This facility is a non-Title V facility. The emergency generators shall only be used as a backup power source to provide continuous electrical power for medical care services being provided to patients. Potential emissions of nitrogen oxides shall be less than 2 tons per year. Potential emissions of CO, VOCs, PM and SOx will each be less than 1 ton per year. The 750 kW emergency generators, as a set, will be restricted to a total of 100 hours of operation per year. The same hourly restriction will also apply to the 260 kW emergency generators. The Plan Approval will contain recordkeeping and further operating restrictions designed to keep the facility operating within the allowable emissions and all applicable air quality requirements.

40-328-004B: GEO Specialty Chemicals--Trimet Product Group (2409 North Cedar Crest Boulevard, Allentown, PA 18104) for modification and operation of calcium formate process at their facility in Whitehall Township, Lehigh County. This modification involves installation of a baghouse to collect nuisance fugitive dust emitted from the calcium formate process equipment. GEO Specialty Chemicals is a major facility subject to Title V permitting requirements. The modification will increase particulate emissions by 780 pounds per year from the facility. The Plan Approval will contain conditions requiring the source to show compliance with the emission rates. The modification includes the construction of a three-deck screen and various conveyors. The plant is a non-Title V (State-only) facility. The crushing plant is subject to 40 CFR Part 60, Subpart OOO--Standards of Performance for Nonmetallic Mineral Processing Plants. This modification will increase the potential particulate emissions by 4.6 tons per year. 06-03126: Lehigh Cement Co. (204 Windsor Avenue, Hamburg, PA 19526-8341) for installation of three bin vent collectors to control emissions from five bulk storage silos at their Hamburg Facility in Windsor Township, Berks County. The mortar mixing and packaging plant is a non-Title V (State-only) facility. The approval will include monitoring, work practices, recordkeeping and reporting requirements designed to keep the source operating within all applicable air quality requirements.
07-05033B: Grannas Brothers Stone and Asphalt Co., Inc. (P. O. Box 488, Hollidaysburg, PA 16648) to modify the operation of the existing no. 2 fuel oil fired hot mix asphalt plant to allow the plant to be fired on no. 2/recyled oil at the Ganister Quarry in Catharine Township, Blair County. This source is subject to 40 CFR Part 60, Subpart I--Standards of Performance for Hot Mix Asphalt Facilities. The proposed operation will increase the potential emissions of SOx from 42.6 to 52.1 tons per year. The potential HAPs emissions will be less than 9.50 tons per year. The approval will contain production limits, monitoring, work practice, recordkeeping and reporting requirements designed to keep the source operating within all applicable air quality requirements.
29-03008A: H. B. Mellott Estate, Inc. (100 Mellott Drive, Suite 100, Warfordsburg, PA 17267) for installation of a replacement fabric filter at a limestone crushing plant in Bethel Township, Fulton County. Potential emissions from the plant are estimated at several tons per year and no emissions increase is expected from this action. The crushing plant is subject to 40 CFR Part 60, Subpart OOO--Standards of Performance for Nonmetallic Mineral Processing Plants. The Plan Approval and State-only Permit will contain emission limits, testing, monitoring, work practices, recordkeeping and reporting requirements to ensure the facility complies with the applicable air quality requirements.
36-03145: NTN-BCA Corp. (P. O. Box 1400, Lititz, PA 17543) for installation of a batch vapor cleaning machine (degreaser) in their facility at 401 West Lincoln Avenue in Lititz Borough, Lancaster County. The batch vapor cleaning machine has a potential to emit 14 tons per year of VOCs. Actual VOC emissions are expected to be less than 4 tons per year. The plan approval will contain monitoring, work practices, recordkeeping and reporting requirements to ensure the facility complies with the applicable air quality regulations. When the process is accepted by the Department, NTN-BCA Corporation will be able to convert the plan approval to a State-only operating permit.

