 A hearing will be conducted before Hearing Officer Thomas A. Blackburn, beginning at 9:30 a.m. on Thursday, December 12, 2024, to receive evidence concerning the revocation of Mountain View Care and Rehabilitation Center's nursing home license. The hearing will be conducted in accordance with 2 Pa.C.S. §§ 501—508 and 701—704 (relating to Administrative Agency Law), 1 Pa. Code Part II (relating to General Rules of Administrative Practice and Procedure) (GRAPP) and the Department of Health's regulations at 37 Pa. Code §§ 197.81—197.94 (relating to appeals from decisions pertaining to licensure) to the extent they supplement or supersede inconsistent provisions of GRAPP.

 The hearing will be held in Hearing Room 2 at 2400 Thea Drive, Suite 201, Harrisburg, PA. The parties shall consult the security officer to confirm the hearing room assignment.
