 On September 26, 2018, Blythe Fatimah Crawley, LPN, license No. PN301543, last known of Conshohocken, Montgomery County, was ordered to pay a $1,000 civil penalty, complete 3 hours of continuing education and was suspended for at least 2 years, the first 3 months to be served as an active suspension, with the ability to stay the remaining 21 months to probation, based on having committed fraud or deceit in the practice of practical nursing or in securing her admission to the practice by failing to report having pled guilty to a crime of moral turpitude to the State Board of Nursing (Board).

 This order represents the final Board decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of their petition for review. The Board contact for receiving service of appeals is the previously-named Board counsel.
