Clarks Summit State Hospital is a major facility subject to Title V permitting requirements. The modification involves installation of a scrubber system to eliminate the opacity problem from the ash handling system. Particulate emissions from this system will be 652 pounds per year. The Plan Approval will contain conditions requiring the source to show compliance with the emission rates. The Plan Approval and Operating Permit will contain additional record keeping and operating restrictions designed to keep the facility operating within all applicable air quality requirements.
40-399-036A: Johns Manville International (Valmont Industrial Park, 600 Jaycee Drive, Hazleton, PA 18201) for modification of an insulation foam process (blowing agent) and cricket saw in Hazle Township, Luzerne County. This facility is a non-Title V facility. Modification of this line will replace the existing blowing agents with a new agent and the expected VOC emissions from the facility will be under 15 tons per year, which are controlled by the use of a RTO. Expected particulate emissions will be less than 0.04 grain/dscf. The company will monitor and record the combustion chamber temperature in the RTO and maintain the system in accordance with the good engineering practices to assure proper operation of the system.
45-318-030A: Custom Design and Manufacturing Co., Inc. (Route 940 and Harvest Lane, P. O. Box 216, Pocono Summit, PA 18346) for installation of the Cattinair Spray Booth and woodworking dust collector in Tobyhanna Township, Monroe County. This facility is a non-Title V facility. Installation of this line will replace the existing spray booth and the expected VOC emissions from the facility will remain under 25 tons per year. Expected particulate emissions will be less than 0.04 grain/dscf from the painting operations and less than 0.02 grain/dscf from the wood working baghouse. The company will monitor the pressure drop across the baghouse and maintain the system in accordance with the good engineering practices to assure proper operation of the system.
48-309-115: Essroc Cement Corp. (3251 Bath Pike, Nazareth, PA 18064-8928) for installation of an air cleaning device (fabric collector) for the coal/clinker/gypsum unloading hopper in Lower Nazareth Township, Northampton County. This installation will result in a particulate emission rate of 6.0 tons per year. The Plan Approval and Operating Permit will contain additional recordkeeping and operating restrictions designed to keep the facility operating within all applicable air quality requirements. The facility currently has a Title V Operating Permit No. 48-00004. This plan approval will, in accordance with 25 Pa. Code § 127.450, be incorporated into the Title V Operating Permit through an administrative amendment at a later date.
54-399-016C: World Resources Co. (170 Walnut Lane, Pottsville, PA 17901) for modification of a metals reclamation process and associated air cleaning device in Norwegian Township, Schuylkill County. The industrial recycling facility is a non-Title V (State only) facility. The modification will allow the plant to utilize a spin flash dryer to recycle metal-bearing wastes. Particulate matter emissions from the process will be controlled through the use of a Venturi scrubber and existing packed towers and will be less than 8.8 tons per year. The approval will include monitoring, record keeping and reporting requirements designed to keep the facility operating within all applicable air quality requirements.
48-303-005C: Highway Materials Inc. (1750 Walton Road, Blue Bell, PA 1942-0465) for modification of an asphalt plant to allow for the combustion of alternate fuels, specifically to allow the use recycled fuel oil, virgin oil, #2 fuel oil, #4 fuel oil and natural gas at the facility in Blue Bell, Northampton County. This facility is a non-Title V facility. Modification will not increase the existing emission rates for the pollutants. The installation of new burner as a part of the modification will actually reduce the actual emission rates. The modification will provide the facility necessary flexibility with respect to the uncertainty of future fuel cost. The company will operate the facility and maintain the system in accordance with the good engineering practices to assure proper operation of the system.

06-05024A: Reliant Energy Mid-Atlantic Power Holdings, LLC (1001 Broad Street, Johnstown, PA 15907-1050) for modification of a coal handling operation controlled by covers and wet suppression at the Titus Power Plant in Cumru Township, Berks County. The facility is subject to Title V. This modification will have the potential to emit 25 tons per year of particulate. The net increase in the potential emissions is 10 tons per year. The applicant will be required to conduct monitoring, recordkeeping and reporting designed to keep the source operating within all applicable air quality requirements. The plan approval will be added to the Title V operating permit through an administrative amendment at a later date.
