[Saturday, April 18, 2015]On March 9, 2015, Bobbi K. Smith, LPN, license no. PN260859L, last known of Hagerstown, MD, had her license to practice practical nursing automatically suspended for 10 years from the date of conviction due to her felony conviction under The Controlled Substance, Drug, Device and Cosmetic Act (35 P. S. §§ 780-101—780-144).
Individuals may obtain a copy of the adjudication by writing to Judith Pachter Schulder, Board Counsel, State Board of Nursing, P. O. Box 69523, Harrisburg, PA 17106-9523.
Individuals may file a request for a hearing to challenge the validity of the notice and order of automatic suspension along with an answer within 20 days of publication of this notice. If a request for hearing and answer are not filed within the time period set forth previously, a final order suspending their license will be issued by the State Board of Nursing (Board). The Board contact for receiving service of appeals is the previously-named Board counsel.
