DISCHARGE OF CONTROLLED INDUSTRIAL WASTE AND SEWERAGE WASTEWATER
(Part I Permits) The following parties have applied for an NPDES permit to discharge controlled wastewaters into the surface waters of this Commonwealth. Unless otherwise indicated, on the basis of preliminary review and application of lawful standards and regulations, the Department of Environmental Protection (Department) proposes to issue a permit to discharge, subject to certain effluent limitations and special conditions. These proposed determinations are tentative.
Where indicated, the EPA, Region III, Regional Administrator has waived the right to review or object to this proposed permit action under the waiver provision 40 CFR 123.6E.
Persons wishing to comment on the proposed permit are invited to submit a statement to the office noted above the application within 30 days from the date of this public notice. Comments received within this 30-day period will be considered in the formulation of the final determinations regarding this application. Responses should include the name, address and telephone number of the writer and a concise statement to inform the Department of the exact basis of a comment and the relevant facts upon which it is based. A public hearing may be held if the responsible office considers the public response significant.
Following the 30-day comment period, the Program Manager will make a final determination regarding the proposed permit. Notice of this determination will be published in the Pennsylvania Bulletin at which time this determination may be appealed to the Environmental Hearing Board.
The application and related documents, proposed effluent limitations and special conditions, comments received and other information are on file and may be inspected and arrangements made for copying at the office indicated above the application.

For the purpose of evaluating effluent requirements for TDS, NO2-NO3, fluoride and phenolics, the existing downstream potable water supply intake considered during the evaluation was Steelton Municipal Waterworks located in Steelton. For the purpose of evaluating effluent requirements for TDS, NO2-NO3, fluoride and phenolics, the existing/proposed downstream potable water supply considered during the evaluation is Emlenton Water Company on Allegheny River located at Emlenton Boro approximately 47.5 miles below point of discharge.

Applications under the Pennsylvania Clean Streams Law
(Part II Permits) The following permit applications and requests for plan approval have been received by the Department of Environmental Protection (Department). Persons objecting on the grounds of public or private interest to the approval of an application or submitted plan may file a written protest with the Department at the address indicated above each permit application or plan. Each written protest should contain the name, address and telephone number of the protester, identification of the plan or application to which the protest is addressed and a concise statement or protest in sufficient detail to inform the Department of the exact basis of the protest and the relevant facts upon which it is based. The Department may conduct a fact-finding hearing or an informal conference in response to any given protest or protests. Each protester will be notified in writing of the time and place of any scheduled hearing or conference concerning the plan or action or application to which the protest relates. To insure consideration by the Department prior to final action on permit application and proposed plans, initial protests and additions or amendments to protests already filed should be filed within 15 calendar days from the date of this issue of the Pennsylvania Bulletin. A copy of each permit application and proposed plan is on file in the office indicated and is open to public inspection.

Limitations are provided in the permit as erosion and sedimentation control measures and facilities which restrict the rate and quantity of sediment discharged.
Where indicated, the EPA, Region III, Regional Administrator has waived the right to review or object to this proposed permit action under the waiver provision of 40 CFR 123.24(d).