Code Chapter 127, Subchapter G. The proposed Title V Operating Renewal does not adopt any new regulations and does not reflect any change in air emissions from the facility. 06-05030: Caraustar Mill Group, Inc. (3110 Paper Mill Road, Sinking Spring, PA 19608) for operation of a paperboard mill in Spring Township, Berks County. The facility is a non-Title V (State-only) facility. The facility will be required to limit the emissions to less than 100 tons of particulate, SOx, NOx and CO, 50 tons of VOCs and 10/25 tons of HAPs, during any consecutive 12-month period. Limits will be placed on the hours of operation of the boiler's use of fuel oil. Records of the emissions, hours of operation and fuel usage will be required monthly and on a 12-month rolling total basis. The permit will include monitoring, work practices, recordkeeping and reporting to keep the facility operating within all applicable air quality requirements.
22-03055: Kimmel's Coal and Packaging (Machamer Avenue, P. O. Box 1, Wiconisco, PA 17097) for operation of a coal preparation facility in Wiconisco Township, Dauphin County. This is a non-Title V (State-only) facility. The Operating Permit will contain appropriate conditions designed to keep the facility operating below Title V thresholds for criteria pollutants and within all other applicable air quality requirements.
28-05027: The Chambersburg Hospital (112 North 7th Street, Chambersburg, PA 17201) for operation of their boiler plant in Chambersburg Borough, Franklin County. The facility emissions of SO2, NOx, CO and PM10 will each be limited to less than 100 tons per year. The facility VOC emissions will be limited to less than 50 tons per year. Facility emissions of individual and aggregate HAPs will be limited to less than 10 tons per year and 25 tons per year, respectively. The State-only Operating Permit will include testing, monitoring, recordkeeping and reporting requirements, emission restrictions and work practice standards designed to keep the facility operating within all applicable air quality requirements.
36-03107: Pennfield Corp. (711 Rohrerstown Road, Lancaster, PA 17604) for operation of a livestock feed plant at 1088 East Main Street, Mount Joy, PA 17552 in Mount Joy Borough, Lancaster County. The plant has a potential to emit 5 tons per year of PM10, 10 tons per year of NOx and 37 tons per year of SOx. The State-only operating permit shall contain testing, monitoring, recordkeeping and reporting requirements, emission restrictions and work practice standards designed to keep the facility operating within all applicable air quality requirements.
36-03024: Astro Machine Works, Inc. (470 Wenger Drive, Ephrata, PA 17522) for operation of a manufacturing facility in Ephrata Borough, Lancaster County. The facility has the potential to emit 46.4 tons per year of VOCs, 22.8 tons per year of combined HAPs, 2.3 tons per year of NOx, 1.9 tons per year of CO, less than 1 ton per year of SOx and PM. The State-only Operating Permit shall contain additional testing, monitoring, recordkeeping and reporting requirements, emission restrictions and work practice standards designed to keep the facility operating within all applicable air quality requirements.

