PUBLIC WATER SUPPLY (PWS) PERMITS Under the Pennsylvania Safe Drinking Water Act (35 P. S. §§ 721.1—721.17), the following parties have applied for PWS permits to construct or substantially modify public water systems.
Persons wishing to comment on permit applications are invited to submit statements to the office listed before the application within 30 days of this public notice. Comments received within this 30-day comment period will be considered in the formulation of the final determinations regarding an application. A comment should include the name, address and telephone number of the writer and a concise statement to inform the Department of the exact basis of a comment and the relevant facts upon which it is based. A public hearing may be held after consideration of comments received during the 30-day public comment period.
Following the comment period, the Department will make a final determination regarding the proposed permit. Notice of this final determination will be published in the Pennsylvania Bulletin at which time this determination may be appealed to the Environmental Hearing Board.

LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

PREAMBLE 1
Acknowledgment of Notices of Intent to Remediate Submitted under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101—6026.907)
Sections 302—305 of the Land Recycling and Environmental Remediation Standards Act (act) (35 P. S. §§ 6026.302—6026.305) require the Department to publish in the Pennsylvania Bulletin an acknowledgment noting receipt of Notices of Intent to Remediate. An acknowledgment of the receipt of a Notice of Intent to Remediate is used to identify a site where a person proposes to, or has been required to, respond to a release of a regulated substance at a site. A person intending to use the background standard, Statewide health standard, the site-specific standard or intend to remediate a site as a special industrial area shall file a Notice of Intent to Remediate with the Department. A Notice of Intent to Remediate filed with the Department provides a brief description of the location of the site, a list of known or suspected contaminants at the site, the proposed remediation measures for the site and a description of the intended future use of the site. A person who demonstrates attainment of one or a combination of cleanup standards or receives approval of a special industrial area remediation identified under the act will be relieved of further liability for the remediation of the site for contamination identified in reports submitted to and approved by the Department. Furthermore, the person shall not be subject to citizen suits or other contribution actions brought by responsible persons not participating in the remediation.
Under sections 304(n)(1)(ii) and 305(c)(2) of the act, there is a 30-day public and municipal comment period for sites proposed for remediation using a site-specific standard, in whole or in part, and for sites remediated as a special industrial area. This period begins when a summary of the Notice of Intent to Remediate is published in a newspaper of general circulation in the area of the site. For the following site, proposed for remediation to a site-specific standard or as a special industrial area, the municipality, within which the site is located, may request to be involved in the development of the remediation and reuse plans for the site if the request is made within 30 days of the date specified as follows. During this comment period, the municipality may request that the person identified as the remediator of the site develop and implement a public involvement plan. Requests to be involved and comments should be directed to the remediator of the site.
For further information concerning the content of a Notice of Intent to Remediate, contact the environmental cleanup program manager in the Department regional office listed before the notice. All-American Hose (217 Titusville Road), 217 Titusville Road, Borough of Union City, Erie County. AECOM Technical Services, Gulf Tower, 707 Grant Street, Pittsburgh, PA 15219, on behalf of Parker Hannifin Corporation 6035 Parkland Boulevard, Cleveland, OH 44124, submitted a Notice of Intent to Remediate. The site was developed in approximately 1976 for the manufacture of hose. Manufacturing processes consisting of rubber and polyurethane hose extrusion, rubber curing, polyurethane normalizing, circular weaving and hydrostatic hose pressure testing occur at the site. Chemicals used at the site include glycol (in a small above-ground storage tank), oils (lubricating and compressor), cleaning agents, silicone, paints, lacquer thinners, rubber, and polyurethane. Latex was also used at the site until approximately 2009. Over 60 soil samples and 14 groundwater samples were collected and analyzed to characterize current site conditions. Arsenic exceedances of the Statewide Health Standard in soil were detected in two soil samples located beneath engineered structures (the building slab and parking lot) and in two subsurface soil samples approximately 10 feet deep located near the northeast portion of the building. The property is zoned ''light industrial'' and it is likely that the property will continue to be used in the foreseeable future for industrial purposes. Therefore, pathway elimination will be used to demonstrate attainment of the Act 2 Site-Specific Standard. Institutional and engineering controls, including restricting groundwater use in the future and ensuring that the site remains non-residential, will be identified in an Environmental Covenant. The Notice of Intent to Remediate was published in The Erie Times News on January 24, 2014.
All American Hose, Inc., (6420 West Ridge Road), Fairview Township, Erie County. AECOM Technical Services, Inc., Gulf Tower, 707 Grant Street, Pittsburgh, PA 15219, on behalf of Parker Hannifin Corporation, 6035 Parkland Boulevard, Cleveland, OH 44124, submitted a Notice of Intent to Remediate. The site was developed in approximately 1979. Historical use of the site has included injection molding of spotlights and flashlights; assembly and testing of solenoid valves; in-house print shop that utilized inks and cleaners; to the current manufacturing of hose liners and associated warehousing. The current manufacturing processes at the site include rubber and polyurethane liner extrusion, yarn twisting, and hardware assembly. The intended future use of the site will continue to be industrial. The selected remediation standards for the site will be combined Site-Specific and Statewide Health. The Notice of Intent to Remediate was published in The Erie Times-News on January 24, 2014.
OPERATE WASTE PROCESSING OR DISPOSAL AREA OR SITE

