 On July 1, 2012, the State Board of Pharmacy suspended the license of Phila RX Pharmacy, license no. PP481266, of Philadelphia, Philadelphia County for nonpayment of a previously issued civil penalty.

 Individuals may obtain a copy of the order of suspension by writing to Kerry E. Maloney, Board Counsel, State Board of Pharmacy, P. O. Box 2649, Harrisburg, PA 17105-2649.