49-00053: Shumaker Industries, Inc. (924 Water Street, Northumberland, PA 17857) for operation of their cement drum refurbishing facility in Northumberland Borough, Northumberland County. The facility's main sources include shot blasting and surface coating operations. The particulate matter emissions are controlled by a cartridge collector and over spray filters. The facility has the potential to emit SOx, NOx, CO, PM10, VOCs and HAPs below the major emission thresholds. The proposed operating permit contains all applicable regulatory requirements including monitoring, recordkeeping and reporting conditions.
17-309-024: Mt. Savage Specialty Refractories Co. (P. O. Box 60, Curwensville, PA 16833) for operation of a refractories manufacturing facility incorporating an apron feeder, roll crusher, jaw crusher, transfer chute, natural gas-fired rotary dryer, 27 associated belt conveyors, elephant trunk, monolithics bins, 32 tipple bins, syntron feeders, maximuller, material bucket elevator, material sizing screens, return chute to maximuller from screens, 44 sized material storage bins, 2 holding/additive hoppers, batch car/weigh lorry, 3 mixers, 2 skip hoists, 2 weigh stations, 2 supersack filling stations, additives conveyor, mixer and bagging station in Pike Township, Clearfield County.
The particulate matter emissions from the apron feeder, jaw crusher, roll crusher, transfer chute, rotary dryer and five associated conveyors are controlled by a wet scrubber. The particulate matter emissions from the additional operations are controlled by several fabric collectors. The refactories manufacturing facility could emit up to 20.2 tons of PM per year, of which 4.4 tons per year could be PM10. Emissions of other air contaminants, including HAPs, are minimal.
The Department has determined that the equipment in the refractories manufacturing facility has been reactivated or constructed and is operating, in accordance with all conditions of Plan Approval 17-309-024 as well as in compliance with all applicable regulatory requirements pertaining to air contamination sources and the emission of air contaminants including the best available technology requirements of 25 Pa. Code §§ 127.1 and 127.12. The Department consequently intends to issue an operating permit for the operation of the respective refractories manufacturing facility.
The following is a summary of the conditions the Department proposes to place in the operating permit to be issued to ensure compliance with all applicable regulatory requirements:
1. The PM in the exhaust of the scrubber shall not exceed .02 grain per dry standard cubic foot.
2. The PM in the exhaust of each fabric collector shall not exceed .01 grain per dry standard cubic foot. Additionally, there shall be no visible emissions from the exhaust of any fabric collector at any time.
3. There shall be no visible fugitive emissions from any portion of the refractories manufacturing facility other then what the Department determines to be of minor significance.
4. The permittee shall process in the refractories manufacturing facility only Mulcoa 47, Mulcoa 60, refractory brick bats or additives as identified in the application and supplemental materials submitted for plan approval.
5. The rotary dryer shall be fired on natural gas only. Additionally, the dryer shall not be operated at a process rate in excess of 25 tons per hour.
6. The scrubber shall operate at a minimum scrubbing solution recirculation rate of 60 gallons per minute and at a minimum differential pressure of 14.0 inches of water.
7. The scrubber shall be equipped with instrumentation to monitor the differential pressure across the scrubber on a continuous basis and flow monitoring equipment capable of measuring the scrubbing solution recirculating flow rate on a continuous basis. Each fabric collector shall be equipped with instrumentation to measure the differential pressure across the collector on a continuous basis.
8. Under no circumstances shall the scrubber be used to simultaneously control the air contaminant emissions from the apron feeder, jaw crusher, roll crusher, transfer chute and five associated conveyors and the air contaminant emissions from the rotary dryer.
9. The scrubber shall be drained and cleaned at the end of each 5 day workweek.
10. Each fabric collector shall be inspected at least once every 3 months for damaged or worn bags.
11. For each occurrence of rotary dryer operation, a record shall be maintained of the number of hours the dryer operated and the identity and amount of material processed.
12. A logbook shall be maintained in which all occurrences of scrubber draining/cleaning, documentation of all scrubber inspections and repairs, documentation of all fabric collector inspections and repairs and documentation of all fugitive emission and visible air contaminant emission observations are recorded.

The following permit applications to conduct mining activities have been received by the Department of Environmental Protection (Department). A copy of the application is available for inspection at the District Mining Office indicated before an application. Where a 401 Water Quality Certification is needed for any aspect of a particular proposed mining activity, the submittal of the permit application will serve as the request for certification.
Written comments, objections or requests for informal conferences on applications may be submitted by any person or any officer or head of any Federal, State or local government agency or authority to the Department at the same address within 30 days of this publication, or within 30 days after the last publication of the applicant's newspaper advertisement, as provided by 25 Pa. Code §§ 77.121--77.123 and 86.31--86.34.
Where any of the mining activities listed will have discharges of wastewater to streams, the Department will incorporate NPDES permits into the mining activity permits issued in response to these applications. NPDES permits will contain, at a minimum, technology-based effluent limitations as identified in this notice for the respective coal and noncoal applications. In addition to the previous, more restrictive effluent limitations, restrictions on discharge volume or restrictions on the extent of mining which may occur will be incorporated into a mining activity permit, when necessary, for compliance with water quality standards (in accordance with 25 Pa. Code Chapters 93 and 95). Persons or agencies who have requested review of the NPDES permit requirements for a particular mining activity within the previously mentioned public comment period will be provided with a 30 day period to review and submit comments on those requirements.
Written comments or objections should contain the name, address and telephone number of the person submitting comments or objections; application number; and a statement of sufficient detail to inform the Department on the basis of comment or objection and relevant facts upon which it is based. Requests for an informal conference must contain the name, address and telephone number of requestor; application number; a brief summary of the issues to be raised by the requestor at the conference; and a statement whether the requestor wishes to have the conference conducted in the locality of the proposed mining activities.

