Service of Reminder Notice of Citation Complaint

 Attention Oussama Moutki, last known address of 5011 Sansom Street, Philadelphia, PA 19139, this is a reminder notice that Citation Complaints T-28737, T-28738 and T-28739 (Citations) were issued against you by the Enforcement Department of the Philadelphia Parking Authority's (Authority) Taxicab and Limousine Division (TLD) via publication in the Pennsylvania Bulletin on October 12, 2019 (see 49 Pa.B. 6048), citing you have committed the following: in citation T-28737, a violation of 52 Pa. Code § 1017.24 for ''failure to properly activate and display the meter'' which carries a recommended penalty of $100; in citation T-23738, a violation of 52 Pa. Code § 1021.11 for ''rates charged other than tariff'' which carries a recommended penalty of $500; and in citation T-28739, a violation of 52 Pa. Code § 1021.11 for ''discourage, prevent or refuse use of a credit card'' which carries a recommended penalty of $1,000. Pursuant to 52 Pa. Code § 1005.13(b) (relating to answers to citations), an answer to the Citations was due no later than October 28, 2019.

 Respondents must either request a hearing on a citation or pay the penalty identified in a citation within fifteen (15) days of issuance. See 52 Pa. Code § 1005.13(b). If you fail to take immediate action by either requesting a hearing or paying the penalty amounts identified above, it may result in the entry of a default order as to the Citations and the penalties; along with imposing any suspension or revocation that may be identified in the Citations.

 Please take the necessary actions to satisfy the Citations immediately. Payments made payable to the Philadelphia Parking Authority or requests for a hearing may be submitted to:
