PROPOSED NUTRIENT MANAGEMENT PLANS RELATED TO APPLICATIONS FOR NPDES PERMITS FOR CAFOs This notice provides information about agricultural operations that have submitted nutrient management plans (NMPs) for approval under 3 Pa.C.S. Chapter 5 and that have or anticipate submitting applications for new, amended or renewed NPDES permits, or Notices of Intent (NOIs) for coverage under a general permit, for CAFOs, under 25 Pa. Code Chapter 92a. This notice is provided in accordance with 25 Pa. Code Chapter 92a and 40 CFR Part 122, implementing The Clean Streams Law and the Federal Clean Water Act.
Based upon preliminary reviews, the State Conservation Commission (SCC) or County Conservation Districts (CCD) working under a delegation agreement with the SCC have completed an administrative review of NMPs described. These NMPs are published as proposed plans for comment prior to taking final actions. The NMPs are available for review at the CCD office for the county where the agricultural operation is located. AIR QUALITY
PLAN APPROVAL AND OPERATING PERMIT APPLICATIONS NEW SOURCES AND MODIFICATIONS The Department has developed an ''integrated'' plan approval, State Operating Permit and Title V Operating Permit program. This integrated approach is designed to make the permitting process more efficient for the Department, the regulated community and the public. This approach allows the owner or operator of a facility to complete and submit permitting documents relevant to its application one time, affords an opportunity for public input and provides for sequential issuance of the necessary permits.
The Department received applications for Plan Approvals or Operating Permits from the following facilities.
Copies of these applications, subsequently prepared draft permits, review summaries and other support materials are available for review in the regional office listed before the applications. Persons interested in reviewing the application files should contact the appropriate regional office to schedule appointments.
Persons wishing to receive a copy of a proposed Plan Approval or Operating Permit shall indicate interests to the Department regional office within 30 days of the date of this notice and shall file protests or comments on a proposed Plan Approval or Operating Permit within 30 days of the Department providing a copy of the proposed documents to persons or within 30 days of its publication in the Pennsylvania Bulletin, whichever comes first. Code §§ 127.44(b) and 127.424(b), the Department of Environmental Protection (DEP) intends to issue a revised Plan Approval No. 39-309-076 to Lafarge North America, Inc., 5160 Main Street, Whitehall, PA 18052, for their plant located in Whitehall Township, Lehigh County. The facility currently operates under Title V Operating Permit No. 39-00011. This plan approval will be incorporated into the Title V Operating Permit through an administrative amendment at a later date, and the action will be published as a notice in the Pennsylvania Bulletin.
Plan Approval No. 39-309-076 was issued by DEP on May 19, 2010 for the installation and operation of a Dry Absorbent Addition (DAA) system and Selective Non-Catalytic Reduction (SNCR) system for the control of sulfur dioxide (SO2) and nitrogen oxide (NOx) emissions from Kiln Nos. 2 and 3 operated at Lafarge's Whitehall facility. The Department had imposed an ammonia ''slip'' emission limit for both kilns in Plan Approval No. 39-309-076. Lafarge filed a protective appeal specifically in relation to conditions restricting ammonia slip to 10 ppmvd at 15% O2. Following a series of meetings and negotiations with representatives from PADEP, a series of actions were initiated that included (1) Lafarge's measurement and determination of ''background'' ammonia concentrations from Kiln No. 2 via emission testing per- formed during the period from October 20 through 28, 2010, (2) a systematic review of alternative conditions to the ammonia limits included in Plan Approval No. 39-309-076, and ultimately (3) an ammonia risk assessment modeling analysis to evaluate the impact of potential ammonia emissions on the surrounding area. The results of the ammonia risk assessment modeling analyses were reported to DEP. The Department concluded that the modeled ammonia concentrations were below the applicable acute and chronic ammonia exposure guidelines for both kilns. Based on study and risk assessment conducted by the company, the Department has changed conditions in the Plan Approval No. 39-309-076 for both kilns to include short term (hourly) and long term (annual) ammonia injection rate restrictions, monitoring requirements, which include the monitoring of ammonia injection rates and clinker production. Additionally, the revised Plan Approval includes recordkeeping requirements to ensure that ambient ammonia concentrations do not exceed the acute and chronic ammonia exposure guidelines.