36-05117: Dart Container Corp. of Pennsylvania (110 Pitney Road, Lancaster, PA 17602), for construction of an Expandable Polystyrene (EPS) Cup Manufacturing facility and two boilers to control emissions of pentane in East Lampeter Township, Lancaster County.
Based on the information provided by the Applicant and DEP's own analysis, this operation has the potential-to-emit 313.97 tons per year (tpy) of volatile organic compounds (VOCs) emissions. This is a major facility subject to the requirements of 25 Pa. Code Chapter 127, Subchapter E (relating to new source review). The company will be required to purchase VOC emission reduction credits (ERCs) in the amount of 406.72 tpy. This equals 9.62 tpy of potential VOC flue emissions offset at a ratio of 1.15 to 1 and 304.35 tpy of potential fugitive emissions offset at a ratio of 1.30 to 1. As required by 25 Pa. Code § 127.206(d)(2), the company shall not commence operations or increase emissions from the proposed construction until all of the required ERCs have been purchased and documented by the Department.
The facility will be subject to the following NSR Plan Approval conditions:
General Requirements
1. This plan approval constitutes a New Source Review (NSR) and Lowest Achievable Emission Rate (LAER) determination for volatile organic compound emissions as per 25 Pa. Code Subchapter E.
2. ERCs shall be purchased to offset the total of the net increase in the potential-to-emit VOC emissions in accordance with 25 Pa. Code § 127.205(4).
3. The two Cleaver Brooks CB-800 boilers are subject to the Standards of Performance for Small Industrial-Commercial-Institutional Steam Generating Units, of 40 CFR Part 60, Subpart Dc.
Emission Limitations
4. The capture efficiency of the control system shall be equal to or greater than 90%. All captured VOC emissions from the blenders, holding tanks and pre-expanders shall be vented to the boilers and reduced at a minimum destruction efficiency of 95%, as pentane, at the outlet.
5. The VOC emissions from the facility shall be limited to 313.97 tons per year based on a throughput limitation of 11,500 tons of expanded polystyrene during any consecutive 12-month rolling period.
6. In accordance with 25 Pa. Code § 127.205(3), the permittee shall purchase ERCs in the amount of 406.72 tons. This equals 9.62 tons of potential VOC flue emissions offset at a ratio of 1.15 to 1 and 304.35 tons of potential fugitive emissions offset at a ratio of 1.30 to 1. As required by 25 Pa. Code § 127.206(d)(2), the permittee shall not commence operations or increase emissions from the proposed construction until all of the required ERCs have been purchased and documented by the Department.
7. Each of the Cleaver Brooks CB-800 boilers shall meet the opacity requirements of 40 CFR 60.43c(c) or the limitations of 25 Pa. Code § 123.41, whichever is more stringent.
8. Each of the Cleaver Brooks CB-800 boilers shall not emit particulate matter into the outdoor atmosphere in excess of 0.4 pound per million Btu of heat input.
9. No person shall permit the emission into the outdoor atmosphere of sulfur oxides, expressed as SO2, from each of the boilers, in excess of the rate of 4 pounds per million Btu of heat input over a 1-hour period.
10. The sulfur content of the No. 2 fuel oil shall not exceed 0.5% by weight.
11. Each of the boilers NOx and CO emissions shall be equal to or less than:
30 ppmdv NOx at 3% O2 when firing natural gas
90 ppmdv NOx at 3% O2 when firing No. 2 fuel oil
400 ppmdv CO at 3% O2Operating Requirements
12. The two Cleaver Brooks CB-800 boilers shall be fired on natural gas and No. 2 fuel oil only.
13. The permittee shall install, operate and maintain devices to measure and record the fuel usage to each boiler and the pentane concentration and airflow rate of the captured air stream.
14. The permittee shall install, operate and maintain devices to measure and record the pressure drop across the filter and flame arrestor.
15. The capture system shall be operational at all times including start-up and shutdown of operations.
16. The monitoring devices in Condition #13 and #14 shall be calibrated in accordance with the manufacturer's specifications.
Testing Requirements
17. Within 60 days after achieving the maximum production rate, but no later than 180 days after initial start-up, the owner or operator shall conduct a performance test to determine the destruction efficiency of the two boilers. The tests shall be performed in accordance with the latest revision of the Department's Source Testing Manual and 25 Pa. Code Chapter 139. The tests shall be performed at or near maximum production rates in the measurement of actual VOC emissions.