The applicant proposes to remove the existing bridge and appurtenant structures and an existing 36-inch diameter CMP culvert outfall and to construct and maintain a two span prestressed concrete box beam bridge, one cell having a span of 57.75 feet and underclearance of 8.6 feet and the other cell having a span of 57.75 feet and an underclearance of 6.3 feet over Tenmile Creek (TSF, perennial), to construct and maintain three 18-inch diameter stormwater outfalls to Tenmile Creek and one concrete pipe arch culvert having a span of 4.88 feet and underclearance of 3.00 feet to Tenmile Creek and to construct and maintain a temporary stream crossing consisting of three 36-inch diameter CMP culverts. Also proposed is the construction and maintenance of two separate temporary roadway crossings in Saxman Run each consisting of four 30-inch diameter pipes and having lengths of 95.0 feet and 26.0 feet respectively, the temporary placement and maintenance of fill in 0.0418 acre of PEM wetland and the permanent placement and maintenance of fill in a de minimis area of PEM wetland equal to 0.0122 acre. The applicant proposes to remove the existing structure and to construct and maintain a 50-foot long concrete box culvert having a 12-foot wide by 5-foot high waterway opening in Jackson Run (WWF, perennial) on Railroad Street approximately 0.4 mile west of the intersection of SR 86 and U. S. Route 19. Project includes construction of wingwalls on the downstream end of the culvert and CIP concrete walls connected to the existing channel walls and culvert on the upstream end of the culvert. The project proposes to directly affect a total of approximately 60 linear feet of stream channel.

The applicant proposes to operate and maintain a concrete arch bridge having a three clear spans of approximately 89 feet each and having an underclearance of approximately 21 feet on a 90 degree skew across Tionesta Creek (CWF, perennial) on (T-394) Elm Street approximately 0.8 mile southeast of the intersection of SR 36 and U. S. Route 62 in Tionesta Borough and Tionesta Township, Forest County. The proposed maintenance project involves: (1) construction of a 0.5-foot facing on the piers and abutments up to the underside of the arches; (2) repairs to concrete on the underside of the arches and spandrel walls; and (3) placement of R-6 riprap at the upstream end of Pier No. 2 to fill a scour hole.

The proposed project is to construct approximately 6.5 miles of new four lane limited access highway on a new alignment in Tioga and Lawrence Townships and Lawrenceville Borough, Tioga County. The new highway will be part of the U. S. Route 15 transportation improvement project and will continue into Steuben County, NY. The Commonwealth portion of the project will extend from approximately the intersection of existing Route 15 and SR 287 to the New York state line. A total of 11.09 acres of wetlands are proposed to be impacted by the project (9.41 acres PEM, 0.65 acre PSS, 1.02 acre PFO and 0.01 acre POW). In addition, the project will impact the following streams: Bentley Creek, Tioga River, Cowanesque River and 13 unnamed tributaries to the Tioga River. All potentially impacted streams have a Chapter 93 water use protection designation of WWF. Stream and wetland mitigation will be required.

D51-002. Fairmont Park Commission, Memorial Hall Park West, P. O. Box 21601, Philadelphia, PA 19131. Project proposes to modify an existing fish passage facility at the Fairmont Dam, City of Philadelphia, Philadelphia County across the Schuylkill River (WWF, MF) to increase efficiency of the existing structure. The exit channel and gate will be reconstructed to alleviate one of the primary problems, which is the accumulation of trash and debris at the exit of the ladder. A narrow section of the old entrance weir will also be removed and a new weir gate to control entrance flow will be constructed. Existing current baffles have a 12-inch door opening and have an average pool drop of 12 inches. The baffles will be modified to 18-inch minimum door openings with a pool drop of 9 inches to meet current standards. A 5-foot nonoverflow section on the crest of the dam will also be constructed to prevent water from the spillway from competing with water from the fish ladder entrance (Philadelphia, PA-NJ, N: 16.85 inches; W: 8.95 inches).
