STATE BOARD OF NURSING

Bureau of Professional and Occupational Affairs v. Kathy D. Godfrey, L.P.N.; Doc. No. 0241-51-04

   On May 21, 2004, Kathy D. Godfrey, L.P.N., license no. PN-093568-L, of Pittsburgh, Allegheny County, was suspended for at least 3 years, retroactive to February 13, 2004, based on her violation of the terms and conditions of her voluntary recovery program consent agreement and State Board of Nursing (Board) order.

   Individuals may obtain a copy of the final order by writing to Martha H. Brown, Board Counsel, State Board of Nursing, P. O. Box 2649, Harrisburg, PA 17105-2649.

   This final order represents the final Board decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of the petition for review. The Board contact for receiving service of appeals is the previously named Board counsel.