18. Within 60 days after achieving the maximum production rate, but no later than 180 days after initial start-up, the owner or operator shall conduct a Capture Efficiency test of the preexpander capture system in accordance with EPA's ''Guidelines for Determining Capture Efficiency,'' dated January 9, 1994, and EPA's Method 204.
19. A test protocol including a sketch with dimensions including the location of sampling ports and other data to ensure the collection of representative samples shall be submitted to the Regional Air Quality Program Manager. A test protocol shall be submitted for both the performance testing of the boilers and the CE testing of the preexpander capture system. Two copies of each of the test protocols shall be submitted to the Department at least 60 days prior to testing.
20. The permittee shall notify the Department at least 2 weeks in advance of the date and time of the tests, so that an observer may be present.
21. Two copies of the test reports shall be submitted to the Regional Air Quality Program Manager for review within 60 days of the completion of the tests
22. The permittee shall repeat the test for VOC destruction efficiency at least 180 days prior to expiration of the facility's operating permit. The results of the tests shall be included with the permit renewal application.
Monitoring Requirements
23. The permittee shall monitor and record the following:
Daily EPS throughput rate
Daily fuel usage to each of the boilers
Monthly ink usage
Monthly usage of clean-up solvents
Concentration and flow rate of the captured air ducted to the boilers24. The permittee shall install, operate and maintain a recording device to continuously monitor and record the flow rate of the emission capture system, the pentane concentration in the air stream and the pentane flow rate.
Reporting and Record Keeping Requirements
25. The permittee shall comply with the record keeping and certification requirements in accordance with 40 CFR 60.46c(e), 60.42c(h) and 60.48c(f)(1).
26. Within 60 days after initial start-up, the permittee shall submit a Phase I application in accordance with the latest revision of the Continuous Source Monitoring Manual for certification of the pentane analyzers.
27. The permittee shall maintain daily fuel consumption records for each of the boilers in accordance with 40 CFR 60.48c(g). Records shall be kept for the fuel firing rates of the combustion units on a monthly basis in order to determine sulfur dioxide Emissions in accordance with 40 CFR 60.48c(d).
28. The permittee shall calculate all VOC emissions associated with the cup manufacturing process, printing operations, boiler usage and clean-up solvents, on a monthly basis. In order to assure compliance with the limitation of condition 5, the monthly emissions shall be summed with the previous 11 months to determine the 12-month rolling total.
29. Calculated monthly VOC emissions shall be submitted to the Department on a quarterly basis.
30. The permittee shall maintain all monitoring records at the facility for a period of 5 years and be made available to the Department upon request.
31. Semiannual reports for the two Cleaver Brooks boilers, shall be submitted in accordance with 40 CFR 60.48c(d) and 60.48c(e)(11).
32. Under 40 CFR 60.4, the permittee shall submit copies of all requests, reports, applications, submittals and other communications to both the EPA and the Department. All correspondence shall be sent to the following:

6. The 22.00 tpy of VOC ERCs previously registered to Mace Security International were generated from the shutdown of sources at the Saltsburg facility in Indiana, PA on October 3, 1995. The Department certified and registered the 22.00 tpy of VOC ERCs on November 1, 1997.
7. The 30.00 tpy of VOC ERCs previously registered to Kem Plastic Playing Cards, Inc., were generated from the change over to powder coatings at the Poughkeepsie facility in New York on July 1, 1991.
8. The 43.50 tpy of VOC ERCs previously registered to Prestolite Electric Inc., were generated from the shutdown of sources at the Arcade facility in New York on August 8, 1997.
9. The 171.48 tpy of VOC ERCs previously registered to Crown Cork & Seal Company, Inc., were generated from the shutdown of sources at the Philadelphia facility in Pennsylvania during the period of 1992 to 1998. The Department certified and registered the 171.48 tpy of VOC ERCs on May 5, 1999.
10. The 146.18 tpy of VOC ERCs previously registered to Avery Dennison Corporation were generated from the shutdown of sources at the Quakertown facility in Bucks County, PA on August 1, 1996. The Department certified and registered the 146.18 tpy of VOC ERCs on November 7, 1997.
