Application and Request for Plan Approval of a Mutual-to-Stock Conversion

[Saturday, April 12, 2008]Lebanon Mutual Insurance Company, a Pennsylvania domiciled mutual property insurance company, has submitted a Plan of Mutual-to-Stock Conversion, whereby it proposes to convert from a mutual insurance company to an insurer organized as a stock company. The initial filing was made under requirements set forth under the Insurance Company Mutual-to-Stock Conversion Act (40 P. S. §§ 911-A--929-A).
Persons wishing to comment on the grounds of public or private interest to the issuance of the Insurance Department's (Department) order approving of this conversion is invited to submit a written statement to the Department within 60 days from the date of this issue of the Pennsylvania Bulletin. Written statements must include name, address and telephone number of the author, identification of the application to which the statement is addressed, and a concise statement with sufficient detail to inform the Department of the exact basis of the statement and the relevant facts upon which it is based.
