 Attention Oussama Moutki, last known address of 5011 Sansom Street, Philadelphia, PA 19139, this notice advises that a Default Order of Hearing Officer, Hon. Sheldon C. Jelin was issued and entered on February 9, 2021, with the Philadelphia Parking Authority's (Authority) Taxicab and Limousine Division's (TLD) Office of the Clerk.

 On August 30, 2019, the Enforcement Department of the TLD of the Authority issued the following citations to Oussama Moutki (Respondent): Citation T-28737 for a violation of 52 Pa. Code § 1017.24, failure to properly activate and display meter; Citation T-28738 for a violation of 52 Pa. Code § 1021.11, rates charged other than tariff; and Citation T-28739 for a violation of 52 Pa. Code § 1021.11, discourage, prevent, refuse use of a credit card.

 The complaints were served by proof-of-mail on August 30, 2019, but were returned to the TLD marked as ''return to sender, attempted, moved, unable to forward.''

 Service of the complaints was effectuated on October 12, 2019, via publication in the Pennsylvania Bulletin pursuant to 52 Pa. Code § 1001.51(e) (relating to service by the Authority). See 49 Pa.B. 6048.

 Respondent did not pay the penalty amount identified on each citation or file a request for a hearing with the TLD Clerk within 15 days after the date of service as provided in 52 Pa. Code § 1005.13(b) (relating to answer to citations).

 On November 9, 2019, a notice was published in the Pennsylvania Bulletin to remind Respondent of the outstanding citations; to either pay the penalties, request a hearing, or suffer a default. See 49 Pa.B. 6792.

 To date, more than 15 days have expired and Respondent has failed to pay the penalties or file a request for a hearing with the TLD Clerk. Therefore, pursuant to 52 Pa. Code 1005.13(c) the complaint is sustained and a Default Order is entered against Respondent.

 A penalty of $100 is imposed on Citation T-28737, a penalty of $500 is imposed on Citation T-28738, and a penalty of $1,000 is imposed on Citation T-28739 along with an administration fee of $75 for a total of $1675.

 Respondent is Ordered to pay $1,675 by February 24, 2021.

 Pursuant to 52 Pa. Code § 1001.13(b) this Order will become effective on February 24, 2021 as an adjudication of the Authority if not selected for review by the Authority.

 Respondent is hereby notified to cease and desist from further violations of the Taxicab and Limousine regulations, 52 Pa. Code, §§ 1001.1 et seq.