Conewago Township, Adams CountyThe Department of Environmental Protection (DEP) under the authority of the Hazardous Sites Cleanup Act (35 P. S. §§ 6020.102--6020.1303) (HSCA) is initiating an interim response at the Hanover Scrap Quarry Site in Conewago Township, Adams County, Pennsylvania.
Hanover Scrap Quarry is a 5-acre site located along Narrow Drive 1/2 mile from Route 194 near the village of Mount Pleasant. The site is situated in a rural area that is predominately residential and agricultural. The former quarry occupies 1 acre of the site.
The site operated as a quarry during the 1940s and later was used as an unregulated dumping area for industrial and municipal wastes until the 1970s when the pit was completely filled. In 1987 homes using wells with contaminated groundwater were provided public water by the estate of the property owner (Estate). The quarry was determined to be the source of volatile organic compounds (VOCs) which contaminated the groundwater. In 1992, the Estate, under a Consent Decree and Order with DEP removed 200 abandoned drums of waste from the site. Sampling by DEP in 1993 showed metals contamination of site soils to be above health based risk levels.
DEP determined that an interim response is necessary to protect the public health and safety. Two alternatives were considered:
* No action, which would allow contaminated soils to remain in place.
* The removal of soils above health based risk levels and arranging for the treatment and/or disposal of these soils at a permitted facility.
DEP determined that the no action alternative would not be protective of human health and the environment since the risk of contact with contaminated soils would remain.
DEP is providing this notice under sections 505(b) and 506(b) of HSCA (35 P. S. §§ 6020.505(b) and 6020.506(b)) and the publication of this notice starts the administrative record period under HSCA. The Administrative Record which contains information about this site and which supports DEP's decision to perform this action at the site is available for public review and comment. The Administrative Record can be examined from 8 a.m. to 3:30 p.m. at DEP's office at 909 Elmerton Avenue, Harrisburg, PA 17110, by contacting Barbara Faletti at (717) 705-4864. Acknowledgment of Notices of Intent to Remediate submitted under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908).
Sections 302 and 303 of the Land Recycling and Environmental Remediation Standards Act (act) require the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin an acknowledgment noting receipt of any Notices of Intent to Remediate. An acknowledgment of the receipt of a Notice of Intent to Remediate identifies a site where a person proposes to, or has been required to, respond to a release of a regulated substance at a site. Persons intending to use the background or Statewide health standard to remediate a site must file a Notice of Intent to Remediate with the Department. A Notice of Intent to Remediate filed with the Department must provide a brief description of the location of the site, a list of known contaminants at the site, the proposed remediation measures for the site, and a description of the intended future use of the site. A person who demonstrates attainment of one or a combination of the cleanup standards identified under the act will be relieved of further liability for the remediation of the site for any contamination identified in reports submitted to and approved by the Department and shall not be subject to citizen suits or other contribution actions brought by responsible persons not participating in the remediation.

Non-Major Sources and Modifications The Department of Environmental Protection (Department) has developed an integrated plan approval, State operating permit and Title V operating permit program. This integrated approach is designed to make the permitting process more efficient for Department, the regulated community and the public. This approach allows the owner or operator of a facility to complete and submit all the permitting documents relevant to its application one time, affords an opportunity for public input and provides for sequential issuance of the necessary permits.
The Department has received applications for plan approvals and/or operating permits from the following facilities. Although the sources covered by these applications may be located at a major facility, the sources being installed or modified do not trigger major new source review or prevention of significant deterioration requirements.

Notice of Intent to Issue Title V Operating Permits
Under 25 Pa. Code § 127.521, notice is given that the Department of Environmental Protection (Department) intends to issue a Title V Operating Permit to the following facilities. These facilities are major facilities subject to the operating permit requirements under Title V of the Federal Clean Air Act and 25 Pa. Code Chapter 127, Subchapters F and G (relating to operating permit requirements; and Title V operating permits).

