STATE CONSERVATION COMMISSION
PROPOSED NUTRIENT MANAGEMENT PLANS RELATED TO APPLICATIONS FOR NPDES PERMITS FOR CAFOs This notice provides information about agricultural operations that have submitted nutrient management plans (NMPs) for approval under 3 Pa.C.S. Chapter 5 and that have or anticipate submitting applications for new, amended or renewed (National Pollutant Discharge Elimination System) NPDES permits, or Notices of Intent (NOIs) for coverage under a general permit, for CAFOs, under 25 Pa. Code Chapter 92a. This notice is provided in accordance with 25 Pa. Code Chapter 92a and 40 CFR Part 122, implementing The Clean Streams Law and the Federal Clean Water Act.
Based upon preliminary reviews, the State Conservation Commission (SCC) or County Conservation Districts (CCD) working under a delegation agreement with the SCC have completed an administrative review of NMPs described. These NMPs are published as proposed plans for comment prior to taking final actions. The NMPs are available for review at the CCD office for the county where the agricultural operation is located. Persons wishing to comment on an NMP are invited to submit a statement outlining their comments on the plan to the CCD, with a copy to the SCC for each NMP, within 30-days from the date of this public notice. LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

PREAMBLE 1
Acknowledgment of Notices of Intent to Remediate Submitted Under the Land Recycling and Environmental Remediation Standards Act (35 P.S. §§ 6026.101—6026.908).
Sections 302—305 of the Land Recycling and Environmental Remediation Standards Act (Act) require the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin an acknowledgment noting receipt of any Notices of Intent to Remediate. An acknowledgment of the receipt of a Notice of Intent (NOI) to Remediate is used to identify a site where a person proposes to, or has been required to, respond to a release of a regulated substance at a site. Persons intending to use the background standard, Statewide health standard, the site-specific standard, or who intend to remediate a site as a special industrial area, must file a (NOI) to Remediate with the Department. A NOI to Remediate filed with the Department provides a brief description of the location of the site, a list of known or suspected contaminants at the site, the proposed remediation measures for the site, and a description of the intended future use of the site. A person who demonstrates attainment of one, or a combination of the cleanup standards, or who receives approval of a special industrial area remediation identified under the Act, will be relieved of further liability for the remediation of the site for any contamination identified in reports submitted to and approved by the Department. Furthermore, the person shall not be subject to citizen suits or other contribution actions brought by responsible persons not participating in the remediation.
Under Sections 304(n)(1)(ii) and 305(c)(2) of the Act, there is a 30-day public and municipal comment period for sites proposed for remediation using a site-specific standard, in whole or in part, and for sites remediated as a special industrial area. This period begins when a summary of the NOI to Remediate is published in a newspaper of general circulation in the area of the site. For the following identified site(s), proposed for remediation to a site-specific standard or as a special industrial area, the municipality, within which the site is located, may request to be involved in the development of the remediation and reuse plans for the site if the request is made within 30-days of the following specified date. During this comment period the municipality may request that the following identified person, as the remediator of the site, develop and implement a public involvement plan. Requests to be involved, and comments, should be directed to the remediator of the site.

AIR QUALITY
PLAN APPROVAL AND OPERATING PERMIT APPLICATIONS The Department has developed an ''integrated'' plan approval, State Operating Permit and Title V Operating Permit program. This integrated approach is designed to make the permitting process more efficient for the Department, the regulated community and the general public. This approach allows the owner or operator of a facility to submit permitting documents relevant to its application for all sources related to a facility or a proposed project, affords an opportunity for public input, and provides for a decision on the issuance of the necessary permits.
The Department received applications for Plan Approvals or Operating Permits from the following facilities. Copies of the application, the Department's analysis, all pertinent documents used in the evaluation of the application and subsequently prepared proposed plan approvals/operating permits are available for public review during normal business hours at the appropriate Department Regional Office. Appointments for scheduling a review must be made by calling the appropriate Department Regional Office. The address and phone number of the Regional Office is listed before the application notices.
Persons wishing to file a written protest or provide comments or additional information, which they believe should be considered prior to the issuance of a permit, may submit the information to the Department's Regional Office. A 30-day comment period from the date of this publication will exist for the submission of comments, protests and information. Each submission must contain the name, address and telephone number of the person submitting the comments, identification of the proposed Plan Approval/Operating Permit including the permit number and a concise statement regarding the relevancy of the information or objections to issuance of the permit.