41-00086A: Knight-Confer Funeral Home, Inc. (1914 Memorial Avenue, Williamsport, PA 17701) for a plan approval to construct a human crematorium at their facility in Williamsport, Lycoming County. The respective facility is a natural minor facility for which a state only operating permit has not been issued.
The Department's review of the information submitted by Knight-Confer Funeral Home, Inc. indicates that construction of the crematorium will meet all applicable air quality regulatory requirements pertaining to air contamination sources and the emission of air contaminants including the best available technology requirements of 25 Pa. Code §§ 127.1 and 127.12. Based on these findings, the Department intends to approve the application and issue plan approval for construction of the crematorium. Additionally, if the Department determines the crematorium is operating in compliance with all plan approval conditions, the conditions established in the plan approval will be incorporated into a state only operating permit.
The emission of air contaminants from the crematorium will not exceed the following limits in any 12 consecutive month period: NOx-0.82 ton; CO-0.69 ton; VOCs-0.04 ton; SOx-0.01 ton; PM10-1.58 tons.
In addition to the emission limitations, the following is a summary of the types of conditions the Department intends place in the plan approval to ensure compliance with all applicable regulatory requirements:
1. The particulate matter emissions from the crematory shall not exceed 0.08 grain per dry standard cubic foot of exhaust, corrected to 7% oxygen.
2. The visible emissions from the crematory shall not equal or exceed 10% opacity for a period or periods aggregating more than 3 minutes in any one hour or equal or exceed 30% opacity at any time.
3. The crematory shall only be fired on natural gas or LP gas.
4. Only human remains and associated containers, which are constructed of cardboard, particleboard or wood, shall be incinerated in the crematory. Under no circumstances shall the crematory be used for the incineration of hospital waste, infectious waste or any other type of waste other than human remains and associated containers.
5. The crematory shall be equipped with instrumentation to continuously monitor and record the primary and secondary combustion chamber exit temperatures any time the crematory is in operation.
6. The crematory shall be equipped with instrumentation to continuously monitor the opacity of the effluent gas any time the crematory is in operation.
7. A secondary combustion chamber temperature of at least 1800°F shall be maintained for a residence time of at least one (1) second at any time that cremation is occurring in the crematory.
8. The crematory shall be equipped with an interlock which prevents the primary combustion chamber burner from firing unless the secondary combustion chamber is at a temperature of at least 1800°F.
9. The crematory shall be equipped with an interlock which prevents the primary combustion chamber burner from firing and sounds an alarm any time the opacity of the visible emissions exceeds 10%.
10. The charge door of the crematory shall be kept shut during the cremation cycle except for short periods of time in order to inspect and/or reposition the remains.
11. The permittee shall maintain accurate and comprehensive records of the primary and secondary combustion chamber temperature chart recordings.

Emissions from this facility arise from one natural gas fire Solar Centaur Turbine Model T-5802S rated at 59.25 mmbtu/hr and 6,166 hp, one Onan emergency generator rated at 17 hp with an operating limit of 500 hours per year, fugitive emissions from piping components, vents, blowdowns, tanks and tank transfers.
This facility's potential to emit, on a rolling 12-month basis, is 28.3 tons of carbon monoxide, 39.4 tons of nitrogen oxides, 6.5 tons of volatile organic compounds, 0.9 ton of SO2, 1.7 tons of PM10, 0.81 ton of hazardous air pollutants and 29,312 tons of carbon dioxide equivalents (greenhouse gases). The subject facility consists of two sand storage tipples equipped with dust collectors, three cement/fly ash storage silos and a mixing operation equipped with a dust collector, two HCl storage tanks equipped with an acid scrubber, an HCl transloading operation equipped with an acid scrubber, a waste oil heater, a diesel storage tank, and paved and unpaved roads and surfaces. The potential emissions from the facility are estimated to be: 0.0 tpy NOx, 0.0 tpy CO, 0.0 tpy SOx, 9.8 tpy PM, 8.2 tpy PM10, 0.0 tpy VOC, and 0.2 tpy HCl.
The facility is required to conduct a weekly survey of the facility during daylight hours while the facility is operating to ensure compliance with the visible emission, fugitive emission and malodor restrictions. Records of the weekly survey performed must be recorded. Monthly preventative maintenance inspections are to be performed on the control devices and recorded in an on-site log. The facility is also required to water all in-plant roads as needed to prevent fugitive emissions across the property line, maintain a set vehicle pattern, post speed limit sign of 15 mph as well as promptly remove earth or other material from paved roads onto which earth or other material has been transported by trucking or earth moving equipment, or other means. Particulate matter emissions are not to exceed 0.04 gr/dscf. The proposed authorization is subject to State and Federal Regulations. The permit includes emission limits as well as operation, monitoring, reporting, and recordkeeping requirements.

