STATE CONTRACTS INFORMATION
DEPARTMENT OF GENERAL SERVICES
   Notices of invitations for bids and requests for proposals on State contracts for services and commodities for which the bid amount is reasonably expected to be over $10,000, are published in the State Contracts Information Section of the Pennsylvania Bulletin prior to bid opening date. Information in this publication is intended only as notification to its subscribers of available bidding and contracting opportunities, and is furnished through the Department of General Services, Vendor Information and Support Division. No action can be taken by any subscriber or any other person, and the Commonwealth of Pennsylvania is not liable to any subscriber or any other person, for any damages or any other costs incurred in connection with the utilization of, or any other reliance upon, any information in the State Contracts Information Section of the Pennsylvania Bulletin.    EFFECTIVE JULY 1, 1985, A VENDOR'S FEDERAL IDENTIFICATION NUMBER (NUMBER ASSIGNED WHEN FILING INCOME TAX DOCUMENTS) OR SOCIAL SECURITY NUMBER IF VENDOR IS AN INDIVIDUAL, MUST BE ON ALL CONTRACTS, DOCUMENTS AND INVOICES SUBMITTED TO THE COMMONWEALTH.

   Act 266 of 1982 provides for the payment of interest penalties on certain invoices of "qualified small business concerns". The penalties apply to invoices for goods or services when payments are not made by the required payment date or within a 15 day grace period thereafter.
   Act 1984-196 redefined a "qualified small business concern" as any independently owned and operated, for-profit business concern employing 100 or fewer employees. See 4 Pa.Code § 2.32. The business must include the following statement on every invoice submitted to the Commonwealth: "(name of business) is a qualified small business concern as defined in 4 Pa. Code 2.32."
   A business is eligible for payments when the required payment is the latest of:
      The payment date specified in the contract.
      30 days after the later of the receipt of a proper invoice or receipt of goods or services.
      The net payment date stated on the business' invoice.
   A 15-day grace period after the required payment date is provided to the Commonwealth by the Act.

081-98-7000-41 Conduct a Statewide survey of student drug use in Pennsylvania similar to assessments conducted in 1989, 1991, 1993, 1995 and 1997. The 1999 survey will be administered to approximately 60,000 students in grades 6, 7, 9 and 12 during the 1998--99 school year. Participating schools and students must be selected to form a representative Statewide sample. The survey must collect information on: students' use of alcohol, tobacco and other drugs; behaviors and attitudes concerning factors relating to substance use; knowledge about risks of substance use; physical fighting and weapons carrying; the presence of gangs in school; risks associated with vehicles such as driving under the influence; and other information deemed important. Survey must be confidential and anonymous to protect the identity of the respondents, and be capable of being completed in one standard classroom period. Contractor will be responsible for: constructing the Statewide sample and recruiting schools to participate in the survey; furnishing an adequate supply of surveys and instructions to participating schools and arranging for the return of the completed surveys; analyzing the survey results and producing a report of the findings similar in format to the 1997 report; and providing participating schools with a basic report of the survey findings for their students.
