 On November 30, 2015, Carly Ann Mienzenger, LPN, license No. PN284128, last known of Phoenixville, Chester County, was issued a notice and order of automatic suspension based on her misdemeanor conviction under The Controlled Substance, Drug, Device and Cosmetic Act (35 P.S. §§ 780-101—780-144).