Southeast Region: Regional Solid Waste Manager, 2 East Main Street, Norristown, PA19401
Permit Application No. 101680. Waste Management Disposal Services of Pennsylvania, Inc., 1000 New Ford Mill Road, Morrisville, PA 19067. This permit renewal application is for extending the current operation at GROWS North Landfill for an additional 4.25 years, existing permit expires on November 15, 2014. The GROWS North Landfill is a municipal waste landfill located in Falls Township, Bucks County. The application was received by the Southeast Regional Office on October 31, 2013.
Permit Application No. 101699. Waste Management of Fairless LLC, 1000 New Ford Mill Road, Morrisville PA 19067. This Phase II application is for Waste Management of Fairless, LLC (WMF) to develop the Fairless Landfill as a new 198-acre municipal waste landfill to be known as Fairless Landfill received under 25 Pa. Code § 271.202. An alternative project timeline of 600 days was negotiated for the review of the Phase I and II components of the application. The Fairless Landfill is located in Falls Township; Bucks County. The application was received by the Southeast Regional Office on February 5, 2014.
AIR QUALITY
PLAN APPROVAL AND OPERATING PERMIT APPLICATIONS
NEW SOURCES AND MODIFICATIONS The Department has developed an ''integrated'' plan approval, State Operating Permit and Title V Operating Permit program. This integrated approach is designed to make the permitting process more efficient for the Department, the regulated community and the public. This approach allows the owner or operator of a facility to complete and submit permitting documents relevant to its application one time, affords an opportunity for public input and provides for sequential issuance of the necessary permits.
The Department received applications for Plan Approvals or Operating Permits from the following facilities.
Copies of these applications, subsequently prepared draft permits, review summaries and other support materials are available for review in the regional office listed before the applications. Persons interested in reviewing the application files should contact the appropriate regional office to schedule appointments.
Persons wishing to receive a copy of a proposed Plan Approval or Operating Permit shall indicate interests to the Department regional office within 30 days of the date of this notice and shall file protests or comments on a proposed Plan Approval or Operating Permit within 30 days of the Department providing a copy of the proposed documents to persons or within 30 days of its publication in the Pennsylvania Bulletin, whichever comes first. Interested persons may also request that hearings be held concerning a proposed Plan Approval or Operating Permit. A comment or protest filed with the Department regional office shall include a concise statement of the objections to the issuance of the Plan Approval or Operating Permit and relevant facts which serve as the basis for the objections. If the Department schedules a hearing, a notice will be published in the Pennsylvania Bulletin at least 30 days prior the date of the hearing.

39-00011A: Lafarge North America, Inc. (5160 Main Street, Whitehall, PA 18052) for conversion of their No. 2 raw mill baghouse from reverse air to pulse jet at their Whitehall Plant in Whitehall Township, Lehigh County.
The current emission limits for this source will not change as a result of this modification. The Plan Approval will contain additional recordkeeping, testing and operating restrictions designed to keep the facility operating within all applicable air quality requirements. The facility currently has a Title V Operating Permit No. 39-00011. This plan approval will, in accordance with 25 Pa. Code § 127.450, be incorporated into the Title V Operating Permit through an administrative amendment at a later date.

54-00022A: SAPA Extrusions Inc. (53 Pottsville Street, Cressona, PA 17929-1217) to electively apply for federally enforceable limits on natural gas usage for P3 and P4 preheat furnaces at their facility in Cressona Borough, Schuylkill County.
In accordance with 25 Pa. Code §§ 127.44(b) and 127.424(b) the Department of Environmental Protection (DEP) intends to issue a plan approval to SAPA Extrusions, Inc. to electively apply for federally enforceable limits on natural gas usage for P3 and P4 preheat furnaces at their facility located in Cressona Borough, Schuylkill County. The facility currently has a Title V Operating Permit No. 54-00022. Plan approval 54-00022A will subsequently be incorporated into the Title V Operating Permit through an administrative amendment in accordance with 25 Pa. Code § 127.450.
Plan Approval 54-00022A seeks new federally enforceable limits on natural gas usage. The application seeks to limit P3 and P4 preheat furnaces to a total of 257.04 mmcf/year for both units combined. NOx emissions will decrease by 11.11 TPY. SAPA Extrusion is a major facility subject to Title V permitting requirements and is located in Cressona Borough, Schuylkill County. The plan approval will include all appropriate monitoring, record keeping, and reporting requirements designed to keep the equipment operating within all applicable air quality requirements. Further details on the conditions and the reasons for their inclusion are available upon request.