Notice of Intent to Approve
Plan Approval Application #11-005B Notice is hereby given, under 25 Pa. Code § 127.44, that the Department of Environmental Protection (Department) intends to issue a Plan Approval to Bar Technologies, Inc. to modify the existing caster equipment to produce leaded steel grades on the continuous caster. This will also include the installation of air cleaning equipment (baghouse) specifically for leaded steel production. Bar Technologies, Inc. (1001 Main Street--Gate 3 Johnstown, PA 15909) is located at Franklin Borough, Cambria County.
The proposal is to manufacture leaded steel instead of carbon steel and may result in the emission of lead and lead compounds of less than 2.37 pounds per hour and 3,000 pounds per year.
Copies of the application, Department's analysis and other documents used in evaluation of the application are available for public inspection during normal business hours at the address below.
In order to assure compliance with all applicable standards, Department proposes to place the following conditions on the Plan Approval:
General Conditions
3. This plan approval is for the modification of the continuous billet/bloom caster and for installation of a baghouse and a ventilation system on the caster by Bar Technologies, Inc. at their Johnstown facility in Franklin Borough, Cambria County. This is in accordance with 25 Pa. Code § 127.32.
4. The facility's potential to emit and annual allowable emission rates are established based on a maximum steel-making rate of 1.2 million tons per year in accordance with 25 Pa. Code § 127.12b.
5. The maximum caster production rate for all steels shall not exceed 5,200 tons per day and 1.2 million tons per year in accordance with 25 Pa. Code § 127.12b.
6. The new caster baghouse shall be constructed as stated in the application. The baghouse shall be equipped with Gortex membrane felted polyester bags (or equipment) in accordance with 25 Pa. Code § 127.25 and § 127.1.
7. Particulate Matter < 10 microns (PM-10) emissions from the caster baghouse shall not exceed an outlet grain loading of 0.003 gr/dscf. Lead and lead compound emissions shall not exceed 0.0025 gr/dscf, 2.37 lbs/hr, and 3,000 pounds per year in accordance with 25 Pa. Code § 127.12b and § 127.25.
8. A mechanical or electrical interlock shall be installed to prevent lead from being fed to the caster if the baghouse is not operating in accordance with 25 Pa. Code § 127.12b.
9. The two bay doors nearest the caster torch cutting station shall remain fully closed when leaded steel is being produced in accordance with 25 Pa. Code §§ 127.123.1 and 127.12b.
10. Visible emissions from the continuous caster baghouse shall not equal or exceed 10% opacity in accordance with 25 Pa. Code §§ 127.1 and 127.12b.
11. There shall be no fugitive emissions present from this facility in accordance with 25 Pa. Code §§ 123.1 and 127.12b.
12. The owner/operator shall perform stack testing for particulate matter (as PM-10) and for lead and lead compound emissions in accordance with 25 Pa. Code Chapter 139 regulations and the Department's source testing manual. Section 6.1 of the Clean Air Act and 25 Pa. Code § 127.12b.
13. The stack test shall be performed while the continuous billet/bloom caster is operating at the maximum normal casting and lead injection rate. If stack testing is performed while the caster is operating at a lower casting rate or lower lead injection rate, then production shall be limited to the levels at which compliance has been demonstrated. Section 6.1 of the Clean Air Act and 25 Pa. Code § 139.11.
14. Two copies of a pretest protocol shall be submitted to the Department for review at least 60 days prior to the performance of the stack test in accordance with 25 Pa. Code § 139.2.
15. The Department shall be notified at least 2 weeks prior to the stack test of the date and time of the test so that an observer may be present in accordance with 25 Pa. Code § 139.2.
16. Two copies of the stack test results shall be supplied to the Department for review within 60 days of completion of testing in accordance with 25 Pa. Code § 139.2.
17. The Department may revise the allowable emission rates based on the results of the stack tests in accordance with 25 Pa. Code §§ 127.1 and 127.12b.
18. The owner/operator shall install, maintain and operate the caster baghouse and the caster ventilation system in accordance with manufactures specifications and in accordance with good air pollution control practices. This shall be done in accordance with 25 Pa. Code § 127.25.
