Rule 15.11. Notice of Right to File Statement of Medical, Personal, or Social History Information.
The clerk, in accordance with Rule 4.6, shall transmit the decree of termination to the parent whose rights are terminated or to counsel for that parent, if represented. In that mailing, the clerk shall include a reference to information and instructions for the parent to file with the clerk and with the Department medical, personal, or social history information and to update the information filed, whether or not the medical condition is in existence or discoverable at the time of adoption. The clerk shall also include a reference to information and instructions to redact the birth parents name from the childs original birth certificate.
Explanatory Comment The clerk may include in the mailing to the birth parent or birth parents counsel any forms promulgated by the Department, any forms promulgated by the Department of Health, and any forms adopted by local rule, including forms authorizing the release of information, withholding authorization to release information, or revoking any prior authorization to release information.