11. The offsetting VOC ERCs identified in this plan approval are approved for use at Dart Container of Pennsylvania's Leola facility operated in Upper Leacock Township, Lancaster County. This approval is in accordance with the requirements of 25 Pa. Code Chapter 127, Subpart E (relating to new source review) including §§ 127.205(4), 127.210 and 127.211.
12. Under 25 Pa. Code § 127.208(2), upon the issuance of this plan approval, the 722.00 tpy of VOC ERCs not generated by the over-control of emissions are no longer subject to the 10-year expiration date under § 127.206(f), except as specified in § 127.206(g). If the VOC ERCs identified in this plan approval are not used and are subsequently reentered into the ERC registry, the applicable 10-year expiration date for ERCs generated from the shutdown of sources will not be extended.
13. This plan approval is granted solely for the transfer and use of ERCs according to 25 Pa. Code § 127.208(2) and does not authorize a person to construct, modify or reactivate an air contaminant source or allow for the installation of an air cleaning device on an air contaminant source.
36-05112: PPL Global LLC, (Suite 400, 11350 Random Hills Road, Fairfax, VA 22030) for the construction of ten gas fired simple cycle combustion turbines nominally rated at 45 megawatts each for electrical generation located in West Earl Township, Lancaster County.
The project is subject to the applicable requirements of 40 CFR Part 60, Subpart GG, Standards of Performance for Stationary Gas Turbines, 40 CFR Parts 72, 73, 75 and 77 and the Oxides of Nitrogen (NOx) Budget Program established at 25 Pa. Code §§ 123.102--123.121 and Chapter 145. Appropriate requirements for restrictions, testing, monitoring, record keeping, reporting and work practice standards relating to Subpart GG and all other applicable air quality requirements have been incorporated in the plan approval.
Facility emissions of NOx, sulfur dioxide (SO2), carbon monoxide (CO) and particulate matter (PM) shall be less than 100 tons per year (each) during any consecutive 12-month period. Facility emissions of volatile organic compounds (VOC) shall be less than 50 tons per year (TPY) during any consecutive 12-month period. Facility emissions of aggregate Hazardous Air Pollutants (HAP) and any individual HAP shall be less than 25 TPY and 10 TPY, respectively, during any consecutive 12-month period. Ammonia slip will be limited to 10 ppm maximum. The electrical generating units shall be operated using only natural gas. The sulfur content of the natural gas shall not exceed 1.5 grains /100 dry standard cubic foot. Operating at full load at an approximate annual average ambient temperature of 59°F, emissions per turbine for NOx, CO, VOC, PM and SO2, shall not exceed 8.2, 7.4, 3.0 7.2 and 1.9 pounds per hour, respectively. Each unit is to be equipped with a water injection system and selective catalytic reduction for the control of NOx emissions and oxidation catalyst to control CO and VOC emissions. A condition of the plan approval prohibits commencement of construction until PPL demonstrates that the project has the required change or variance to an existing comprehensive plan or zoning ordinance. A condition of the plan approval prohibits commencement of construction until PPL demonstrates that the project has the applicable zoning and building approvals. A condition of the plan approval prohibits operation of the source without a permit that states its acid rain program requirements.
67-03107: Hoover Manufacturing Co., Inc. (P. O. Box 146, Glen Rock, PA 17327) for construction of three spray paint booths controlled by dry panel filters at its woodworking facility in Springfield Township, York County. The facility is a non-Title V (State only) facility. The plan approval will allow the facility to equal or exceed 2.7 tons per year of VOC emissions during any consecutive 12-month period. MINING ACTIVITY APPLICATIONS Applications under the Surface Mining Conservation and Reclamation Act (52 P. S. §§ 1396.1--1396.19a); the Noncoal Surface Mining Conservation and Reclamation Act (52 P. S. §§ 3301--3326); The Clean Streams Law (35 P. S. §§ 691.1--691.1001); the Coal Refuse Disposal Control Act (52 P. S. §§ 30.51--30.66); The Bituminous Mine Subsidence and Land Conservation Act (52 P. S. §§ 1406.1--1406.21). Mining activity permits issued in response to such applications will also address the applicable permitting requirements of the following statutes: the Air Pollution Control Act (35 P. S. §§ 4001--4015); the Dam Safety and Encroachments Act (32 P. S. §§ 693.1--693.27); and the Solid Waste Management Act (35 P. S. §§ 6018.101--6018.1003).
