§ 41.11. Title of document.
(a) Legal documents in a provider appeal commenced by a request for hearing, other than the initial pleading, shall display a caption at the top of the first page in the following form:
COMMONWEALTH OF PENNSYLVANIA
DEPARTMENT OF HUMAN SERVICES
BUREAU OF HEARINGS AND APPEALS
[Name of Provider] v. [Name of Program Office]
BHA I.D. No.:
Docket No.:
[Descriptive Title of Document](b) The descriptive title of a legal document must identify the party on whose behalf the filing is made.
(c) Subsections (a) and (b) supersede 1 Pa. Code § 33.1 (relating to title).
