 On April 7, 2016, Timothy Woodard t/d/b/a Timothy Woodard Barber Shop, barber shop license no. BO085898L, last known of Philadelphia, Philadelphia County, was issued a $1,000 civil penalty for having operated on a lapsed or expired license.
