 On August 11, 2017, Carly Ann Mienzenger, LPN, license No. PN284128, last known of Phoenixville, Chester County, was issued a final order of automatic suspension based on her misdemeanor conviction under The Controlled Substance, Drug, Device and Cosmetic Act (35 P.S. §§ 780-101—780-144).

 Individuals may obtain a copy of the automatic suspension by writing to Judith Pachter Schulder, Board Counsel, State Board of Nursing, P.O. Box 69523, Harrisburg, PA 17106-9523.
