§ 5200.33. Discharge.

 (a)  A psychiatric outpatient clinic shall complete a discharge summary for each individual at least 45 days before discharge from services that includes the following:

   (1)  Summary of services provided and outcomes.

   (2)  Reason for discharge.

   (3)  Referral or recommendation for other services if needed.

 (b)  The psychiatric outpatient clinic shall provide the following information to individuals at discharge:

   (1)  Contact information for the local crisis intervention service.

   (2)  Contact information for any referrals.

   The provisions of this §  5200.33 adopted under sections 201(2) and 1021 of the Human Services Code (62 P.S. § §  201(2) and 1021); sections 105 and 112 of the Mental Health Procedures Act (50 P.S. § §  7105 and 7112); section 201(2) of the Mental Health and Intellectual Disability Act of 1966 (50 P.S. §  4201(2)); and section 4 of the Outpatient Psychiatric Oversight Act (Act of May 31, 2018) (P.L. 123, No. 25).