The following permit applications to conduct mining activities have been received by the Department of Environmental Protection (Department). A copy of the application is available for inspection at the District Mining Office indicated above each application. Where a 401 Water Quality Certification is needed for any aspect of a particular proposed mining activity, the submittal of the permit application will serve as the request for such certification.
Written comments or objections or requests for informal conferences on applications, may be submitted by any person or any officer or head of any Federal, State or local government agency or authority to the Department at the same address within 30 days of this publication or within 30 days after the last publication of the applicant's newspaper advertisement, as provided by 25 Pa. Code §§ 77.121--77.123 and 86.31--86.34 (relating to public notices of filing of permit applications, opportunity for comment and informal conferences).
Where any of the mining activities listed below will have discharges of wastewater to streams, the Department will incorporate NPDES permits into the mining activity permits issued in response to these applications. The NPDES permits will contain, at a minimum, technology-based effluent limitations (as described in the Department's regulations--25 Pa. Code §§ 77.522, 87.102, 88.92, 88.187, 88.242, 89.52 and 90.102) for iron, manganese, suspended solids, settleable solids, alkalinity and pH. In addition to the above, more restrictive effluent limitations, restrictions on discharge volume or restrictions on the extent of mining which may occur will be incorporated into a mining activity permit, when necessary, for compliance with water quality standards (in accordance with 25 Pa. Code Chapters 93 and 95). Persons or agencies which have requested review of the NPDES permit requirements for a particular mining activity within the above-mentioned public comment period will be provided with a 30-day period to review and submit comments on those requirements.
Written comments or objections should contain the name, address and telephone number of persons submitting comments or objections; application number; and a statement of sufficient detail to inform the Department on the basis of comment or objection and relevant facts upon which it is based. Requests for an informal conference must contain the name, address and telephone number of requestor; application number; a brief summary of the issues to be raised by the requestor at the conference; and a statement whether the requestor desires to have the conference conducted in the locality of the proposed mining activities.

FEDERAL WATER POLLUTION CONTROL ACT, SECTION 401 The following permit applications and requests for Environmental Assessment approval and requests for Water Quality Certification have been received by the Department of Environmental Protection. Section 401 of the Federal Water Pollution Control Act (FWPCA) (33 U.S.C.A. § 1341(a)), requires the State to certify that the involved projects will not violate the applicable provisions of sections 301--303, 306 and 307 of the FWPCA (33 U.S.C.A. §§ 1311--1313, 1316 and 1317) as well as relevant State requirements. Initial requests for 401 Water Quality Certification will be published concurrently with the permit application. Persons objecting to approval of a request for certification under section 401 or to the issuance of a Dam Permit or Water Obstruction and Encroachment Permit or the approval of an Environmental Assessment must submit any comments, suggestions or objections within 30 days of the date of this notice as well as any questions to the office noted above the application. Comments should contain the name, address and telephone number of the person commenting, identification of the certification request to which the comments or objections are addressed and a concise statement of comments, objections or suggestions including the relevant facts upon which they are based.
The Department may conduct a fact-finding hearing or an informal conference in response to comments if deemed necessary. Each individual will be notified, in writing, of the time and place of a scheduled hearing or conference concerning the certification request to which the comment, objection or suggestion relates. To construct and maintain the Chester Springs Road Crossing (S.R. 1024) over the Pine Creek tributary (HQ-TSF) to Pickering Creek, which will replace the previous stone, arch structure that has been removed. A 24-foot by 7-foot precast, open-bottom arch culvert is proposed for the site. The structure will span 24 feet and will be 28 feet wide with a maximum under clearance of 5-feet 9-inches. The proposed structure is to be constructed on the footprint of the previous bridge. Associated with the proposed structure is riprap scour protection to be placed at either end of the structure to protect the footings. It will extend for a length of 5 feet along the channel and 38 feet across the streambed and embankment slopes on either side. The project site is located adjacent to the intersection of Old Kimberton Road and North Chester Springs Road (Downingtown, PA Quadrangle North: 18.5 inches; West: 4.7 inches).