63-00990C: Donora Dock, LLC (1501 Ligonier Street, Latrobe, PA 15650). In accordance with 25 Pa. Code §§ 127.44 and 127.45, the Pennsylvania Department of Environmental Protection (DEP) is providing notice that it intends to issue an Air Quality Plan Approval (PA-63-00990C) to Donora Dock LLC to authorize the expansion from five (5) petroleum coke and coal stockpiles, each of which are 1.4 acres in size to ten (10) petroleum coke and coal stockpiles, each of which are 1.4 acres in size as well as extension of haul roads to each of the stockpiles at the facility located in Carroll Township, Washington County. Allowance of temporary operation of the expansion described previously is also intended for issuance through this plan approval. Control of emissions from the facility will remain the same as permitted in the current synthetic minor SOOP as follows: a pressurized water truck equipped with water cannon and roadway spray nozzles; a sweeper, and pressurized water truck for roadway control; water sprays at each transfer point; minimization of drop heights, a barge loading by telescopic chute.
Upon authorization of this plan approval, the facility's potential to emit will not exceed 71.0 tons PM10 and 0.1 ton of HAPs. The facility is subject to the best available technology (BAT). The emission limit restrictions, throughput limitations, monitoring, recordkeeping, reporting, and work standards practice conditions of the proposed plan approval have been derived from the applicable requirements of 25 Pa. Code Chapters 121—145. Once compliance with the Plan Approval is demonstrated, the applicant will be required to revise the current synthetic minor SOOP application in accordance with 25 Pa. Code Subchapter F.
Donora Dock's Plan Approval application, the Department's Air Quality Review Memorandum, and the Proposed Air Quality Plan Approval for this project are available for review by any interested party at the Pennsylvania Department of Environmental Protection, Southwest Regional Office, 400 Waterfront Drive, Pittsburgh, PA 15222. 20-145C: Vitro Flat Glass LLC (5123 Victory Boule- vard, Cochranton, PA 16314), for the proposed Plantwide Applicability Limits (PAL) at the Vitro Flat Glass—Meadville Plant in Greenwood Township, Crawford County. This is a Title V facility. The public notice is required for sources required to obtain a Plan Approval in accordance with 25 Pa. Code § 127.44. This plan approval will, in accordance with 25 Pa. Code § 127.450 or § 127.505, be incorporated into the facility operating permit at a later date.
Plan approval No. 20-145C is for the proposed PALs for the facility. This Plan Approval will contain emission restrictions, testing, recordkeeping, work practice standard and additional requirement conditions, which will satisfy the requirements of 25 Pa. Code § 127.12b (pertaining to plan approval terms and conditions) and will demonstrate Best Available Technology (BAT) for the source including, but are not limited to, the following:
• Site Level Requirements
• Pursuant to the requirements of 40 CFR 52.21(aa) and 25 Pa. Code § 127.218, the following PSD/NNSR PAL is established for the following pollutants:
• PM (TSP)—299.5 tpy based on a 12-month rolling total
• PM10—287.09 tpy based on a 12-month rolling total
• PM2.5—256.55 tpy based on a 12-month rolling total
• SOx—313.65 tpy based on a 12-month rolling total
• NOx—1859.59 tpy based on a 12-month rolling total
• The emissions shall include fugitive emissions, to the extent quantifiable, from all emissions units that emit or have the potential to emit the PAL pollutant at the major facility as required by 25 Pa. Code § 127.218(c)(4) and 40 CFR 52.21(aa)(4)(i)(d).
• For each month during the PAL effective period after the first 12 months of establishing a PAL, the owner or operator of the major facility shall show that the sum of the monthly emissions from each emissions unit under the PAL for the previous 12 consecutive months, expressed as a 12-month rolling total, is less than the PAL. For each month during the first 11 months from the PAL effective date, the owner or operator of the major facility shall show that the sum of the preceding monthly emissions from the PAL effective date for each emissions unit under the PAL is less than the PAL as required by 25 Pa. Code § 127.218(c)(1) and 40 CFR 52.21(aa)(4)(i)(a).
• The owner or operator of a major facility shall monitor all emissions units in accordance with 25 Pa. Code § 127.218(m) and 40 CFR 52.21(aa)(12).
• The owner or operator shall retain a copy of the records necessary to determine compliance with the PAL, including a determination of the 12-month rolling total emissions for each emissions unit, for 5 years as required by 25 Pa. Code § 127.218(n)(1) and 40 CFR 52.21(aa)(13)(i).
• The owner or operator shall retain a copy of the following records for the duration of the PAL effective period and 5 years after the PAL permit expires as required by 25 Pa. Code § 127.218(n)(2) and 40 CFR 52.21(aa)(13)(ii):
• A copy of the PAL permit application and applications for revisions to the PAL permit.
• Each annual certification of compliance required under Title V of the Clean Air Act (42 U.S.C.A. §§ 7661—7661f) and regulations adopted under the act and the data relied on in certifying the compliance.
• The owner or operator of a major facility shall retain the records required under 25 Pa. Code § 127.218(n) and 40 CFR 52.21(aa)(13) and they shall be retrievable onsite. Such records may be retained in an electronic format.
• The owner or operator of a major facility shall submit the reports required under 25 Pa. Code § 127.218(o) and 40 CFR 52.21(aa)(14) by the required deadlines.
• Emission calculations for compliance purposes shall include emissions from startups, shutdowns and malfunctions as required by 25 Pa. Code § 127.218(g)(4) and 40 CFR 52.21(aa)(7)(iv).
• The owner or operator of a major facility shall use the calculation procedures contained in Air-Emissions, Meadville_CY2018rv2 & Air-Emissions, Meadville_CY2019rv2 spreadsheet from the PAL application (or a Department approved equivalent) to convert the monitoring system data to monthly emissions and annual emissions based on a 12-month rolling total for each month as required by 25 Pa. Code § 127.218(n)(1) and 40 CFR 52.21(aa)(13)(i).
• The PSD and NNSR PAL for emissions are estab- lished for the following sources at the facility:

