[Saturday, November 13, 2010]Terri Lynne Howell, LPN, license no. PN272078, of Wilkes-Barre, Luzerne County, had her license indefinitely suspended for a period of no less than 3 years. The suspension may be stayed in favor of probation upon submission to the Bureau of Professional and Occupational Affairs, Professional Health Monitoring Program (PHMP), Disciplinary Monitoring Unit of all properly executed forms and releases required by the PHMP, and her enrollment in the urine testing program required by the PHMP.

This adjudication and order represents the State Board of Nursing's (Board) final decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court, must serve the Board with a copy of their petition for review. The Board contact for receiving service of the appeals is the previously-named Board Counsel.
