 Nicholas F. Breen, DDS, Pennsylvania license No. DS026746L, last known of Pennsauken, NJ, was denied his request for reinstatement of dental license, based on failure to appear for hearing and satisfy his burden of proof.

 Individuals may obtain a copy of the adjudication by writing to Kerry E. Maloney, Board Counsel, State Board of Dentistry, P.O. Box 69523, Harrisburg, PA 17106-9523.

 This order represents the final State Board of Dentistry (Board) decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of their petition for review. The Board contact for receiving service of appeals is the previously-named Board counsel.