• The PAL effective period is 10 years from plan approval issuance.
• In accordance with the plantwide applicability limit (PAL) provisions incorporated by reference at 25 Pa. Code § 127.83 and those of 25 Pa. Code § 127.218(k), if the owner or operator of the facility submits a timely application to renew the PAL, the PAL permit will not expire at the end of the PAL effective period. The PAL permit shall remain in effect until the Department issues a revised PAL permit.
• If not required by condition (c)(2), the Department shall renew the PAL unchanged, if the emissions, calculated in accordance with 25 Pa. Code § 127.218(f) and 40 CFR 52.21(aa)(10)(iii) are below 80% of the PAL level solely due to reduced utilization.
• The Department may reduce the PAL in accordance with 25 Pa. Code §§ 127.218(i)(1)(ii), 127.218(i)(2), 127.218(k)(4)(iii), or 40 CFR 52.21(aa)(10)(iv)(c)(1) as necessary.

A physical change or change in method of operation at an existing emissions unit will not be subjected to BAT requirements of this chapter unless the emissions unit is modified so that the fixed capital cost of new components exceeds 50% of the fixed capital cost that would be required to construct a comparable entirely new emissions unit as required by 25 Pa. Code § 127.218(g)(10).

The process change has a potential emission increase of 3.39 tons per year of hazardous air pollutants and 3.39 tons per year of volatile organic carbons. Emissions increases are due to processing lower purity phenol in the distillation columns. 03-00173: Armstrong Terminal, Inc./Schenley Terminal (1 Railroad Street, Schenley, PA 15682). The Department intents to issue a Natural Minor Operating Permit renewal for the operation of barge unloading and bulk commodity storage located in Gilpin Township, Armstrong County. The facility consists of a single dock for unloading barges with one clam shell crane. Requests for an informal conference, or a public hearing, as applicable, on a mining permit application, as provided by 25 Pa. Code § 77.123 (relating to public hearing-informal conferences) or § 86.34 (relating to informal conferences), must contain the name, address and telephone number of the requestor; the application number; a brief summary of the issues to be raised by the requestor at the conference; and a statement whether the requestor desires to have the conference conducted in the locality of the proposed mining activities.

NOTICE OF PROJECTS UNDER THE ENVIRONMENTAL GOOD SAMARITAN ACT The Environmental Good Samaritan Act (27 Pa.C.S.A. §§ 8001—8114) provides certain protections and immunities from civil liability for landowners and persons who voluntarily undertake reclamation and abatement projects to address land and water adversely affected by mining or oil or gas extraction or exploration for natural resources and left in an unreclaimed condition or left discharging water pollution. In order for landowners and persons to qualify for immunity, the projects must be approved by the Department.

SWN Production Company, LLC is planning to plug an abandoned oil and gas well (API No. 015-90007) located in Stevens Township, Bradford County. The P Brown well is located in a wooded area with adjacent agricultural fields surrounding the location. The well plugging project will affect an area of 5 acres. MINING ACTIVITY NPDES DRAFT PERMITS This notice provides information about applications for a new, amended or renewed NPDES permits associated with mining activity (coal or noncoal) permits. The applications concern industrial waste (mining) discharges to surface water and discharges of stormwater associated with mining activities. This notice is provided in accordance with 25 Pa. Code Chapters 91 and 92a and 40 CFR Part 122, implementing provisions of The Clean Streams Law (35 P. S. §§ 691.1—691.1001) and the Federal Clean Water Act (33 U.S.C.A. §§ 1251—1376).
The Department of Environmental Protection (Department) has prepared a draft NPDES permit and made a tentative determination to issue the NPDES permit in conjunction with the associated mining activity permit.
Effluent Limits for Coal Mining Activities
For coal mining activities, NPDES permits, when issued, will contain effluent limits that are the more stringent of technology-based (BAT) effluent limitations or Water Quality Based Effluent Limits (WQBEL).
The BAT limits for coal mining activities, as provided in 40 CFR Part 434 and 25 Pa. Code Chapters 87—90 are as follows:

