THE CLEAN STREAMS LAW AND THE FEDERAL CLEAN WATER ACT--NPDES AND WQM PART II PERMITS

INDUSTRIAL WASTE AND SEWERAGE WASTEWATER

   The Department of Environmental Protection (Department) has taken the following actions on previously received permit applications and requests for plan approval.

   Persons aggrieved by these actions may appeal, under section 4 of the Environmental Hearing Board Act (35 P. S. § 7514) and 2 Pa.C.S. §§ 501--508 and 701--704 (relating to the Administrative Agency Law), to the Environmental Hearing Board, Second Floor, Rachel Carson State Office Building, 400 Market Street, P. O. Box 8457, Harrisburg, PA 17105-8457, (717) 787-3483. TDD users may contact the Environmental Hearing Board (Board) through the Pennsylvania Relay Service, (800) 654-5984. Appeals must be filed with the Board within 30 days of publication of this notice in the Pennsylvania Bulletin, unless the appropriate statute provides a different time period. Copies of the appeal form and the Board's rules of practice and procedure may be obtained from the Board. The appeal form and the Board's rules of practice and procedure are also available in Braille or on audiotape from the Secretary to the Board at (717) 787-3483. This paragraph does not, in and of itself, create any right of appeal beyond that permitted by applicable statutes and decision law.

   NPDES Permit No. PA0035653, Sewage, Pennsylvania Department of Transportation, Bureau of Design, P. O. Box 3060, Harrisburg, PA 17105-3060. This proposed facility is located in Brush Creek Township, Fulton County.

   Description of Proposed Action/Activity: Authorization to discharge to UNT of Brush Creek in Watershed 11-C for Safety Rest Area Site No. 3.

   NPDES Permit No. PA0021806, Sewage, Township of Annville, P. O. Box 320, Annville, PA 17003. This proposed facility is located in Annville Township, Lebanon County.

   Description of Proposed Action/Activity: Authorization to discharge to Quittapahilla Creek in Watershed 7-D.

   NPDES Permit No. PA0020648, Sewage, Port Royal Municipal Authority, Box 2236, Port Royal, PA 17082-0236. This proposed facility is located in Port Royal Borough and Milford Township, Juniata County.

   Description of Proposed Action/Activity: Authorization to discharge to the Juniata River in Watershed 12-A.

   NPDES Permit No. PA0088102 and 6701402, Sewage, Lynwood J. Gray, Jr., Lynwood Corporation, 236 Ebb Point Lane, Annapolis, MD 21401. This proposed facility is located in Codorus Township, York County.

   Description of Proposed Action/Activity: Authorization for Gray Stone WWTP to discharge to UNT South Branch Codorus Creek in Watershed 7-H.

   WQM Permit No. 6701411, Sewerage, Thomas G. Hofler, 10469 Foust Road, Glen Rock, PA 17327. This proposed facility is located in Springfield Township, York County.

   Description of Proposed Action/Activity: Authorization for the construction/operation of Sewage Treatment Facilities.

   NPDES Permit No. PA0034886, Industrial Waste, Wolverine Tube--Small Tube Products Company, Inc., P. O. Box 1674, Altoona, PA 16603-1674. This proposed facility is located in Allegheny Township, Blair County.

   Description of Proposed Action/Activity: Authorization to discharge to the Beaverdam Branch in Watershed 11-A.

   Description of Proposed Action/Activity: Renewal of permit for industrial waste treatment discharge.

   WQM Permit No. 4196401-A2, Sewage, Lycoming County Water and Sewer Authority, 216 Old Cement Road, Montoursville, PA 17754. This existing facility is located in Fairfield Township, Lycoming County.

   Description of Proposed Action/Activity: Amendment of permit for approval of pump station improvements and chemical addition facilities at the sewage treatment plant.

APPROVALS TO USE NPDES AND/OR OTHER GENERAL PERMITS

   The following parties have submitted: (1) Notices of Intent for Coverage under (1) General NPDES Permits to Discharge Wastewater into the Waters of the Commonwealth. The approval for coverage under these general NPDES permits is subject to applicable effluent limitations. Monitoring, reporting requirements and other conditions set forth in the general permit: (2) General Permits for Beneficial Use of Sewage Sludge or Residential Septage by Land Application in Pennsylvania; (3) General NPDES Permit Authorizing the Discharge of Stormwater Associated with Construction Activities to Waters of the Commonwealth; (4) Notification for First Use Application of Sewage Sludge.

   The approval of coverage for land application of sewage sludge or residential septage under these general permits is subject to pollutant limitations, pathogen and vector attraction reduction requirements, operational standards, general requirements, management practices and other conditions set forth in the respective permit. The Department of Environmental Protection approves the following coverage under the specific General Permit.

   The EPA Region III Administrator has waived the right to review or object to this permit action under the waiver provision 40 CFR 123.23(d).

   The application and related documents, effluent limitations, permitting requirements and other information are on file and may be inspected and arrangements made for copying at the contact office noted.
