APPLICATION PROCEDURES


§ 9.313. Certification period.

 A certification as a coordinated care organization will be valid for 2 years from the date of its issue, unless the certification is earlier suspended or revoked by the Department for reasons specified in section 306(f.2)(4) of the Workers’ Compensation Act (77 P. S. §  531.1(f.2)(7)).

FEES


§ 9.331. Application fees.

 The schedule of fees charged for applications relating to coordinated care organizations (CCOs) is as follows:

   Application for certification as a CCO … $1,500

   Application for expansion of CCO service area … $500
