PUBLIC WATER SUPPLY PERMITS The Department has taken the following actions on applications received under the Pennsylvania Safe Drinking Water Act (35 P. S. §§ 721.1—721.17) for the construction, substantial modification or operation of a public water system.
Persons aggrieved by an action may appeal that action to the Environmental Hearing Board (Board) under section 4 of the Environmental Hearing Board Act and 2 Pa.C.S. §§ 501—508 and 701—704. The Department's review of the sewage facilities update revision has not identified any significant environmental impacts resulting from this proposal. Jamestown, PA 16134Crawford Plan Description: This Plan was submitted under the June 1, 2011 Corrective Action Plan (CAP) developed to address hydraulic overload conditions experienced at the North and South Shenango Joint Municipal Authority (NSSJMA) WWTP through retrofit of one of the process trains with a continuous flow Sequential Batch Reactor (SBR). The normal daily flow to the WWTP will be split evenly three ways between the continuous flow SBR and the two original remaining process trains up to the rate of 0.433 MGD for each unit. At this point, the flow to the existing trains will be held at this level and all peak flows shall be routed to the SBR. A new load control structure will be constructed to control splitting the flows. Peak daily flow will be 2.8 MGD with peak instantaneous capacity of 3.2 MGD. Additional WWTP upgrades include yard piping upgrades to the process air and chemical feed systems, and a solids grinder for the raw wastewater prior to the diversion structure. The Plan has incorporated projections of future flows from the service area. The Department's review of the sewage facilities update revision has not identified any significant environmental impacts resulting from this proposal. Any required NPDES Permits or WQM Permits must be obtained in the name of the Municipality or Authority, as appropriate.
LAND RECYCLING AND
ENVIRONMENTAL REMEDIATION

PREAMBLE 2
The following plans and reports were submitted under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101—6026.907).
Provisions of Sections 301—308 of the Land Recycling and Environmental Remediation Standards Act (act) (35 P. S. §§ 6026.301—6026.308) require the Department to publish in the Pennsylvania Bulletin a notice of submission of plans and reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the act's remediation standards. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed and summaries of sampling analytical results which demonstrate that remediation has attained the cleanup standard selected. Submission of plans and reports, other than the final report, will also be published in the Pennsylvania Bulletin. These include the remedial investigation report, risk assessment report and cleanup plan for a site-specific standard remediation. A remedial investigation report includes conclusions from the site investigation; concentration of regulated substances in environmental media; benefits of reuse of the property; and, in some circumstances, a fate and transport analysis. If required, a risk assessment report describes potential adverse effects caused by the presence of regulated substances. If required, a cleanup plan evaluates the abilities of potential remedies to achieve remedy requirements.

The report was submitted to document partial fulfillment of the Site-Specific Standard for soil and groundwater.
Shawnee Investment Corp-Clapper House, River Road, Smithfield Township, Monroe County, Gregg Walters, MEA, Inc., has submitted a Final Report on behalf of this client, Russ Gladd, Shawnee Investment Corp., P. O. Box 67, Shawnee on Delaware, PA 18356, concerning the remediation of soil and groundwater from No. 2 fuel oil due to an overfill to an abandoned tank fill port no longer connected to the tank. The applicant proposes to remediate the site to meet the Residential Statewide Health Standards for soils and groundwater. The intended future use of the site is residential. A public notice regarding the submission of the Final Report was published in The Pocono Record, on October 8, 2012. A Notice of Intent to Remediate was simultaneously submitted.

The Department may approve or disapprove plans and reports submitted. This notice provides the Department's decision and, if relevant, the basis for disapproval.