06-05096D: Reading Hospital (PO Box 16052, Reading, PA 19612-6052) to install two (2) new emergency generators at their facility in West Reading Borough, Berks County.
In accordance with 25 Pa. Code §§ 127.44(a) and 127.45(a), the Department of Environmental Protection (DEP) has received an application and intends to issue a Plan Approval to the abovementioned company for the abovementioned project.
Each 5.65 MMBTU/hr diesel generator is expected to operate less than 500 hours per year. The company shall be subject to and comply with the 25 Pa. Code Chapter 123 emission standards for PM, SO2 and sulfur content of the diesel fuel. Compliance with EPA Tier 2 emission limits meets best available technology (BAT). The plan approval will contain additional recordkeeping and operating restrictions designed to keep the facility operating within all applicable air quality requirements. The expected potential air emissions from the units are not expected to exceed 11.16 tpy of NOx, 0.86 tpy of SOx, 6.36 tpy of CO, 0.37 tpy of PM, 0.59 tpy of VOCs, and negligible HAP amounts. Among other items, the conditions include provisions derived from 40 CFR 63, Subpart ZZZZ—National Emissions Standards for Hazardous Air Pollutants for Stationary Reciprocating Internal Combustion Engines and 40 CFR 60, Subpart IIII—Standards of Performance for Stationary Compression Ignition Internal Combustion Engines

A person may oppose the proposed plan approval, or may provide the Department with additional information to consider in its review, by filing a written protest with the Department at the address listed above. 41-00084: Panda Patriot LLC (4100 Spring Valley Road, Suite 1001, Dallas, TX 75244) for the initial Acid Rain Permit, TIVOP 41-00084, for the proposed Panda Patriot Generation Plant in Clinton Township, Lycoming County. This Title V facility is subject to the Acid Rain Requirements of Title IV of the Clean Air Act. The application is subject to the regulatory requirements specified in 40 CFR Part 72. The Acid Rain Permit will cover the two (2) combined cycle combustion turbine units at the facility. The Acid Rain Permit incorporates the applicable requirements to the units, as specified in 25 Pa. Code § 127.531 and 40 CFR Parts 72 through 80. The SO2 allowance allocated by the Acid Rain Program pursuant to 40 CFR Part 73 for this facility is zero. The Title IV permit includes special permit conditions for the allowance tracking system. The units are not subject to the NOx emission limitations of 40 CFR Part 96 since they are only authorized to combust natural gas for operation. The subject facility consists of two clipping lines where veneer is trimmed to desired size. All wood waste is collected, sent to a chipper controlled by a 40,000 cfm baghouse and deposited into a silo for storage. Wood waste is used to fuel the 28.7 MMBtu biomass boiler. The boiler is used for building heat only. The facility's potential emissions are based on the heat season of 5,112 hours per year and AP-42 emission factors for the boiler and maximum operation of the baghouse: 36 tpy NOx; 44 tpy CO; 2 tpy SOx; 2 tpy VOC; and 82 tpy PM. In accordance with 25 Pa. Code §§ 127.44(a) and 127.45(a), the Department of Environmental Protection (DEP) has received and intends to issue a Plan Approval to Maid-Rite Specialty Foods LLC (107 Keystone Industrial Park, PO Box 172, Dunmore, PA 18512-0172) for their facility located in Scott Twp., Lackawanna County. This Plan Approval No. 35-00067A will be incorporated into a Natural Minor Permit through an administrative amendment at a later date.
Plan Approval No. 35-00067A is for the construction of a new cooking line at the facility. Emissions will be controlled by the use of a demister unit followed by a carbon filter bank. The company shall be subject to and comply with 25 Pa. Code § 123.41 for Visible emissions. The facility is subject to 25 Pa. Code § 127.12(a)(5) Best Available Technology (BAT) requirements. The company shall be subject to and comply with 25 Pa. Code § 123.31 for malodorous emissions. These limits will meet BAT requirements for this source. The Plan approval and Operating Permit will include testing, monitoring, record keeping and reporting requirements designed to keep the sources operating within all applicable air quality requirements.
Copies of the application, DEP's analysis and other documents used in the evaluation of the application are available for public review during normal business hours at Air Quality Program, 2 Public Square, Wilkes-Barre, PA 18701-1915.