19. A daily log shall be kept of all material throughput (lead and non-leaded steel production). This file shall be maintained for a period of not less than 2 years and shall be made available to the Department upon request in accordance with 25 Pa. Code § 127.12b.
20. A monthly log shall be kept of all lead used in the production of this leaded steel in accordance with 25 Pa. Code §§ 127.12b and 127.12a.
21. The baghouse and the caster ventilation system must operate at all times when leaded steel is being produced in accordance with 25 Pa. Code § 127.12b and § 127.25.
22. The owner/operator shall maintain spare bags in accordance with the manufactures recommendation to insure adequate maintenance of the air cleaning device in accordance with 25 Pa. Code § 127.25.
23. This Plan Approval authorizes temporary operation of the source covered by this Plan Approval provided the following conditions are met. In accordance with 25 Pa. Code § 127.12b.
a. The Department must receive written notice from the owner/operator of the completion of construction and the operator's intent to commence operation at least 5 working days prior to the completion of construction. The notice should state when construction will be completed and when operator expects to commence operation.
b. Operation is permitted only to facilitate the start-up and shake-down of sources and air cleaning devices, to permit operations pending the issuance of an Operating Permit or to permit the evaluation of the sources for compliance with all applicable regulations and requirements.
c. This condition authorizes temporary operation of the sources for a period of 180 days from the date of commencement of operation, provided the Department receives notice from the owner/operator under subpart (1), above.
d. The owner/operator may request an extension if compliance with all applicable regulations and Plan Approval requirements has not been established. The extension request shall be submitted in writing at least 15 days prior to the end of this period of temporary operation and shall provide a description of the compliance status of the source, a detailed schedule for establishing compliance, and the reasons compliance has not been established.
e. The notice submitted by the owner/operator under subpart (1), above, prior to the expiration of this Plan Approval, shall modify the plan approval expiration date. The new Plan Approval expiration date shall be 180 days from the date of the written notice.
In accordance with 25 Pa. Code § 127.45 the conditions listed in this plan approval do one or more of the following:
1. Identify the source and location.
2. Establish allowable emission rates.
3. Establish monitoring, recordkeeping, testing and reporting requirements.
4. Insure proper operation and adequate maintenance.
5. Insure compliance with other Department programs' requirements.
6. Establish the bases for the existing facility's actual emissions (baseline) and the modified facility's allowable emissions.
Any person wishing to provide the Department with additional information which they believe should be considered prior to the issuance of the Plan Approval may submit the information to the Department of Environmental Protection at the address shown below. A 30-day comment period, from the date of publication, will exist for the submission of comments. Each written comment must contain the following:
Name, address and telephone number of the person submitting the comments.
Identification of the proposed Plan Approval (specify Plan Approval #11-005B).
Concise statements regarding the relevancy of the information or objections to issuance of the Plan Approval. A public hearing may be held, if the Department of Environmental Protection in its discretion, decides that such a hearing is warranted based on the information received. Reasonably Available Control Technology (RACT)
J. L. Clark, Inc., RACT Permit No. 36-2009 The Department of Environmental Protection (Department) has made a preliminary determination to approve a RACT Plan and proposes to revise the State Implementation Plan (SIP) for J. L. Clark, Inc., 303 North Plum Street, in the City of Lancaster, Lancaster County.
The proposed SIP revision does not adopt any new regulations. It incorporates the provisions and requirements contained in RACT Operating Permit No. 36-2009 for the existing facility to comply with current regulations. The preliminary VOC RACT determination for the surface coating and printing operations is the use of compliant coatings and/or incineration.
The preliminary RACT determination, when finally approved, will be incorporated into an operating permit for the facility and will be submitted to the United States Environmental Protection Agency (EPA) as a revision to Pennsylvania's SIP.