Similarly, modified BAT limits apply to iron, manganese and suspended solids in surface runoff, discharges and drainage resulting from these precipitation events and those of greater magnitude in accordance with 25 Pa. Code §§ 87.102, 88.92, 88.187, 88.292, 89.52 and 90.102.
Exceptions to BAT effluent limits may be applicable in accordance with 25 Pa. Code §§ 87.102, 88.92, 88.187, 88.292, 89.52 and 90.102.
Effluent Limits for Noncoal Mining Activities
The limits for noncoal mining activities as provided in 25 Pa. Code Chapter 77 are pH 6 to 9 and other parameters the Department may require.
Discharges from noncoal mines located in some geologic settings (for example, in the coal fields) may require additional water quality-based effluent limits. If additional effluent limits are needed for an NPDES permit associated with a noncoal mining permit, then the permit description specifies the parameters.
In addition to BAT or WQBEL limits, coal and noncoal NPDES permits establish effluent limitations in the form of implemented Best Management Practices (BMPs) identified in the associated Erosion and Sedimentation Plan, the Reclamation Plan and the NPDES permit application. These BMPs restrict the rates and quantities of associated pollutants from being discharged into surface waters in this Commonwealth.
More restrictive effluent limitations, restrictions on discharge volume or restrictions on the extent of mining that may occur are incorporated into an NPDES permit when necessary for compliance with water quality standards and antidegradation requirements (in accordance with 25 Pa. Code Chapters 91—96).
The procedures for determining the final effluent limits, using a mass-balance equation or model, are found in Technical Guidance Document 563-2112-115, Developing National Pollutant Discharge Elimination System (NPDES) Permits for Mining Activities. Other specific factors to be considered include public comments and Total Maximum Daily Load(s). Additional discharge limitations may apply in the event that unexpected discharges occur.
Discharge rates for surface mining activities are precipitation driven. Discharge rates for proposed discharges associated with underground mining are noted in the permit description.
Persons wishing to comment on an NPDES draft permit should submit a written statement to the Department at the address of the district mining office indicated before each draft permit within 30-days of this public notice. Comments received within the comment period will be considered in the final determinations regarding the NPDES permit applications. Comments must include the name, address and telephone number of the writer and a concise statement to inform the Department of the exact basis of a comment and the relevant facts upon which it is based.
The Department will also accept requests or petitions for a public hearing on NPDES permit applications, as provided in 25 Pa. Code § 92a.82(d). The request or petition for a public hearing shall be filed within 30-days of this public notice and contain the name, address, telephone number and the interest of the party filing the request and state the reasons why a hearing is warranted. A public hearing may be held if the Department considers the public interest significant. If a hearing is scheduled, a notice of the hearing on the NPDES permit application will be published in the Pennsylvania Bulletin and a newspaper of general circulation within the relevant geographical area. When a public hearing is held, the Department will consider comments from the public hearing in the final determination on the NPDES permit application.

PA DoT propose to replace a Single Span Concrete T-Beam Bridge with a Single Span Pre-Stressed Concrete Spread Box Beam Bridge over Slab Cabin Run. The existing bridge has a span of 21.21 Ft., a skew of 45 degrees, an underclearance of 8.02 Ft., and a low chord of 1,118.98 Ft. and a hydraulic opening of 181 Ft2. The proposed structure has a span of 30.45 Ft., a skew of 35 degrees, an underclearance of 7.16 Ft., and a low chord of 1,119.66 Ft. and a hydraulic opening of 257 Ft2. Both the existing and proposed structures will convey the 100-year storm. The proposed structure will utilize R-8 Rip Rap, choked with R-3 Rip Rap scour protection around the abutment and wingwalls. The project will utilize temporary roadway and 72-inch pipe to convey traffic during construction. The project will utilize a sandbag diversion dike and 36-inch temporary pipe system to convey the stream during construction. The project will include widening and overlaying 2,350 L.F. of SR 0026 and 962 L.F. of SR 0045. A turning lane between SR 0026 and SR 0045 will be added. A driveway near the intersection will be relocated. The project will utilize a temporary construction access road with 18-inch pipe crossing. The project includes 4,955 cubic yards of temporary fill in the floodplain and 2,116 cubic yards of permanent fill in the floodplain. The project will temporarily impact 0.27 acre and permanently impact 0.17 acre of jurisdictional wetlands. The wetland impacts will be debited from Huntington AWC site. Slab Cabin Run is classified as a Cold-Water Fishery by Title 25, Chapter 93 Water Quality Standards and is listed as a Class A Wild Trout Stream by the PA Fish and Boat Commission.