The report documented attainment of the Residential Statewide Health Standards for soils and was approved on 10/31/12.
Trojan Horse Diesel Fuel Release Site, I-80 Westbound at Mile Marker 301.8, Hamilton Township, Monroe County, Martin Gilgallon, Pennsylvania Tectonics, Inc., had submitted a Final Report on behalf of his client, (PennDOT-Monroe County Office, c/o Mr. Robert Mundrick, 1586 North 9th Street, Stroudsburg, PA 18360), concerning the remediation of contaminated soils from diesel fuel due to a vehicle accident involving Trojan Horse Ltd tractor-trailer. General Permit Number WMGR064. General Permit Number WMGR064, issued on September 22, 2000, authorized the beneficial use of natural gas well brines for roadway pre-wetting (brines mixed with antiskid materials prior to roadway application), anti-icing (brines applied directly to roadway and walkway surfaces prior to precipitation event), and for roadway de-icing purposes (brine is applied directly to roadway surfaces after precipitation event). General Permit Number WMGR064 expired on September 22, 2010.
In the Pennsylvania Bulletin notice dated September 17, 2011 the Department proposed and solicited public input on the renewal and modification of General Permit Number WMGR064 to include the beneficial use of natural gas well brines for (i) dust suppressant and (ii) as a stabilizer for unpaved secondary roadway systems. That public comment period expired on November 16, 2011. The Department has decided to withdraw the proposed renewal and modification of General Permit Number WMGR064. Permit WMGR064 remains expired and unavailable for use.

The facility consists of systems for the receiving, batching and storage of raw materials (controlled with 3 baghouses), one melting tank with associated molding and mold swabbing operations, a chrome plating operation (controlled with a composite mesh pad), and miscellaneous natural gas combustion sources including 5 kilns, 4 firepolishers, 5 forehearths, and 1 lehr. To stay below major source thresholds, permit limits the charge to the melting furnace and limits the use of sodium sulfate. Emissions from the facility, on a 12-month rolling basis, after taking synthetic minor limitations, are limited to 35 tons PM10, 95 tons SOx, 65 tons NOx, 10 tons VOC, 25 tons CO, 0.29 tons HF, 0.04 tons HCl, and 0.0094 tons Chromium.
The facility is subject to 60 CFR 63, Subpart N (related to chrome plating), 40 CFR 98, Subpart N (related to mandatory greenhouse gas reporting), and PA Code Title 25 Chapters 121—145. The proposed SOOP contains conditions related to restrictions, testing, monitoring, recordkeeping, reporting and work practice standards.

To perform the following water obstruction and encroachment activities associated with the construction of approximately 2.5 miles of the Baxter walking trail, a segment of Delaware River Heritage Trail, approximately 10 to 14 feet wide, extending from the proposed bridge over the Pennypack Creek to the Pleasant Hill Park:
1. To construct and maintain 13-foot 8-inches long, 180-foot span single span truss bridge across the Pennypack Creek (WWF). The bridge will have an underclearance of approximately 2.97 feet.
2. To construct and maintain approximately 1658 linear feet of asphalt trail in the floodplain of the Pennypack Creek.
3. To construct and maintain approximately 1843 linear feet of asphalt trail in the floodplain of the Delaware River.
The project permanently impacts approximately 32 linear feet of stream. The jurisdictional area of the project commences within the floodway near the Pennypack Avenue and runs in the southwesterly direction parallel to the shoreline of the Delaware River for approximately 2273 feet and terminates at the proposed bridge in the City and County of Philadelphia (Frankford, PA USGS Quadrangle N: 5.92; W: 0.96).

The proposed amendment will authorize the replacement of the existing 30-inch gas pipeline with a 42-inch pipeline, between Stations 2269 and 2295, across and temporarily impacting the East Branch of the Brandywine Creek (WWF-MF) and Ludwig's Run (WWF-MF) at two locations by utilizing an open cut method. Also, an unnamed tributary to the East Branch of the Brandywine Creek (WWWF-MF) will be impacted by a temporary construction crossing and another unnamed tributary to the East Branch of Brandywine Creek (HQ-TSF, MF) will be crossed by the pump diversion during the use of a coffer dam.
