Citation T-20693 has been issued to you by the Enforcement Department of the Taxicab and Limousine Division of the Philadelphia Parking Authority citing a violation of 52 Pa. Code § 1021.12, ''Driver providing service without a valid driver's license'' for your actions on February 26, 2016. Two (2) separate mailings of this citation to your address of record have been returned by the Postal Service marked ''Return to Sender, attempted, not known, unable to forward.'' You have 15 days to answer this citation from date of publication by either pleading Liable, or by pleading Not Liable, and notifying the Philadelphia Parking Authority, Taxicab and Limousine Division, 2415 South Swanson Street, Philadelphia, PA 19148-4113, attention TLD Clerk.