APPLICATIONS TO CONDUCT COAL AND NONCOAL ACTIVITIES Applications under the Surface Mining Conservation and Reclamation Act (52 P. S. §§ 1396.1--1396.19a); the Noncoal Surface Mining Conservation and Reclamation Act (52 P. S. §§ 3301--3326); The Clean Streams Law (35 P. Where any of the mining activities listed will have discharges of wastewater to streams, the Department will incorporate NPDES permits into the mining activity permits issued in response to these applications. The NPDES permits will contain, at a minimum, technology-based effluent limitations (as described in the Department's regulations--25 Pa. Code §§ 77.522, 87.102, 88.92, 88.187, 88.242, 89.52 and 90.102) for iron, manganese, suspended solids, settleable solids, alkalinity and pH. In addition to the above, more restrictive effluent limitations, restrictions on discharge volume or restrictions on the extent of mining which may occur will be incorporated into a mining activity permit when necessary for compliance with water quality standards (in accordance with 25 Pa. Code Chapters 93 and 95). Persons or agencies which have requested review of the NPDES permit requirements for a particular mining activity within the above-mentioned public comment period will be provided with a 30-day period to review and submit comments on those requirements.

Section 702 of the Bituminous Coal Mine Act (52 P. S. § 702) provides a mechanism for operators to obtain variances from specific requirements of the act (act) to accommodate the adoption of new machinery, equipment, tools, supplies, methods or processes.
Section 242(c) states that where belt conveyors are installed, main stoppings and regulators shall be so arranged as to reduce the quantity of air traveling in the belt conveyor entry to a minimum for effective ventilation and to provide an intake air split as an escapeway from the face area to the main air current.
Summary of the request: Tanoma Mine requests a section 242(c) variance to allow for the common ventilation of belt conveyor entries with other entries.

E15-579. Encroachment, PA Dot, 200 Radnor-Chester Rd, St. Davids, PA 19087, and Tredyffrin Township, 1100 Duportail Rd, Berwyn, PA 19312. To extend Chesterbrook Boulevard (T-843) from U. S. Route 202 to Swedesford Road (S. R. 0252), opposite Cassatt Road (S. R. 1007) and to provide full access to the Chesterbrook Interchange by constructing a new southbound U. S. Route 202 on-ramp and a new northbound U. S. Route 202 off-ramp (Valley Forge, PA Quadrangle N: 11.4 inches; W: 12.3 inches) in Tredyffrin Township, Chester County.
Work consists of:
1. To elongate the existing 6-foot × 6-foot box culvert stream enclosure in and along an unnamed tributary to Little Valley Creek beneath SR 0202 at station 607+50 by extending 17-feet at the downstream end for the new southbound on-ramp, and by 20 feet at the upstream end for the new northbound off-ramp. Work will also include a channel relocation of approximately 120 feet at the upstream end of the extended stream enclosure. The proposed activities will impact 0.15 acre of associated wetlands (PEM/SS/FO).
2. To construct and maintain 180 linear feet of 8-foot by 5-foot reinforced concrete box culvert stream enclosure in and along an unnamed tributary to Little Valley Creek beneath the proposed Chesterbrook Boulevard, and near the intersection of Cassatt Road and Swedesford Road. Work also includes the replacement and maintenance of a headwall for four existing culverts (93-inch × 64-inch Arch pipe, 53-inch × 41-inch arch pipe, 36-inch R.C.P. and 24-inch R.C.P.) within a junction chamber at Swedesford Road. The proposed junction chamber will connect the upstream end of the elongated stream enclosure to the four existing culverts. These proposed activities will impact 0.10 acre of associated wetlands (PEM/SS/FO).
The permittee proposes to provide replacement wetlands.
E15-581. Encroachment, West Grove Borough Authority, P. O. Box 61, West Grove, PA 19390. To replace the existing West Grove Borough Authority sanitary sewer line which crosses an unnamed tributary to the East Branch of the White Clay Creek (CWF). The proposed 8-inch PVC pipe replacement will begin at the intersection of Rosehill Avenue and Oakland Avenue and continue south from this intersection approximately 650 feet. The site is located approximately 1,200 feet southeast of the intersection of Old Baltimore Pike with Oakland Avenue (West Grove, PA-DEL, Quadrangle N: 12.5 inches; W: 10.1 inches) in West Grove Borough, Chester County.
E15-582. Encroachment, Willistown Twp, 688 Sugartown Rd, Malvern, PA 19355. To install and maintain a section of 30-inch HCMP storm sewer line and outfall structure and appurtenant riprap in the 100-year floodplain and floodway of an unnamed tributary to Crum Creek (HQ, CWF) located along and south of Paoli Pike (S. R. 2014) at a point 45 feet west of its intersection with Warren Avenue (Malvern, PA Quadrangle N: 4.55 inches; W: 1.15 inches) in Willistown Township, Chester County.
E46-808. Encroachment, Abington Township, 1176 Old York Road, Abington, PA 19001. To modify and maintain an existing stream enclosure in and along an unnamed tributary to Pennypack Creek (TSF-MF). The proposed project consists of enclosing approximately 350 feet of man-made channel. The existing twin 8-foot by 4.5-foot concrete box culvert which conveys flow beneath Mary Avenue will be extended downstream for approximately 75 feet. The remaining approximately 210 feet of open channel between Irvin Road culvert and Mary Avenue culvert will be enclosed with twin 8-foot by 4-foot concrete box stream enclosure attached to these culverts. This project is the first phase of the proposed four phases of Rockledge Flood Control Project to reduce flooding and stabilize the channel cross-sections. The site is located in the vicinity of Irvin Road and Mary Avenue, approximately 1,500 feet south from the intersection of Huntington Pike (S. R. 232) and Susquehanna Road (Frankford, PA-NJ Quadrangle N: 17.0 inches; W: 12.5 inches in Abington Township, Montgomery County.
E46-809. Encroachment, Goldenberg Development Corporation, 350 Sentry Parkway, Bldg. 630, Suite 300, Blue Bell, PA 19422-2316. To perform the following activities associated with the construction of the Metroplex Land Development Project:

Route 119 Corridor, East Huntingdon Township, Westmoreland County. This service area extends from the Westmoreland County line along Jacobs Creek to the intersection of S. R. 119 and S. R. 31 in East Huntingdon Township, Westmoreland County.
Project Description: Approval of an Update Revision to the Official Sewage Facilities Plan of East Huntingdon Township, Westmoreland County. Project involves the construction of sewage collection, conveyance and treatment facilities for the Route 119 corridor in East Huntingdon Township. This project will service approximately 962 Equivalent Dwelling Units in the project area with the construction of a new sewage treatment plant on the former Modulus Corporation property. The proposed discharge is to Jacobs Creek, a tributary of the Youghiogheny River. The Department's review of the sewage facilities update revision has not identified any significant environmental impacts resulting from this proposal.

The treatment plant will be located south of Hartstown on the east side of PA 18, and treated effluent will be discharged to the Crooked Creek watershed below Mud Lake.

The proposed development is located on the west side of S. R. 0501, approximately 1,400 feet north of Millport Road (T-715), Warwick Township, Lancaster County.
The approved project consists of an eight lot commercial/residential subdivision. Lot No. 1 is proposed for a shopping center with sewage flows of 19,275 gallons per day (gpd). Lot No. 7 contains an existing video store with sewage flows of 325 gpd. Lot No. 8 contains an existing drugstore with sewage flows of 545 gpd. Lot Nos. 2 and 3 are proposed for future commercial development and have been allocated one EDU (225 gpd) each. Lot Nos. 4, 5, and 6 are proposed for future residential development and are allocated one EDU each. The total sewage flows for the development of 21,270 gpd of wastewater will be treated by the Lititz Borough Wastewater Treatment Plant. The proposed development is located on the west side of S. R. 0501, approximately 1,400 feet north of Millport Road (T-715), Warwick Township, Lancaster County.

Provisions of Chapter 3 of the Land Recycling and Environmental Remediation Standards Act (act) require the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin notice of submission of any final reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the act's remediation standards. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or non-residential exposure factors, a description of the remediation performed, and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected.

LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

The Department has taken action on the following plans and reports under the Land Recycling and Environmental Remediation Standards Act (35 §§ 6026.101--6026.908) and Chapter 250 Administration of Land Recycling Program
Provisions of 25 Pa. Code § 250.8 Administration of Land Recycling Program requires the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin a notice of its final actions on plans and reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the Act 2 (Land Recycling and Environmental Remediation Standards Act) remediation standards. Plans and reports required by provisions of Act 2 for compliance with selection of remediation to a site-specific standard, in addition to a final report, include a remedial investigation report, risk assessment report, and cleanup plan. A remedial investigation report includes conclusions from the site investigation, concentration of regulated substances in environmental media, benefits of reuse of the property, and in some circumstances, a rate and transport analysis. If required, a risk assessment report describes potential adverse effects caused by the presence of regulated substances. A cleanup plan evaluates the abilities of potential remedies to achieve remedy requirements. A final report provides a description of the site investigation to characterize the nature and extent of contaminants in environmental media, the basis for selecting the environmental media of concern, documentation supporting the selection of residential or nonresidential exposure factors, a description of the remediation performed and summaries of sampling methodology and analytical results which demonstrate that the remediation has attained the cleanup standard selected. A permit approving the major permit modification for a Gas Management System at this municipal waste landfill, located in Dunmore and Throop Boroughs, Lackawanna County. The modification approval is only for new wells and a collection system to be installed in the Tabor site, the installation of the new vacuum blower, and a booster compressor station. The permit was issued in the Regional Office on June 9, 1998.

E59-359. Encroachment, PA Dept. of Transportation, 715 Jordan Ave., Montoursville, PA 17754. To permanently encroach upon 4.6 acres of wetland and to construct, operate and maintain 10 permanent stream encroachments for the upgrade of a 5.4 mile corridor of an existing highway to a limited access four lane highway. The stream encroachments shall consist of 1) the enclosure of Flower Run with a 66 inch × 591 foot culvert pipe and a 100 foot long rock apron 2) the 200 feet of an unnamed tributary to Johnson Creek channel relocation; 3) a six span bridge across Johnson Creek having a total span of 758 feet and width of 50 feet for the northbound lane 4) a six span bridge across Johnson Creek having a total span of 754 feet and a width of 50 feet for the southbound lane; 5) the enclosure of Mills Creek with a 14 foot × 7 foot × 336 foot box culvert and a 50 foot long rock apron; 6) the 80 feet of Mills Creek channel relocation; 7) the enclosure of Boone Run with a 14 foot × 7 foot × 515 foot box culvert and a 50 foot long rock apron; 8) the 30 feet of Boone Run channel relocation; 9) the 90 feet of an unnamed tributary to Johnson Creek channel relocation; and 10) the enclosure of an unnamed tributary to the Tioga River with a 42 inch × 80 foot culvert pipe; and 11) the placement of 165,000 cubic yards of fill in the floodway of an unnamed tributary to the Tioga River. This permit also authorizes the temporary impact to 1.6 acres of wetland and 1,905 feet of waterway for the associated construction crossings and stream diversions. The project is located along the eastern right-of-way of existing SR 0015 approximately 0.5 mile south of the Blossburg Borough corporate boundary (Blossburg, PA Quadrangle N: 6.0 inches; W: 12.0 inches) in Bloss, Covington, Hamilton, and Liberty Townships and Blossburg Borough, Tioga County. The permittee shall fully restore all temporarily impacted waterways and wetlands. Permanent wetland impacts shall be mitigated by the construction of 4.6 acres of replacement wetlands.
E60-132. Encroachment, Donald A. Byerly, R. R. 3, Box 720, Milton, PA 17847. To construct and maintain a bank-to-bank wooden decked steel I-beam bridge with a clear span of 25 feet, a width of 4 feet and an approximate underclearance of 5 feet across Limestone Creek located approximately 250 feet southwest of the intersection of SR 0045 and T-456 (Lewisburg, PA Quadrangle N: 14.83 inches; W: 4.75 inches) in East Buffalo Township, Union County.
