   By filing no. 96-AA, Capital Blue Cross submits for the Department's review and approval a Member Ambulatory Surgical Facility Prospective Payment Agreement with Ophthalmology Surgical Center, Inc.

   Copies are available for public inspection, by appointment, during normal working hours at the Insurance Department's Office in Harrisburg. This filing consists of 18 pages.
