Sky Run II Suite A1, 4050 Skyron Drive, Doylestown, PA 18901, has submitted a Final Report concerning remediation of site soil contaminated with pesticides. The report demonstrated attainment of the Statewide health standard and was approved by the Department on December 22, 1999.

Bahr Residence, Bushkill Township, Northampton County. William K. Ahlert, Manager, Lawler, Matusky & Skelly Engineers, 609 Hamilton Mall, Allentown, PA 18101, submitted a Final Report (on behalf of his client, Brian Bahr, 791 Golden Eagle Drive, Nazareth, PA 18091) concerning the remediation of site soils found to have been contaminated with petroleum hydrocarbons. The report demonstrated attainment of the Statewide health standard and was approved on December 31, 1999.

Box 312, Coudersport, PA 16915 has submitted a Baseline Environmental Report concerning the remediation of site soil contaminated with heavy metals, solvents and PAHs, and groundwater contaminated with lead and heavy metals. The report was approved by the Department on December 16, 1999.