26-00534: Fayette Thermal, LLC, (755 Opossum Lake Road, Carlisle, PA, 17013), Minor Source Operating Permit for the operation of a coal, waste coal, biomass-fired steam generation plant, known as the Fayette Thermal Steam Plant, located in Luzerne Township, Fayette County.
The facility contains air contamination sources consisting of two coal/waste coal/biomass-fired, 20.7 MMBtu/hr CFB boilers, three natural gas-fired, 9.9 MMBtu/hr, backup boilers, material handling equipment, and three storage silos for limestone, sand, and ash. On the CFB boilers NOx emissions are controlled by flue gas recirculation, SO2 emissions are controlled by limestone injection, and particulate is controlled by fabric filtration. This facility has the potential to emit the following type and quantity of pollutants: 97.9 tons per year of NOx, 52.2 tons of SO2, 39.7 tons per year of CO, 7.2 tons per year of VOC, 2.9 tons per year of PM10, 2.1 tons per year of hydrochloric acid, 2.5 tons per year of the sum of all Hazardous Air Pollutants combined, and 48,225 tons of CO2e greenhouse gases. The facility is limited to a maximum opacity from any processing equipment of 20 percent. The facility is subject to state requirements, as well as 40 CFR 60, Subpart Dc—Standards of Performance for Small Industrial-Commercial-Institutional Steam Generating Units and 40 CFR 63, Subpart JJJJJJ—National Emission Standards for Hazardous Air Pollutants for Area Sources: Industrial, Commercial, and Institutional Boilers. The permit includes emission limitations and operational, monitoring, testing, reporting, and recordkeeping requirements for the facility. No emission or equipment changes are authorized by this action.

Those who wish to provide the Department with additional written information that they believe should be considered prior to the issuance of this State Only Operating Permit may submit the information to Martin L. Hochhauser, P.E., Department of Environmental Protection, 400 Waterfront Drive, Pittsburgh, PA, 15222. Each written comment must contain the name, address and telephone number of the person submitting the comments, identification of the proposed Operating Permit (specify Operating Permit OP-26-00534), and concise statements regarding the relevancy of the information or objections to issuance of the Operating Permit.
All comments must be received prior to the close of business 30 days after the date of this publication. In accordance with 25 Pa. Code § 127.428, prior to issuing an operating permit, the Department may hold a fact-finding conference or hearing at which the petitioner and any persons who have properly filed a protest under § 127.426 (relating to filing protests) may appear and give testimony. The Department is not required to hold a conference or hearing. The applicant, the protestant, and other participants will be notified of the time, place and purpose of a conference or hearing, in writing or by publication in a newspaper or the Pennsylvania Bulletin, unless the Department determines that notification by telephone will be sufficient.

43-00349: Valley Can, Inc. (1264 Fredonia Road, Hadley, PA 16130-2612) for renewal of the Natural Minor Operating Permit for their facility in Perry Township, Mercer County. The facility is manufacturing refuse containers. The facility's emitting sources include: 1) Existing paint spray booth and, 2) Welding & Plasma cutting operation. The potential emission from the facility is VOC: 6 tons per year and, PM-10: 2 TPY. The welding operation of the facility is not subject to 40 CFR Part 63, Subpart XXXXXX because, due to the facility's NAICS Number: 332322. The surface coating operation is not subject to HHHHHH because the coating paint contained no target HAP. The emissions of pollutants from the facility are less than Title V threshold limits. Thus, the facility is considered a natural minor.
